Bacteria are Most Common Cause of Shoe Odor: NY Podiatrist
 
One of the best-kept secrets that podiatrists know about promoting foot health and extending the longevity of your shoes is to invest in a shoe deodorizer. This type of product is designed to kill bacteria, fungus, and viruses that can grow in moist environments, especially in shoes that we work in or wear for long periods of time, says Nelya Lobkova, DPM, a New York City-based podiatrist at Step Up Footcare.

"Upon swab testing, the bacteria found in shoes with bad odors was Staphylococcus aureus (S. aureus) and S. epidermidis, which can come off of the skin and onto the shoes," Dr. Lobkova says. "To effectively kill this bacteria, you would need to heat or vacuum dry the shoes, which shoe deodorizers cannot provide, but they can decrease the bacteria from flourishing within the shoe in the first place by creating a drier environment."

After caring for the West Seattle community for over 33 years, Michael Vickers, DPM of West Seattle Foot & Ankle Clinic is retiring on October 28. Ryan Schwanke, DPM, a West Seattle resident and an experienced podiatrist and podiatric surgeon, will begin seeing patients at the clinic on October 31.

25% of Clinicians Want Out of Healthcare: Survey
 
One quarter of U.S. physicians, advanced practice providers, and nurses are considering switching careers and one third are considering switching employers, according to newly released results from a survey conducted by Bain & Company. Below are some key takeaways from the survey and brief, which was released Oct. 11 and can be found in full here.
 
1. Of the 25 percent of clinicians who are thinking about exiting healthcare entirely, 89 percent cite burnout as the main driver.
2. The top three things clinicians care about most in their profession are compensation, quality of patient care, and workload, according to the survey. Of those three, they are least satisfied with compensation (59 percent expressed satisfaction) and workload (60 percent). Eighty percent said they are satisfied with the quality of patient care. 
3. Burnout shows up throughout clinicians' days, with 63 percent saying they feel worn out at the end of the workday, 51 percent saying they feel they don't have time and energy for family and friends during leisure time, and 38 percent feeling exhausted in the morning at the thought of another workday.

A recent anonymously posted letter on PM News really got me thinking and it actually played out during a recent visit to my health club. Two Medicare beneficiaries were discussing a recent NY Times article, which had been published earlier that day. One beneficiary commented that he couldn’t believe that insurance companies were actually paying doctors to put more diagnoses in their charts.
 
I offered the correction that it was actually the insurance companies and their data mining companies which were defrauding the government and threatening the Medicare Trust Fund. I would like to opine from a concierge, no insurance accepted point of view. I hope to enlighten and encourage those podiatrists who feel they have the skill and knowledge to opt out of insurance. I have been in practice for 37 years and completed a 3-year surgical residency when the typical was one year. I feel I offer excellent services and care for which I charge upfront.
 
I charge $4,000 for a standard Austin bunionectomy; for the Weil, plantar plate, and hammertoe, I charge $5,000, since it is a secondary procedure that I combine/bundle as second ray surgery. I charge 50% for the second procedure, $2,500 for a total of $6,500. This includes all post-op care, x-rays, and all supplies dispensed by me. It is a one and done fee inclusive until they do not need my services from the surgery anymore. I hope this information inspires some of my colleagues to follow suit; we deserve to be compensated for our excellent work.

Thank you, Vince Marino, DPM for a very compelling argument in your well drafted post as well as comments from Dr. Block that preceded  your post. It's time for podiatry to get back to its grass roots, decrease the "over emphasis" on surgical procedures, and increase emphasis on biomechanics and diagnosis of medical conditions, etc. In the past, I ran a general practice emphasizing wound healing and directed a surgical residency program that had as many as 35 internal medicine residents rotating through it yearly. I was fortunate enough to run a private practice while teaching at TUSPM, including at the clinic. During those years, surgery paid substantially more than it does now, with decreased costs. Even during those times when surgery was more lucrative, the biomechanics therapy of my practice was a very important and consistent aspect.
 
I used basic Root theory for my biomechanical examinations, (approximately $200 for 30 minutes) with a five-minute consultation at the end of the exam, explaining the results. Orthotics were charged separately, I believe $500- $600 at that time. It requires much experience to analyze the nuances of foot pathology, especially in those with complicating diagnoses. Today as I continue to be involved with wound healing, these principles are still core to my therapeutic approach.
 
I encourage all the young practitioners to more carefully look at the subspecialty. Embrace it, read more about it, and go to courses that can teach you more about it. You can develop a very successful practice with minimal liability, very successful therapeutic results, and happy patients who will continue to refer more patients to you.

I have appreciated the discussion on the costs of hospital surgery vs clinical outpatient care and certainly agree that income per hour is much higher in the office vs the OR. As was pointed out, some of this is because of the fees. Some of it is also because of the inefficiencies of OR  surgeries. This can be improved by using surgericenters and scheduling back-to-back procedures. However, the hourly reimbursement rate is also improved by performing many procedures such as exostectomies or hammer digit correction in your office.
 
Even if you cannot meet the criteria for a surgericenter, it is not difficult to dedicate a room to sterile surgery. Infection rates are generally lower in this environment than in hospital ORs. Many patients are willing to have procedures done with ...
