RI Podiatrist Stays Active as Soccer Coach
As the team podiatrist for Brown University athletes, Dr. Peter Sardella has had an opportunity to examine a lot of talented feet over the last 10 years. But he's also had a chance to find out what's going on upstairs a little bit, too. As a youth coach and the father of three, Sardella has become very interested in learning what makes athletes tick and how best to connect with them.

"Coaching soccer just gives me one greater chance to be a positive role model for my daughter, giving her guidance on the field, which makes our bond together even stronger. This will hopefully translate into more confidence and stability in her life, and that she never will be afraid to ask me for my guidance in other situations.
Coaching gives Sardella -- a three-sport athlete at Agawam (MA) High School, including an All-Stater in track, as well as a former standout soccer player at Springfield College -- a chance to stay active in sports he enjoys. "It's such a breath of fresh air, getting out of the office, throwing on the soccer boots and, bang, you're out on the soccer field, said Sardella, who also played for a year as a walk-on for the Hatfield Town Football Club in England. "It's just a great feeling."

Kosinski, DPM, Professor in the Department of Primary Podiatric Medical Sciences at the New York College of Podiatric Medicine, has been accepted as the first and only podiatrist member of the Infectious Diseases Society of New York, an organization comprised of infectious diseases specialists in the metropolitan NY area, and a regional arm of the Infectious Diseases Society of America. Earlier in his professional career, Dr. Kosinski completed a Fellowship in Podiatric Infectious Diseases under Leon Smith, MD, at St. Michael's Medical Center in Newark, New Jersey.

CDC Survey Says 22% of U.S. Adults Have Arthritis
Nearly 22% of U.S. adults have physician-diagnosed arthritis, according to an annual CDC survey, the Washington Post reports. The survey of 30,000 people counted various types of arthritis, including rheumatoid arthritis, gout, lupus and fibromyalgia. About 8% of U.S. adults' activities were limited between 2003 and 2005 because of arthritis. In addition, more than 31% of obese adults have been diagnosed with arthritis, the survey found. "Women; older adults; persons with little education; or those who are obese, overweight or physically inactive are more likely affected" by arthritis, the CDC report said. Among normal or underweight people, 16% have arthritis, according to the survey

Query: Footwear for Brittle Diabetic
I have a diabetic patient with a TMA on one foot and loss of all digits on the other. The patient had amputations as result of a sudden loss of peripheral perfusion. The resultant foot types along with extreme atrophy of all soft tissue makes the foot extremely fragile and subject to ulceration with only the slightest provocation. The patient has had many attempts at protective footgear. Does anyone have experience with footwear which might enable this patient to ambulate without the need for a BKA. With any dynamic muscle imbalance or spasticity, when performing a bunionectomy, I prefer to do an arthrodesis. Otherwise, recurrence could be a major issue.

RE: End ABPS Recertification at Age 60 From: Arden Smith, DPM
ABPOPPM's policy for re-certification states that their "mandatory re-certification date will be ten years from the date a diplomate received the first certification and every ten years thereafter, or until age 60." I would like to suggest that the ABPS adopt a similar guideline or flip to a self-assessment and/or C.M.E.] and accept that one's level of experience and prior re-certifications have earned one that privilege by age 60.
For what should be obvious reasons to all of us, both our exam taking ability and our ability to study is usually quite different after about age 60 than it was when we were younger; as is the toll that this whole process takes on one's practice, family life, and total well-being as one matures both in practice and in life. This would allow both of the APMA-approved specialty certification boards to follow similar and consistent guidelines in the area of recertification.

My experience with the patch is similar to what you report. I used it on my knee and within 24 hours, significant relief for weeks. A patient with chronic ankle pain years after a fracture had impressive results. The limiting factor is the cost. I find patients saying the cost is prohibitive.

I already had her chair tilted half-way back so I told my assistant, "we better go all the way (back)". My patient sat up and replied, "I hardly know you, Doctor!" I laughed so hard I could hardly finish the procedure.
