INTERNATIONAL PODIATRISTS IN THE NEWS
Shortage of Irish Podiatrists Impacts Diabetic Foot Care

Between April 2021 and May last year, Diabetes Ireland delivered and evaluated an in-depth education and support program on foot care for newly diagnosed diabetes patients. The risk the condition poses for lower limb health is stark. According to figures obtained by Social Democrats TD Róisín Shortall via a parliamentary question, 3,473 cases of foot ulcers associated with diabetes were admitted to hospital last year. Of those, 671 cases led to an amputation of the lower limb.

Louise McHugh

According to Louise McHugh, senior podiatrist at the Diabetes Ireland care centre in Dublin, spreading awareness of the need for patients to look after their feet is crucial. “Every healthcare professional – anyone who is coming into contact with a patient who is newly diagnosed – has a responsibility to sit down and explain to them what to look out for,” she told IMT. Diabetes Ireland runs two care centres, in Dublin and Cork, with two podiatrists working in each. The demand for the service is high, with more than 5,000 patients travelling from across the country to attend the clinics for annual assessments, or more targeted care.

RESPONSES/COMMENTS (CLINICAL)
RE: Proper Blade for Debridement of Calluses (Ivar E. Roth DPM, MPH)
From: Stephen Peslar, BSc, DCh

I concur with Dr. Roth. Careful scalpel technique is superior to using a dermal curette. 40 years ago, I learned how to enucleate IPKs. I use a Miltex #3 handle and a Swann-Morton #15 blade. For calluses, I use a #10 Swann-Morton blade. From time to time, I do get bleeding points; they are treated with a topical antibiotic and an appropriate sterile dressing.

RESPONSES/COMMENTS (NON-CLINICAL)
From: David Secord, DPM

When I converted to paperless, I hired a place which used a Banctec scanner. They have machines which can do 550+ pages a minute with duplex and color dropout (for scanning those marigold-colored receipts.) We did the entire office in a weekend. I used a UMAX A3 flatbed scanner I owned with a transparency adapter to scan the films on my own and then called a silver reclamation company to take the physical films. You may find that the average desktop scanner is not made for the volume you're looking at.

RE: The Passing of Paul Shromoff, DPM

It is with a heavy heart that I report the death of Dr. Paul Shromoff.

RESPONSES/COMMENTS (OBITUARIES) - PART 2 A
From: Harvey Lemont, DPM

I had the privilege to work with Gary throughout our years together at PCPM and Temple. Gary was the “go to doc” to send the worst diabetic cases with extensive infections, performing the slow tedious work requiring multiple plastic procedures extending over months, with excision of underlying infected bone and skin grafting. Gary had a reputation for taking time with all his patients, which ended in a waiting room filled to the gills. However, once his patients were in his chair and finally seen by him, they were all truly grateful for his meticulous singular focus to their complaint.

He was also very generous with the sharing of his knowledge to students, residents, and fellow physicians. He was a true Drs.’ doctor and a personal friend. His absence will be felt throughout the country. He was a true pioneer in limb salvage.

Harvey Lemont, DPM, Professor Emeritus, Temple University School of Podiatric Medicine

RESPONSES/COMMENTS (OBITUARIES) - PART 2 B
From: Greg Caringi, DPM

I first met Gary at OCPM when he was asked, as a 3rd year student, to lecture on the fine details of tendon anatomy and function, to the 1st year students. He was even then a master anatomist. Starting in 1989, I had the opportunity to work with Gary at PCPM/TUSPM in the lab for his class in surgical anatomy (aka cadaver lab) for 10 years until 1999. I will never forget those days and all that I learned from Gary. We became good friends. I understood his "unusual sense of humor".

Behind the scenes, he admitted to me that he was aware of his sometimes-odd mannerisms and creative language - "Cobanize and amalgamate the dressing" - because he wanted the students to pay attention and remember what he was saying on a difficult, but important subject. I will admit that Gary cased me to rethink podiatry in terms of anatomy. I believe it made me a better diagnostician and...

RESPONSES/COMMENTS (OBITUARIES) - PART 2 C
From: Steven Selby Blanken, DPM

I was from the PCPM class of 1990. One of the reasons I selected this school was for their faculty, especially the Surgery Department which consisted of Drs. Bauer, Vogler, Downey, Malay, and others. They were all famous and fun to be with. I had the pleasure of being with Dr. Bauer on a Saturday morning on the day of his own wedding. We were near the end of the clinic hours and his colleagues in the clinic wanted to play a practical joke on him for working on the same day of his wedding.

I was the student they used to play the joke on Gary. I told him there was a patient of his in a room who needs immediate emergency surgery. I told him he needs to come to the clinic room to see the patient immediately. He denied that there were any more patients in the clinic and he had to go to his own wedding. I told him he was the last one left in the clinic and I was just a...

RESPONSES/COMMENTS (PM NEW QUICK POLLS)
From: Chuck Langman, DPM

So I’m curious. I’m on the board of an opioid foundation and am wondering why a significant percentage of docs would say they are against residents learning about pain management and opioid abuse.
