PODIATRIC RESIDENCIES IN THE NEWS
UTHSCSA Podiatric Residency Celebrates 50 Years

The Podiatric Medicine and Surgery Residency Program at The University of Texas Health Science Center at San Antonio (UThSCSA) celebrates its 50th anniversary in 2022. The program, which includes its clinical partner, University Health in San Antonio and its affiliated clinic, the Texas Diabetes Institute, is one of the oldest programs in the U.S.

Dr. Lee Rogers

When it comes to treating disorders of the foot, Lee C. Rogers, DPM, podiatry division chief and associate professor in the Department of Orthopaedics, who joined the institution in 2021, said, “We come from so many different specialties, which include vascular surgeons, podiatrists, cardiologists, endocrinologists, physical therapists, and many others, all working together in a multidisciplinary team approach. This approach has been reviewed in dozens of respected peer-reviewed papers and shown to have the biggest impact in limb loss prevention.”

INTERNATIONAL PODIATRISTS IN THE NEWS
Foot Pain Can Be Sign of Serious Health Issues: Aussie Podiatrist

New research from the Australian Podiatry Association has shown that 2 out of 5 podiatrists identify life-threatening health issues during their daily schedule of patient foot examinations. The Association this week issued a study as part of Foot Health Week (10-16 October) to get your feet checked by a podiatrist – it could save your life.

Charlotte Bodell

Podiatrist Charlotte Bodell, who is a member of the Australian Podiatry Association, said foot pain can be symptomatic of a much more serious health issue that podiatrists are trained to assess. “People may not realize just how powerful the foot is when it comes to assessing serious underlying health conditions,” said Bodell.

PRACTICE MANAGEMENT TIP OF THE DAY
8 Best Practices for Medical Malpractice Defense

The best way to prevent a malpractice lawsuit is to prepare for the likelihood of being sued, experts say. Check out these 8 medical malpractice best practices.

6) Steps to take after a lawsuit is filed
  • Physicians will need clear thinking in the instance of a claim or a lawsuit, Peter Kolbert said. "When physicians get sued, it's a new universe and they don't fully understand it," he added.
  • Often the cases that end up getting filed are not necessarily the ones physicians would expect, so they may take a physician by surprise, attorney Christopher Ryan said.
  • Senior trial attorney Tom Benvenuto said it's normal for a physician to want to go back in and change the note, but it's a very bad idea. "The first thing you must do is resist the urge to add anything to the note. I don't even mean falsifying it, but just adding information that you didn't put in the first place. Those always get you bitten ."

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A
From: Robert Scott Steinberg, DPM

Mr. Schwartz, You are incorrect. The DPMs I know always discuss shoes. I do know non-DPMs not only do not discuss shoes, but they also give their clients their orthotics in a plastic bag and tell them to put them in their shoes.

Robert Scott Steinberg, DPM, Schaumburg, IL

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1B
From:  Ira Meyers, DPM

I strongly disagree with the post from Robert Schwartz, CPed, regarding biomechanics and footwear. When a patient comes to the office, whether it be for a painful hyperkeratotic lesion, foot ulcer, or any type of overuse injury, the first thing that is checked are the shoes.

With the great variability with different types of running shoes, it is especially important to check shoes. There can be is much as a 20 mm drop difference between different types of athletic shoes, which significantly changes the whole biomechanical exam. Shoes should be the first things that are checked, followed by a static biomechanical exam, then an active biomechanical exam.

In short, lower extremity biomechanics cannot be evaluated without evaluating footwear.

Ira Meyers, DPM, Huntingdon Valley, PA

RESPONSES/COMMENTS (PM EDITORIALS)
From: Vince Marino, DPM

Kudos to Dr. Block for hitting the nail on the head. To emphasize the point, my old associate and friend Fred Youngswick, DPM used to say we make at least 2x the income with 1/2 the liability by being in the office. Let’s take a look at an example. Let’s say you have a patient who needs a bunionectomy with metatarsal head osteotomy, repair of a second hammertoe deformity and a Weil osteotomy with a plantar plate repair for a subluxing and painful 2nd MPJ all on the same foot. These are reimbursements for Marin County in CA (expensive county).

Procedures-
CPT Code Reimbursement                    (Medicare Fee)   Private Insurance (About 25% more)

Bunionectomy with osteotomy (28296)          $597.78               $747.23
Weil Osteotomy  (28308-51)...

Editor's note: Dr. Marino's extended-length letter can be read here.
