AK Podiatrist Advocates for Early Breast Cancer Screening
Dr. Heather Kaufman knows the importance of early screening and hopes that other women will recognize that as well. “I went and had imaging done, and it showed that it was a solid mass," Kaufman said. "Which was biopsied and it came back positive for breast cancer.” Kaufman, who is a podiatrist with Anchorage Foot & Ankle Clinic, said that she discovered a lump on her breast while in the shower.

She went through chemotherapy and radiation, and she said soon, she is expecting to be cancer-free. October is Breast Cancer Awareness Month, and local physicians want the public to know the importance of getting checked.

Crocs Not Recommended for Kids: NJ Podiatrist
Are Crocs okay for your kid’s feet? Or will wearing the clog-esque shoes damage her tootsies? Here’s what a podiatrist wants you to know. “When kids wear Crocs, their heel is allowed to be excessively mobile because of the wide nature of the shoes and the loose strap around the heel,” Dana Canuso, DPM, a podiatrist, tells Romper. “This mobility does not give adequate support to the rearfoot which can lead to flat arches and hyperpronation as they get older.”

Over time, flat feet can cause common foot complaints such as plantar fasciitis, tendonitis, bunions, and hammertoes — exactly what Crocs were created to prevent in the first place. And if you’re walking incorrectly, the problems can become more pronounced as you get older, even causing knee, hip, and back pain.

Healthcare Trends Impacting Physician Practices - Part 3
Physicians are facing new challenges as they balance patient care with business priorities. Restrictive insurance rules, government regulations, and declining compensation are just a few of the obstacles that modern practices are up against. Despite these challenges, successful practices are identifying solutions to these business pressures. Here are five trends shaping the healthcare industry. As we approach 2020, administrators should consider how these trends may impact, and potentially improve, their practices’ outlook.
3. Regulatory Uncertainty - While healthcare regulatory reform has increased some reimbursements, it has also posed potential penalties related to reporting quality standards and the documented use of EHRs. Meanwhile, political attempts to alter or eliminate the Affordable Care Act have brought additional uncertainty for physicians and their practices. As a result, many physicians are postponing important decisions to expand, sell, or participate in new patient-centered medical homes, affordable care organizations and other innovations encouraged by the current law.

Query: Re-evaluation of Traditional Angular Parameters for Austin Bunionectomies
According to standard podiatric textbooks, the maximum angular relationship between the first and second metatarsal should be around 16 degrees but can vary due to other parameters such as metatarsal width. A wider metatarsal would allow greater translocation and maintain adequate bone to bone contact for primary bone healing. In the last year, I have been using a minimal incision neck osteotomy technique which I have found to be quite effective. The technique can be seen on Wright Medical’s website. I have been able to use this technique on intermetatarsal angles of much greater than 16 degrees without complications, and if you watch their video, you will see a bunion repair that shifts the metatarsal head over greater than 50%. I was wondering if any other podiatric surgeon has experienced the same and wondered if re-evaluation of the traditional angular parameters for this “short Austin” should be revisited.

Above is an x-ray of a mid-aged female with a significant bunion and a pre-op intermetatarsal angle of >18 degrees as an example: pre-op and intra-op. I believe the main reason for being able to get so much stable correction is that the first and largest screw used to fixate the head passes through both the medial and lateral cortex of the first metatarsal proximal to the osteotomy and imparts significant stability when advanced into the head of the metatarsal.

RE: Itraconazole Dosage Error

With regards to treating patients with oral antifungals, assuming a PAS has already been performed in the past, I advise ordering a DNA test (PCR Assay) to identify the causative genus and species and choose the drug that best covers that organism. I also strongly recommend proper specimen collection with obtaining proximal nail and subungual debris, not just submitting from the distal nail plate.
The latest evidence-based medical literature reports itraconazole having a 54% success rate (mycologic and clinical cure), an average cost of $2 per pill, and prescribing the Sporanox PulsePak has clear instructions that help reduce patient dosing errors. Terbinafine has been shown to be superior for most patients presenting with onychomycosis, as it has a reported 70% success rate with an average cost of $.30 per pill, but may be less effective against some saprophytes.

RE: APMA Young Physicians' Institute

On October 11-13, along with fellow young member Dr. Jeff Kagan (PGY-1), I attended the APMA Young Physicians' Institute in Nashville, TN. As a member of the APMA Young Physician Leadership Panel, I met with fellow panel members prior to the main YPI program for a strategic planning session. Here the panel discussed key and imminent problems facing young DPMs and established working goals for the next two years. The leadership panel is composed of young podiatrists from across the country, who serve as the intermediaries between the APMA Board of Trustees and the membership at large. The primary goal of the YPLP is to advocate on behalf of all young DPMs.
The formal APMA YPI program took place October 12th and 13th. Attendees representing a wide variety of component states had the opportunity to partake in lectures and workshops focused on building leadership skills. This is a phenomenal yearly program whose goal is to help build the next generation of leaders in podiatry!

When I was in full-time practice, I had these small drill engines attached to Sani-Vacs via black vinyl tubing. We had reamers to clean the tubing. In addition, we had Honeywell cleaners with charcoal filters. We had no problem in 40 years.

There has been a lot of input about air purification, not filters. Nail dust can only be extracted from a room or entire office if you treat the air itself. There is only one way to successfully do this and it is by ozonation and ionization. Remember, air can only be cleaned when it gets to a filter. Ozone cleans the air by using O3 to change the chemical structure of impurities and ions attack the negative change of neutral particles in the air (nail dust). Look into Alpine Industries on the Web, it makes sense.

It is gratifying to see at least some comments on my original call for a single board. It is painfully obvious that change is hard and opinion strong on this topic. However, the future requires growth and development; otherwise it stays stagnant which will move us backward as the rest of the progressive world passes us by. We are only as good as our weakest link. Let’s make sure we have a high standard and strengthen all the links in our chain, keeping in mind that we are a procedure-based profession regardless of the simplicity or complexity of those procedures.

I don’t think a unified board would do anything to mitigate confusion regarding the services podiatrists provide to the public. I also think it would be a disservice to those podiatrists who received specialized training in surgery. Here’s the obvious dilemma: a majority of states require at least 2 years of post-graduate training in a certified residency program. So how can the profession satisfy state laws requiring the minimum residency training and differentiate non-surgical from surgical training?
Additionally, DPMs, by definition, are doctors of podiatric medicine and surgery, so the definition of our degree needs to be addressed. I’m sure there are many other hurdles that would need to be addressed before changes in board status, training, and meaning would be rational.
