MN Podiatrist Discusses "Marathon Foot Syndrome
“A marathon is about as hard an activity on your feet as you could possibly do,” said Dr. Andrew Lundquist, a podiatrist at Mankato Clinic. Stress fractures are the big concern leading up to the race. Lundquist, who has run eight marathons, said people sidelined with stress fractures are easy to spot at a marathon. “At almost every one, you’ll see people in walking boots standing and watching with a gloomy look on their faces,” he said.

Those who do finish the marathon can experience what Lundquist calls “marathon foot syndrome” — basically an extremely sore foot. He’ll see a few patients come in each year following the marathon complaining of this issue.Surprise, the best way to avoid this dreaded foot condition isn’t properly hydrating — although it doesn’t hurt. “That can be alleviated by a cool down,” Lundquist said. “Rather than sitting, walk it off and then ice the feet.”

Wife of IA Podiatrist Speaks at Alzheimer's Disease Event
More than 1,000 people showed up to support finding a cure for Alzheimer's disease, the sixth-leading cause of death in the United States, and the only one in the top ten without a cure. One of those in attendance was Tina Maikon, whose husband Dr. Mark Maikon was diagnosed with a visual variant of Alzheimer's (posterior cortical atrophy - PCA) four years ago.

Like many forms of the deadly disease, PCA's cause is not known. Just four years after his diagnosis, Mark, a podiatrist for two decades, is a wheelchair user. Tina spoke at the 2016 Walk held at the McGrath Amphitheatre, expressing her family's struggles as the disease consumes her husband.

| SUCCESS TIPS FROM THE MASTERS | |
Bret Ribotsky: How are you applying podiatric biomechanics to advanced surgical procedures such as ankle fusions?
 |
|
Dr. Harold Vogler |
Harold Vogler: Once you alter the biomechanics of the musculoskeletal system in the lower extremity, you do have to make accommodation so that they (patients) can have a comfortable gait pattern. There are a variety of ways of doing this. Orthotics have made great advances and strides. There is technology available for orthotics that did not exist 10, 20, or 30 years ago that can improve weight-bearing after these procedures, and make orthotics more functional. So you should very much consider using the skills of a good orthotist after you do these major reconstructive procedures to make the extremity more functional.

|
Dr. John Mattiacci |
Meet the Masters is broadcast each Tuesday Night at 9 PM (EST). The next segment will feature Dr. John Mattiacci, Dean of the Temple University School of Podiatric Medicine. You can register for this event by clicking here

| RELEVANT RESEARCH | |
Close Casting Method May Help Elderly Avoid Ankle Surgery
Close contact casting, a molded below-knee cast with minimal padding, may be a better treatment than surgery for unstable ankle fractures in older adults, a randomized trial indicated. Researchers saw no significant difference in Olerud-Molander Ankle Score (OMAS), a measure of ankle fracture symptoms, with casting (64.5, 95% CI 61.8-67.2) versus surgery (66.0, 95% CI 63.6-68.5) 6 months following fracture among a group of 620 elderly individuals from 24 trauma centers and general hospitals in Great Britain.
But there were other differences in outcomes: complication rates were greater in the surgery group, whereas malunion seen in x-rays was more common with casting. "Close contact casting was delivered successfully for most participants, substantially reducing the number of patients requiring invasive surgical procedures at the outset, and additional operations during a 6-month period," reported Keith Willett, MBBS, FRCS, of the University of Oxford in England, and colleagues in the Journal of the American Medical Association.

New Guidance Lays out HIPAA Obligations for Cloud Computing
The Department of Health and Human Services has published new guidance on complying with HIPAA privacy, security, and breach notification rules when using cloud computing technology. The guidance gives insights for providers, business associates, and cloud computing vendors. Some of the guidance is basic and well-known to many HIPAA-covered entities.
The first question, for instance, considers if a HIPAA-covered entity or business associate may use a cloud service to store or process electronic protected health information (ePHI). The answer is yes, provided the vendor enters into a business associate agreement that specifies how HIPAA compliance will be maintained. Also under HIPAA, providers can use mobile devices to access ePHI from a cloud platform as long as appropriate safeguards and BAAs are in place.

6 Ways to End a Doctor-Patient Relationship - Part 6
As doctors, we have all treated patients who were very difficult to deal with, due to their non-compliance, an abusive attitude, or just a mismatch of personalities. For most of us, there is no need to stay in a relationship that is clearly not serving the patient nor the doctor well. What is the best way to terminate a doctor-patient relationship?
6. Suggest ways they can find new providers. I also tell patients to contact their insurance company for a list of providers or to contact the physician-referral line at one of the local hospitals. You are helping them secure new medical services and cannot be seen to be abandoning them.

Query: Billing An Established Patient Hospital Visit
When a patient has already been established with care by the physician in the last 3 years, and then has to be seen in the hospital for another diagnosis, do we code CPT 99232 (subsequent care hospital) or CPT 99222 for initial hospital care since the diagnosis is new?
Vanessa Sloan, Effingham, IL
Response: When billing a hospital visit on a Medicare patient, you would use the initial hospital care coding. The description is new or established patient. The key is that you are seeing a patient for the first time for that admission. If you need to see that patient again during the same admission, then you would bill the subsequent care code series based upon medical acuity of the services you rendered.
If that same patient were seen X weeks later during a different admission, then you would still bill the initial care code series.
