KS Podiatrist Discusses Footwear for Students

Since feet are vital in holding our bodies up, wearing a good, fitted shoe is crucial. “As a rule of thumb: the thicker the sole, the stiffer the shoe, the better the arch support, the better the shoe is going to be,” says Dr. David Laha, a podiatrist at Kansas City Foot Specialists in Overland Park.

The biggest problem for students is they do a lot of extra walking than they are used to, so having good footwear is going to make the hike around campus more comfortable and avoid injury, Laha says. Wearing a running shoe to get from class to class is the best option. Instead of spending small amounts of money on cheap shoes, save it up and buy one good pair. Motion control running shoes are usually the best for most people because they have features that prevent the foot from rolling inward, Laha says.

OH Podiatrist Names OCPM Walking Path

Aaron Chokan, DPM, class of 2001 and President of the OCPM Alumni Association, named the recently built walking path at the Ohio College of Podiatric Medicine for $50,000. The name of the walking path is now called The Chokan Family Wellness Walking Path. “I am very proud of my alma mater and all of the accomplishments of the college. My family and I wanted to contribute to the college that has provided an excellent opportunity for its students, faculty, and staff to help stay fit.”

The walking path is 1/3 of a mile in length that encircles the Phase 1 property of OCPM’s Land Use Plan. The Phase 1 property also includes lighting, a 20’ x 30’ pavilion, a restroom, and a 65 foot bridge that covers a creek and connects the walking path to OCPM’s main facility.

TN Podiatrist Named Podiatrist of the Year

Dr. David Long has been selected as Podiatrist of the Year at the recent meeting of the Tennessee Podiatric Medical Association in Franklin. The annual award, memorializing Dr. Harry Casson of Maryville, is given to the podiatrist in recognition of outstanding service to the field of podiatry.

Dr. Long has served as chairman of the board of podiatric medical examiners for the past eight years, and as president of the Tennessee Podiatric Medical Association. Long practices at the East Wood Clinic in Paris and in Martin, and specializes in medicine and surgery of the foot and ankle, as well as sports medicine.

Maryland Podiatrist Provides Marathon Training Advice

There is more to training for a marathon than running a few laps around the track. Finishing 26.2 miles, or even getting to the starting line, takes attention to some details about eating and hydrating, miles covered, and aches and pains.

Dr. John Senatore, an avid runner and chief of podiatry in Union Memorial Hospital's sports medicine department says that all marathoners need to taper (gradually decrease) their weekly mileage starting at least two weeks prior to the race. 26.2 miles is a long way, and fresh legs are a must to finish.

Aussie Podiatrist Rebounds From Financial Disaster

Early in March 2006, Port Melbourne entrepreneur Damien James discovered that $300,000 had gone missing from the foot care firm he started after graduating from Latrobe University in 1997 with a podiatry degree. After James confronted a staff member, the suspect fled overseas. After the theft, James had to shut his two podiatry businesses with a combined yearly income of over $1.5 million.

"I went from running two of Australia's largest podiatry companies to being broke within a few weeks. My life was a mess," says James, 37. His reputation in tatters, he pondered leaving the industry. Instead, he re-launched fast under the same name, embarked on a Swinburne University entrepreneurship masters course, and engaged a mentor. James began regaining clients and winning new ones. Last year, his firm, Aged Foot Care Australia, posted a $2.2 million income, he says.

QUERIES (CLINICAL)

Query: Chronic Ankle Ulcers

An 86 year old male in good condition, arrived after 6 years of treatment by his PCP, with both medial and lateral ulcers on the same ankle. Initially the ulcers showed significant improvement with debridement, Venastat, and Amerigel. His ABI was over 0.9 (all 3 arteries, and circulation down to his hallux), so neither ulcer appears ischemic. Initially, I used support hose to treat him. When healing stalled, since the ulcers were not very deep, I decided to try Kalypto. With no further improvement after a month, he and his daughter decided that the pain from the pump was not worth it.

Last week, I applied some Silverlon. The pictures are of the lateral and medial ulcers immediately after dressing removal, and also after debridement. 1. Do you think there is enough difference between Synovis and KCI to make a pitch for the latter? 2. Without the NPWT, I would not have much confidence getting a "take" with either Dermagraft or Apligraf. Comments?

Query: Image Problem with A2D2 Digital X-Rays

Our office is currently having ongoing problems with the quality of the images with an  A2D2 unit, which have never being crisp and certainly not showing the level of detail that would be expected of digital images. The company has been responsive in attempting to remedy the problem, but despite all attempts, the images are still of fair to poor quality, especially when looking for sharp images of cortical margins, etc. 
 
Has anyone else with an A2D2 unit experienced a similar problem, and if so, have you discovered a solution to the problem?

To reduce complications with the TightRope procedure, for starters, the first consideration is PATIENT SELECTION! Risk factors for implant failure include, but are not limited to: osteoporosis (penia), obesity, co-morbidities that affect serum calcium, choice of bunion surgery based on proper criteria, and lastly, surgical technique.
 
That said, overall, I'm not very impressed with this procedure, nor its results, but my patient sample is small. In addition, I did witness one of my partner's patients experience a 2nd metatarsal stress fracture. His technique was spot on and criteria very appropriate. Personally, I've long headed back to traditional osseous corrections based on IM angle severity (Why change what has always worked?), and if patients don't want to endure the fracture healing required, they are allowed to seek the TightRope elsewhere with a thorough explanation of the potential risk factors and failures associated with it.

I'm old school - this is a no-brainer (if you've had surgical training) - ask if the condition is painful or is getting larger. If the answer is yes, excise using local anesthesia, and make a linear incision on the mid-line of the lesion. Use sharp and blunt dissection and take it out.  That's all; case closed in 15-20 minutes. I did one last week and many others in the past 36 years. There is less trauma and fewer problems.

Sure, there have been grades given and we all know the guy or gal in the back who always aced every examination, but what difference does it make when it comes to reality? Has any one of us gone to a physician for personal medical care and first asked to see his or her medical school grades? To the contrary, we might ask for recommendations from other physicians and people in the community. No reference is ever based on the doctor's med school grades.
