CA Podiatrist Gets Stranded on Freeway with New Tesla Model 3

Dana Brems, DPM used to drive a Toyota Prius in college, but she always wanted to have a Tesla. When she became a doctor, she bought one as a gift to herself. Autopilot was a feature Dr. Brems loved. On her third drive, her brand-new Model 3 decided to stop in the left lane of the I-5 highway right after a blind corner. Luckily, nothing worse happened, but the mere possibility made her want to return her car.

Tesla does not want it back and blames her for everything. According to the Tesla Service Center, Dr. Brems was to blame because the vehicle failed for lack of charge – despite still having 29 miles of range when they analyzed the EV. The doctor tried to argue that it had 140 miles and about 40% of charge left. She showed them videos and pictures that proved that.

Aussie Podiatrist Discusses the Importance of Wearing the Right Running Socks
 
“The right socks are really important,” explains Gold Coast podiatrist Sarah Sweeney, who runs her own namesake clinic. “Even more so for people [who are] running... due to the increase in load going through your feet and the amount of sweat your feet produce.” And that’s not all – poor sock choices can put your feet at risk of other issues. “The wrong socks can cause blisters, as they’re not stopping excess friction,” Sweeney tells Body+Soul. “A sock that’s too tight can contribute to leg cramps, cut off circulation, or cause a wound.

If socks aren’t made of the right materials [such as moisture-wicking bamboo or polyester], they can increase the risk of developing tinea, dermatitis, and other skin/fungal problems and cause the feet to smell. If socks are too thin, blisters may occur. If socks are too thick, shoes can become tight, causing corns, ingrown toenails, and pain.” A balance between cushioning and comfort is key, adds Sweeney, pointing out that a more cushioned sock will be thicker and make shoes feel tighter, so you’ll need to compromise on less padding to avoid “pain, swelling, and injury to the toenails.

Organ Donor Plans to Climb the Summits in All 50 States
 
Ben Pearl, DPM interviewed David Ashley, who accomplished climbing the 7 major summits, after a kidney donation to a West Point classmate. He will attempt climbing the highest peaks in all 50 states. He recently made a second organ donation, a lobe of his liver. The liver lobe is able to regenerate.

Ashley prefers wide toe box hiking boots to help avoid blisters and nail problems. He prefers shoes that are made of lighter construction, particularly in the approach to the base camps on the mountains. He has been able to continue his passion for adventure treks and hopes that his donations will inspire others to consider organ donation.

  • The quicker a physician meets with a lawyer, the better, Benvenuto noted. 
  • Benvenuto said that the right lawyer will get back to the physician right away. "If you find an attorney who won't take your call, get another attorney."

Query: Noridian NorCal Medicare and CPT 11721
 
Lately, we have been getting a denial from Noridian NorCal Medicare for debridement of nails (CPT 11721) for patients that have diabetes with onychomycosis and peripheral angiopathy. We have been using the diagnosis codes B35.1 and E11.51. As of July, our billings are being denied as “not medically necessary.” But prior to July, they were always paid. Does anybody have any insight into this?

Response: There have been some updates to the RFC policy from Noridian. I'm not sure why this claim was denied based upon those ICD-10 codes. Did you use the appropriate Q 7-8-9 code? Was anything else billed at the same time?
 
The best way to resolve this is to send a claim to the CPMA for review to better determine the cause. But first, call the provider line and see what they tell you. Get the reference number of the call. That is important. If you are still not satisfied, then contact CPMA and they will route it to their Medicare committee. We need the reference number to intervene on your behalf.

Irrespective of our fee-for-service contracts and taking into account all of the services that podiatrists can provide, we definitely make more money per minute staying in our offices and going from room to room seeing patients. As we all know, some procedures and services seem to pay less than we feel is appropriate for the time and skill and effort involved (and the aftercare); and also procedures done in the hospital or at surgery centers pay us less than if  the same surgery were done in our office setting. If your career is only about the money and efficiency, then never leave your office to do a house call, never drop off a sample to a patient at home, and never do a consult or treat a foot ulcer in the hospital.
 
However, the beauty of our profession is that each doctor can decide what is best for them. Personally, I have found that after 40 years of private practice, the "outside of the office" surgeries that I still do, are both enjoyable and rewarding, both to myself and to my patients. Even if my overall "dollar-per-minute" remuneration would be increased by focusing my attention to only in-office care or procedures, it is kind of relaxing to be in the operating room with the patient not talking, getting the job done, along with teaching our surgical residents (our profession's future).
