IN Podiatrist Produces "Parent Plan for Drug Prevention" Video
 
For 14 years, Dr. Mann Spitler has been telling the story of his daughter’s drug addiction --Manda’s Story -- to every school, church, and community group that would listen. It’s an achingly sad story of loss and waste that ends on a night in April 2004, when the 20-year-old Manda took a lethal dose of heroin and died submerged in the bathtub of their family home in Valparaiso. What Spitler knows now about addiction and its warning signs, about intervention, about the sheer pervasiveness of the heroin and opioid problem - could conceivably have saved Manda’s life. It has become his mission, accordingly, to give other parents the tools they need to save their own child’s life.

The video opens with old family snapshots of Manda, as an infant, a toddler, a bright and cheerful little girl, a lovely young woman. The sound track: the horrific audio tape of the 911 call which Spitler made from his home 12 years ago after discovering Manda unconscious and unresponsive, in the bathtub. “For 30 years, I was a podiatrist with a DEA license to prescribe controlled substances,” Spitler says, talking from the now empty home where his daughter died. “I was knowledgeable about these drugs. But I was ignorant about street drugs, the drug culture, and addiction. We live in an ocean of drugs. Our children are drowning in it. And yes, potentially your child.”

Lionel Messi is back. The Argentine star is in training for Barcelona after recovering from his adductor injury and is in line to play some part in the Catalans' Liga clash against Deportivo La Coruna on Saturday. "Messi is a very explosive player whose game features quick bursts of pace," sports podiatrist Mirja Marco told Goal International. "That means he needs to do a lot of stretching because those bursts and sprints, starting and stopping, affect him greatly in terms of his muscle insertions.

AZ Podiatrist Extols the Virtues of 3D Printing
 
It is the potential of 3D printing to facilitate just–in-time, specific or tailored objects, manufactured on a small scale, but cheaply and without the need for supply chains, that has got Dr. David Armstrong, Professor of Surgery and Director of the Southern Arizona Limb Salvage Alliance (SALSA) at University of Arizona, interested. “I’m evangelical about technology,” he says, “but I’m always asking myself how can we leverage it to help our people. If it doesn’t work, I move on.

The diversity of potential 3D printing applications in medicine and other fields is exciting. 3D printing models are available to download, can be created by computer-assisted design, using a 3D scanner, or a digital camera and online software. “It’s like the Napster of things”, says Armstrong. “If the knob on your toaster breaks—you could just go online, get the file, and print another one.”

Trading Heels for Ballerina Flats is Wrong Move: DC Podiatrists
 
After being inundated with information about how bad stilettos are for your feet, it makes sense that you’ve resorted to their opposite: a flat, paper-thin shoe you can wear during your commute, then store in your bag or toss under a desk. But those ballet flats might be doing more damage than you realize. The barely-there soles offer little shock absorption, hence causing heel pain from the repetitive stress, says Dr. Michael Stempel, chief of podiatry at the GW Medical Faculty Associates.

And those flats that you bend in half to fit in your clutch? Their flexibility means they offer almost no support. The foot then moves more and the tendons have to work harder to stabilize it, leading to tendinitis, says Dr. Eleanor Wilson, a local podiatrist with Kaiser Permanente.

GTEF "Summits" Matterhorn
 
The Greater Texas Education Foundation (GTEF) took 30 attendees to Northern Italy and Switzerland for their International Symposium, Sept 30-Oct 8. The trip was led by Dr. Sam Mendicino, GTEF Director and is a fundraiser for the Houston residency programs under his direction. Drs. Mendicino and Mark Hofbauer presented riveting interactive talks and case discussions. Attendees also enjoyed tours of Milan and the Northern Italian lakes - with great views, wine, food, silk design, and a stay in the Grand Borromees Palace.

The trip culminated in Zermatt, Switzerland where most of the group rode to the top of the Matterhorn Paradise lift system which is the highest cable car station in Europe (3,883 m/ 12,200ft). The GTEF group met American and Swiss Olympic ski teams, since Zermatt is one of the few places to ski this early. GTEF has been holding international CME trips since 1999.

MACRA UPDATE
Final MACRA Rule Expands Exemptions, Flexibility
 
Nearly a third of physicians could be exempt from Medicare's new merit-based incentive payment system under a final rule the CMS issued Friday for implementing the Medicare Access and CHIP Reauthorization Act. The CMS also signaled it would broaden the opportunities for physicians to participate in alternative models that make them eligible for bigger rate increases and bonuses. To help ease the impact on small providers, the CMS will exempt physician practices with less than $30,000 in Medicare charges or fewer than 100 unique Medicare patients per year. 
 
The draft rule set the threshold at $10,000 a year. An analysis by the American Medical Association found that about 16% of all MIPS-eligible clinicians would be exempt under the proposed version of the rule. The threshold in the final rule would exclude 30% of physicians, according to the AMA analysis. The CMS noted that more than 93% of Medicare Part B charges would still be subject to the incentive framework, which was devised to nudge physicians toward value-based care.

ONC Finalizes Rule Giving it More Oversight of EHRs
 
The Office of the National Coordinator for Health IT has finalized a rule that will give it more oversight over certifying electronic health records and other technologies that store, share, and analyze health information for consumers. The rule also gives the ONC the authority to ask developers to pull non-compliant products from the market.
 
The ONC will now have the power to decertify health IT products that don't comply with regulations or are found to pose a risk to public health or safety, for example, if they caused medical errors. If the ONC decertifies a product, its developer will be required to notify affected customers and providers who purchased the products. The ONC could also issue a cease-and-desist notice to prevent the future sale or marketing of the product.

6 Ways to End a Doctor-Patient Relationship - Part 5
 
As doctors, we have all treated patients who were very difficult to deal with, due to their non-compliance, an abusive attitude, or just a mismatch of personalities. For most of us, there is no need to stay in a relationship that is clearly not serving the patient nor the doctor well. What is the best way to terminate a doctor-patient relationship? 
 
5.You cannot just terminate a patient on the spot. You need to remain available for a certain period of time in case they have emergencies. I usually give them 30 days, only for emergencies, to find a new doctor. And this in no way means you need to refill their opioids that you do not agree they should be on or perform optional procedures on them. I have found that once a patient receives the letter, they no longer wish to see me anyway. But, if you do not allow them adequate time to find a new physician, you are in fact abandoning them and can be held liable if they suffer a bad outcome. If they have not found a new doctor in the allotted time, then that is on them. They were notified and failed to act on it, and you cannot be held responsible for their inaction.

Dr Albright is correct that Medicare and the rest of the insurance world may find it easier doing business with large medical conglomerates rather than small practices. This may work in cities such as Indianapolis or where I practice which is a suburb of New York City. Every day in my area, we see that more and more primary and specialty practices are being gobbled up by hospitals or private medical conglomerates.
 
If we look closely at the map of the U.S., the majority of land is rural and in these non-city, non-suburban communities, people rely heavily on the local  familty doctor who is in solo practice to provide them with medical care. It would be a great disservice to these communities to put these doctors out of business for no other reason than to make it more convenient for the government or the insurance industry.
