PODIATRISTS AND SPORTS MEDICINE
Ankle Injuries Often Require Immediate Medical Attention: CA Podiatrist

Considered by many to be the best gymnast of all time, Japan’s Kohei Uchimura’s remarkable reign as world all-around champion ended abruptly earlier this month at the 2017 FIG Artistic Gymnastics World Championships due to an ankle injury. Uchimura was later diagnosed with torn ligaments in his left ankle. Gymnasts' feet and ankles take an awful lot of abuse, which can lead to sudden traumatic injuries such as sprains, torn ligaments, ruptured tendons, and broken feet, ankles and toes,” said foot and ankle surgeon Thomas Elardo, DPM, a private practitioner in Los Gatos, CA. Dr. Elardo is himself a former competitive gymnast and is very familiar with the wide array of injuries that can afflict gymnasts’ feet.

Dr. Thomas Elardo

“An ankle injury occurs when the ankle joint is twisted too far out of its normal position. The type of injury is determined by the kind of tissue – bone, ligament, or tendon – that’s damaged. Uchimura’s injury was to the ligaments, the strong elastic bands of connective tissue that keep the bones in place and allow for normal ankle motion. “If after an injury any of the following are experienced: pain, swelling, bruising, tenderness to the touch, or inability to bear weight on the injured joint; a podiatrist should be seen immediately,” Dr. Elardo stressed.

INTERNATIONAL PODIATRISTS AND SPORTS MEDICINE
UK Podiatrist Provides Post-Marathon Advice

If you have completed an endurance sport such as a marathon or mountain hike, your feet are probably hurting and in need of some attention, so make sure you have a game plan in place. Podiatrist Emma Supple says, "Deal with the blisters first and make sure you do not take the top layer of skin off of them as that will make them even more painful. It is best to pop them in controlled conditions using a sterilized needle. Simply pierce the blister to expel the fluid and tape down the blister afterwards so it can self-heal.

Emma Supple

"Rest your feet for a few days – walking gently is the best way to recover and Epsom salt baths are said to be very helpful too. Any blackened toenails and the likes will develop over a few days, so see your podiatrist if you have concerns," says Supple.

PRACTICE MANAGEMENT TIP OF THE DAY
Things to Consider When Retiring from Practice

5.  Know your numbers.

"I own the office building" is not a retirement savings plan. Remember 2007-2008? Confer with your lawyer, estate planner, investment advisor, insurance broker, and accountant — as a group. Schedule an advisor team meeting in which you ask each to present their perspectives on financial and tax issues. It's quite instructive, and you will make better decisions in this collaborative meeting format than if you hastily try to do so between cases, or while dashing from exam room 2 to exam room 3.

CODINGLINE CORNER
Query: Diagnostic Block of the Ankle Joint

How would you code a diagnostic ankle block into the ankle joint?

Michele Kurlanski, DPM, Scarborough, ME

Response: I suggest coding CPT 64450 (injection, anesthetic agent; other peripheral nerve or branch).

I have been hearing about denials on this from podiatrists around the country. I suggest you check on coverage for this before using it.

If this is a Medicare patient, check your LCD and if a private payer, see if they have a policy for peripheral nerve blocks. If this is a non-Medicare payer, you can also attempt to get a pre-authorization.

From: Jesus C. Vazquez, PMAC

Sonoma County is very grateful for the support from first responders during the fire disaster affecting our community. Dr. John D. Hollander is going to be providing foot and ankle care at no cost to police, firefighters, utility responders, and paramedics helping our community through this crisis. The office will be providing walk-in and evening appointments to accommodate different working shifts of first responders.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2

RE: Podiatrists Serving in the Military

I believe that most of our profession are not knowledgeable about the history of podiatrists who served in the military as podiatrists who were commissioned officers. Osteopathic physicians did not serve as officers until 1967. Some podiatrists served as commissioned officers in the Navy in World War II. In 1957, podiatrists were routinely commissioned in the armed services as podiatrists, but were not actually in the medical corps.

I believe that when podiatrists began serving as officers in the military and began working alongside physicians as fellow healthcare professionals that the image of our profession was improved considerably. I would like to see an article about some of these pioneers in our profession whom I believe have been overlooked. I had three of my four years of podiatric medical school subsidized by the military. I have always been grateful for the military podiatrists who came before me and played a part in making this possible.
