A Good Boot Can Act Like an AFO: GA Podiatrist
"Boots are interesting because there's a device we use called an ankle foot orthotic (AFO) which basically stables the ankle. A boot is essentially an AFO if it's a good boot," says Aaron Solomon, DPM of Village Podiatry Centers.
|
|
Dr. Aaron Solomon |
"We want to have a modest heel, one and a half to two inches at the most. We don't want a boot with an elevated heel or too pointy a toe because we can get into forefoot problems such as bunions and hammertoes," says Dr. Solomon.

CA Podiatrist Retires to Become Comedy Writer
Ceres CA is losing its only foot doctor on Nov. 1, but it may be gaining its newest comedy writer in the process. Dr. William Kalanta’s decision to retire allows him to pursue writing humor. “People ask me what I’m going to do after I retire,” said Kalanta. “After 40 years of being a foot doctor, I figured out how a foot doctor and comedian pretty much do the same thing all day long – we’re just pulling your leg.”

Dr. William Kalanta and his book Just Pulling Your Leg |
When asked how he chose podiatry, humor creeps into his answer: “When I was in medical school I didn’t know what to specialize in so I figured I’d start at the bottom and work my way up.” Besides, he said, he figured, “I’ll be a foot doctor and everybody would try to get their foot in my door.”

Aussie Podiatrist Promotes Foot Health Week
Foot Health Week is from October 14-20 and aims to promote general foot health and encourage better health outcomes for all Australians. Ricky Lee is a registered podiatrist and a director with the Australian Podiatry Association.

Ricky Lee |
"As podiatrists, we manage and treat all concerns of the lower limb, from assessing children as they start walking and developing, to sports people with knee or foot injuries who are training for their next marathon, to diabetic foot management and general foot care such as ingrown toenails, corns, and calluses," he said.

Healthcare Trends Impacting Physician Practices - Part 4
Physicians are facing new challenges as they balance patient care with business priorities. Restrictive insurance rules, government regulations, and declining compensation are just a few of the obstacles that modern practices are up against. Despite these challenges, successful practices are identifying solutions to these business pressures. Here are five trends shaping the healthcare industry. As we approach 2020, administrators should consider how these trends may impact, and potentially improve, their practices’ outlook.
4. Competition for skilled workers - Physician practices looking to hire non-physician providers are increasingly competing for candidates who have a wide range of job options. The strong demand has increased the number of physician assistants by 40 percent from 2010 to 2017 and has propelled nurse practitioners to No. 3 in the U.S. News and World Report’s Best Jobs Rankings for 2018.

RE: Re-evaluation of Traditional Angular Parameters for Austin Bunionectomies
From: Eric Baskin, DPM
I have also had success with the minimally invasive Chevron Akin osteotomy (MICA). The technology of the chamfered screw has dramatically changed my pre-operative considerations/parameters. I agree that piercing the far lateral cortex with the chamfered screw provides the stable construct for the correction.
|
|
Post and pre-op x-rays |
I also believe that healing of this aggressive correction can be attributed to minimal disruption of the periosteum, soft tissues, and joint capsule. As with other types of MIS bunionectomies currently being performed, joint range of motion is essentially unaffected because the osteotomies are extra-capsular. The case (above) presented is approximately 10 weeks post-operative.
Disclosure: I am a paid consultant for Wright Medical.

From: Leonard A. Levy, DPM, MPH
Alan Sherman, DPM, concluded that a single podiatric board is what is needed to “eliminate current confusion as to what a podiatrist is.” Allopathic and osteopathic physicians, prior to the middle of the 20th century, overwhelmingly were general practitioners. Now, virtually all these graduates enter residency training leading to qualification in a specific specialty. As a result, the numerous specialty boards have training requirements and examinations that assure the public as well as members of these professions that they are qualified in their specialty.
Podiatric medicine also evolved considerably in the last several decades, creating the specialties of podiatric surgery and, yes, podiatric medicine. Both are quite complex, requiring significant graduate medical education called residency training. Unfortunately, we still hear members of our profession refer to...

From: David M Kaufmann, DPM
I read with interest Dr. Kaufman’s (no relation) breast cancer experience. The positive outcome is certainly good news. Concurrently, I would like to point out that as many as 30% of successfully treated early stage breast cancers may resurface over the span of many years as stage 4 cancers.
The conclusion is: successful treatment in early stage breast cancer is salutary, but ongoing awareness and prevention is necessary. Very little resources are afforded Stage 4 breast cancer at present.

From: Jon Purdy, DPM
Here is yet another one of numerous examples of how single payer insurance can manipulate and hold a practitioner hostage. I discontinued the program because of across-the-board denials. As we know, the cost and burden of DME is placed on the dispenser. After purchasing 30 pairs of shoes and inserts, and all appeals denied, I had no choice but to file with the ALJ. Of course, there is no right to a speedy trial in the insurance world so I waited 3 YEARS! I did not lose one case and in some instances, the ALJ apologized to me, stating they didn’t understand why the claims were denied and that they were sorry I was put through that course of action.
If the government wishes to deny patients care, and doctors payment, just burden the provider with endless paperwork, repeated unsubstantiated denials, then make them wait three years for payment after their capital outlay.
Jon Purdy, DPM, New Iberia, LA
