NY Podiatrist’s Organization Donates 180 Pairs of Back-to-School Shoes 
 
Shoes For All (SFA), the not-for-profit organization founded by Williston Park podiatrist Dr. Mary Carlson, donates new and gently worn shoes to those in need in the community. Recently, her organization donated new back-to-school sneakers to the entire student body at Martin DePorres School in Elmont. Every student at Martin DePorres School received a brand new pair of sneakers from SFA. This school educates challenged students in grades 1-12. 84% of this student population lives below poverty level, so these sneakers were truly needed.

IL Podiatrist Joins OrthoIllinois

OrthoIllinois has announced the addition of Nicholas Brissey, DPM to their Algonquin clinic team. Dr. Brissey graduated from the Rosalind Franklin University of Medicine and Science in North Chicago. After graduating, he pursued surgical training in orthoplastics along with advanced reconstruction of the foot and ankle at Saint Anthony Medical Center in Rockford, Illinois.

Prior to becoming a podiatrist, Dr. Brissey joined the Army in 2000 and worked in military intelligence as a Chinese language crypto-linguist. In 2002, while serving at the U.S. embassy in Beijing, he developed a foot condition that was successfully treated by a podiatrist. This experience piqued his interest in the field and ultimately led to a career in healthcare. He is multi-lingual, speaking English, Spanish, and Chinese.

The International Diabetic Foot Conference, better known as DFCon, celebrated its 18th year with speakers from the world’s top academic institutions as well as a renewed focus on the next generation of men and women working in limb preservation.

The symposium’s signature event, the Edward J. Olmos Award for Advocacy in Limb Preservation was given to Lawrence A. Lavery, DPM, MPH, Professor of Plastic and Reconstructive Surgery at University of Texas Southwestern. DFCon19 will return to Universal Studios, Hollywood, October 17-19, 2019.

U.S. Podiatrists Featured at Spanish Annual Meeting 
 
Three years ago, Luke Cicchinelli, DPM moved to Vigo, Spain to help elevate the training of Spanish podologos. This year, he invited several U.S. podiatrists including Drs. Amol Saxena, Richard Blake, Scot Malay, Michael Downey, Dennis Frisch, Tracey Vlahovic, and Thomas Zgonis to speak at their 49th annual national meeting, chaired by Borja Perez Serrano.

Negotiating Your Contracts: Hidden Pitfalls and Traps - Part 6
 
Negotiating and reviewing contracts are among the more unnerving tasks physicians face. That’s good! Your subconscious is sending you “danger” signals. You simply need to channel this emotion into action to protect yourself.
 
6. Keep bona fide purchasers in mind. In many cases, you, the simple purchaser, may be asked to sign more than just an SaaS contract. The vendor may produce a finance contract, which is a type of negotiable instrument, called a “promissory note.” Under the promissory note, you become not only a purchaser, but also a debtor, who promises to pay the payee the total obligation under the SaaS contract. This can have very negative consequences because you may be required to pay the note, even if the SaaS doesn’t work.
 
Even if the SaaS vendor is the payee, the note will usually be sold to a bank or finance company unrelated to the vendor. When that happens, the bank or finance company is considered a bona fide purchaser if it pays value to purchase the note without knowledge of any defenses or reasons why the note might not be owed, such as the product is defective. This means you still must pay the bona fide purchaser note even if the product failed, the same as if you had paid cash up front. Your only recourse is against the vendor.

Query: Wound Debridement Combination Coding
 
Can you bill CPT 11042 and CPT 97597 together if they are for different wounds? If you have an ulcer on the left foot at one depth and another ulcer on the right foot at a different depth….can you bill CPT 11042 for the left and CPT 11042 for the right? Is CPT 11042 based on size per wound or total wounds size if different wounds?

Response: Yes, you can. The payments for wound debridements are indeed based on overall size debrided, but also on the depth and type of the wound. That said, we may encounter wounds that are at different depths and locations so those can be billed separately, uniquely.  Example: 10 sq cm of muscle depth ulcer debrided from the left foot and 15 sq cm of skin depth ulcer debrided from the right foot, separate wounds.
 
CPT 11043 1 unit
CPT 97957-59 1 unit
 
The CCI allows these to be debrided together but requires the -59 appended to the column 2 code, CPT 97597.

RE: Requests to Trim Fingernails

Trimming or debridement of fingernails might be interpreted as part of good podiatric care concerning a comprehensive podiatric exam in conjunction with podiatric treatment. The purpose  of such an exam is to ultimately treat the foot so that its condition remains as healthy as possible.
 
Look how far we came. In NYS, for example, podiatrists can cut above the malleolus to perform epidermal nerve fiber density (EFND) testing so long as the purpose of the exam is to treat a foot condition. The law says that we cannot cut above the malleolus, but I believe this refers to...

Dr Kass raises a good point. Why shouldn't podiatrists be allowed to cut fingernails when manicurists with no college education are permitted to do so? I looked up what a manicurist must do to be licensed in my state. He or she must take a 250-hour course and then pass an exam. There are also separate licensing fees as there are with any other profession. 
 
The solution to those who feel impelled to cut someone’s fingernails in a state where it is out of scope for podiatry can choose to take such a course at one of the cosmetology schools, pass the exam, pay the licensing fees, and then if a patient asks you to cut his or her fingernails, you will be able to do it  without worrying about violating the law.

I think the discussion about our cutting fingernails has gotten a little out of hand. I can’t provide any legal advice because I’m not a lawyer, but the following is my personal common sense opinion. I believe it goes beyond the question of “Should podiatrists be allowed to trim fingernails?” The issues seem to be whether our license covers it, whether we can be sued if we cause some damage, and whether our professional liability insurance would cover us. 
 
There’s a difference between our trimming fingernails as part of our professional license vs. trimming fingernails as a lay-person, which anybody is “allowed” to do. I can give someone a...

Dr. Bass, in simple terms, was a mensch. As a Kern Hospital resident, he taught us all the importance of knowledge, professionalism, and humility. He always took his time to simply listen, a trait that many of us fail to do with our patients and even our friends and family. He taught us how to relate to patients and colleagues. And yes, he taught us how to enjoy our lives. Dr. Bass, you are already sorely missed and many of us who were lucky to know him have a big hole in our hearts this week.

Diabetic Limb Salvage Fellowship

Two positions for a 12-month Diabetic Limb Salvage fellowship beginning July 1, 2019. The UCSF Limb Preservation Fellowship educates the podiatric physician as a clinically skilled podiatrist in a multidisciplinary approach to the diabetic and vascular complications of lower extremities, as well as generating new knowledge in the treatment, management, and prevention of lower extremity amputations.
