Lexington Podiatry will host their first annual open house and Heart and Sole mobile blood drive on Oct. 19th. Also, the Lexington Humane Society will be in attendance to receive a $2,048 check, which the Lexington Podiatry team raised for the shelter in the month of September.

IL Podiatrist Selected as  Chairman of  Graduate Education Commission 
 
The Commission on the Economics of Graduate Education in Illinois has selected Dr. Sev Hrywnak as their new Chairman to lead the organization. The Commission, composed of medical educators and administrators of medical programs, has been tasked with evaluating the cost of graduate education, as it relates to tuition and loans and the return on investment in the medical career field.

"The schools have an obligation to the taxpayers who front the student loans, as well as an obligation to the students who use the loans to ensure that once students graduate, they have the financial means to repay them in a timely manner,” said Dr. Hrywnak. “Our first order of business will be to review the tuition structure of the Scholl College of Podiatric Medicine in Illinois and ensure the tuition provides proper clinical education in the 3rd-4th years of school, as well as assurance of a residency program for the students.”

CA Podiatrist Raising More Money Than Incumbent
 
Veteran GOP Rep. Buck McKeon has represented California's 25th District for over 20 years, and in all that time, he never really faced a competitive general election. But what had long been a comfortably Republican seat has trended sharply toward Democrats in recent years, and redistricting hasn't even really played a role. This shift seemed to catch McKeon unaware, and he beat his Democratic opponent in 2012, podiatrist Dr. Lee Rogers, by just a 55-45 margin—by far the smallest of his career.

Rogers is running again this cycle, and though he raised only $374,000 all told last time, he took in $227,000 in the last quarter alone, outraising McKeon. In fact, McKeon, who is 75, has not actually confirmed his re-election plans, and despite last year's scare, you have to wonder if he still hasn't shaken off that rust.

CLINICAL RESEARCH
Medical Honey is Overestimated in Infectious Disease Prevention
 
Physicians had hoped that they would be able to apply medical grade honey instead of antibiotics in infectious disease prevention due to the honey's broad range of antibacterial effects. However, an Australian study undertaken in kidney patients who had received peritoneal dialysis showed that honey had no advantages compared to antibiotics. The study was published in The Lancet Infectious Diseases.
 
186 patients applied a daily dose of 10 milligrams of medical grade honey to the site of catheter insertion, the remaining 185 were tested for nasal carriage of S. aureus bacteria and treated with antibiotics if they tested positive, and with routine wound care if they tested negative. The examination showed that honey had no advantages compared to the antibiotic treatment.

QUERIES (THE AFFORDABLE CARE ACT)
 
I have no clue where we, as podiatrists fit into exchanges? When I read a passage like the one in today's article "Of the more than 1,000 practices surveyed by the MGMA last month, 40.2% said they are still evaluating whether they will participate in exchange health plans, 29.2% said they plan on participating (with 26.8% of those reporting they have an “all-products” contract clause requiring them to participate), 16.2% said they just don't know, and 14.4% said they will not be participating." 
 
I ask what plans are they referring to? What exactly are exchanges? Is there a webinar/seminar offered for podiatrists to educate ourselves about this?

RE: Repair of PADnet
 
I have a PADnet unit that I just paid off last month. It is having technical issues and is in need of service. I called Biomedix and they said I need to sign a $1,200 per year contract to receive any repair service. Does anyone have any alternative ideas on having the unit repaired without going through Biomedix?

A uric acid of 7.5, even though WNL, is a high normal (pending the labs reference). Prescribe a steroid taper, apply a pneumatic boot, and go over his diet. Keep us posted.

This is possibly gout. You may be able to aspirate a tophus to check for crystals. Give colchicine or Naproxen for two weeks, then re-check the uric acid. After that, start on Allopurinol or Uloric and continue with anti-inflammatory medication. Get the uric acid <6, the lower the better. Address diet issues.

Reconstructive Foot and Ankle Surgery - This fellowship is a one-year experience during which the Fellow will develop a rational approach to revisional and reconstructive foot and ankle surgery. The Fellow will function as an Instructor / Clinical and participate in the teaching of Podiatric medical students and residents, and have ample opportunity to be involved in clinical and basic research projects. The fellowship will provide the Podiatric Surgeon, further expertise in Charcot reconstruction, trauma and deformity correction.

This sounds like acute gout, even with a normal serum uric acid result. Although the flare-up was coincidental with his swimming, this was not likely the cause. The pressure and tightness from an Unna boot can make the pain worse. I suggest continue NWB/crutches and dispense a walker brace. Do not apply a BK cast because you will have to remove it. Put him on a Medrol dose pack, with elevation of the foot and leg when possible, and teach him pressure relief techniques for sleeping to off-load the heel.

The problem with ordering expensive blood tests for a patient presenting with peripheral neuropathy is that you don't want to miss a diagnosis where a timely treatment might have made a difference in the patient's outcome. The blood tests you mentioned may all be appropriate. You gave an example of Lyme disease. I practice in an area where Lyme disease is endemic. When patients present with peripheral neuropathy of unknown etiology, I order that test. 
 
Rarely, have I had a positive result, but this summer, I had two neuropathy patients who did have positive Lyme disease tests. I made timely referrals to appropriate specialists. Could they have sued me if I had not ordered the appropriate tests? Perhaps, but that is not what should make us tick as doctors. To the contrary, it should be the joy and great desire to get our patients well. In that context, if appropriate tests are needed, they should be ordered. It is doubtful that a patient with severe neuropathic pain would argue about a $3,600 dollar bill if his or her life and well-being were at risk.

It is one of the only periodic email publications that I receive that I routinely read every day. When Barry takes a break and shuts down publication for brief periods of time, I feel somehow disconnected. 
 
PM News provides a vital and comforting connection for me to the profession, very much the way Facebook provides a vital and comforting connection for me to my friends. I’ve made the statement recently that Facebook has improved my quality of life, by keeping me connected with a larger community of friends than I could manage otherwise.  In much the same way, PM News has improved the quality of my professional life, and for that, I am very grateful to Barry and Al.
