AT THE COLLEGES

IL Podiatrists and Scholl Students Volunteer at the 2019 Chicago Marathon

The podiatric community was well represented at the 42nd Annual Chicago Marathon which took place on October 13th. Jeffrey Baker, DPM of the Weil Foot and Ankle Institute and Michael Chin, DPM of the Running Institute were podiatric co-coordinators for the event again for 2019. Other attending volunteers, which included practicing podiatrists, fellows, and residents, were Chris Collazo, DPM, Katie Landry, DPM, Mark Magnus, DPM, Panah Nabill, DPM, Thomas Nordquist, DPM, Nadia Sadeghi, DPM, and Alison Young, DPM

Volunteering as well were 90 podiatric medical students through the American Academy of Podiatric Sports Medicine Scholl Chapter. The podiatry volunteers were stationed at two medical tents and provided podiatric medical care to over 100 runners throughout the day.

Truck Hits Office of NJ Podiatrist 
 
There was a close call as a pick-up truck struck the building in Holmdel, NJ. A driver visiting the Holmdel Executive Plaza apparently jumped the curb and hit the building. This building houses the office of podiatrist Patrick Caputo, DPM.

The Building Department was notified and EMS assisted one patient complaining of chest pain. There was damage to the building and to Dr. Caputo's office as seen in the photos.

Healthcare Trends Impacting Physician Practices - Part 2
 
Physicians are facing new challenges as they balance patient care with business priorities. Restrictive insurance rules, government regulations, and declining compensation are just a few of the obstacles that modern practices are up against. Despite these challenges, successful practices are identifying solutions to these business pressures. Here are five trends shaping the healthcare industry. As we approach 2020, administrators should consider how these trends may impact, and potentially improve, their practices’ outlook.
 
2. Increase in medical consumerism - Retail medical clinics and urgent care centers are becoming more popular — and are increasingly owned or operated by physicians. Today, more than 7,600 urgent care centers in the U.S. see a combined total of more than 890 million patients per year. Meanwhile, U.S. retail clinics took in about $1.4 billion in revenue in 2016, more than doubling their revenue from 2010. Since most insurers cover non-traditional care outlets, these clinics continue to emerge as tiny-yet-mighty competitors to larger physician practices.

Query: Shoes and Visit for Diabetes Management
 
A patient with diabetes who qualifies for shoes, has seen the PCP recently. However, the reason for the visit was for suture removal. Diabetes is on the problem list, but this visit was not specifically for management of diabetes. Does this still qualify as a visit for the reason of a qualifying PCP visit for the MTSP? The PCP still has to sign off on my notes, but I am waiting to see if the patient will have to return for an “official” diabetes visit.

Response: I’ll assume that your notes that are to be co-signed by the MD/DO will contain the required A-F requirements which are located in the LCD. I also assume the MD/DO who signed the certification statement has completed this correctly and the dates on all the documents are correct.
 
That being said, your narrative stated that the MD/DO has the history of diabetes in the note but no management is listed. The word management itself does not have to be exactly listed as management. It can be listed as medications, diet, etc. It can even refer to a previous note (which you might want to obtain). A copy of a recent lab test signed by the MD/DO could also be a reflection of management. With the simple narrative you provided, I would not want to go on record as stating it is either okay or not okay.
 
If you are at all uncomfortable, then obtain a note previous to the one you referred to (it can be up to six months prior to the date the shoes are dispensed). If the previous visit is more than six months prior and you feel uncomfortable about the most recent note, then I would agree with you and refer the patient back to their MD/DO who is treating the DM for a more comprehensive exam (and hopefully note). If you do resort to that, make sure the MD/DO knows exactly what Medicare’s requirements are. Make sure the MD/DO also knows they don’t need to do the foot exam and that the certification statement can’t be signed on a date prior to the exam.

Itraconazole/Sporanox is an excellent antifungal and has a much broader spectrum than terbinafin. However, it’s best to prescribe appropriately. Since the medication was taken at half dosage, I recommend and suggest you delay further medication for the next 3 months. I assume there is medication left over, and I recommend giving a booster dose correctly at 200mg BID for one week.
 
Please make sure it is taken after a meal for best absorption. The changes in the nail will be revealed after 6-8 months. If you are using it for this period of time, let it run the course to about 1 year post-meds. You may want to do another course of treatment for 3 months. After the first year, evaluate the results. I have used this protocol personally for 30 years of treating hallux nails with complete success, including non-recurrence of either skin or nail issues.

RE: Re-use of Single-Use Injectables
 
A podiatrist in a local hospital has been given by the nurses used single-use vials of Marcaine, Xylocaine, Celestone, and Decadron at the conclusion of his cases. He then uses these same single-use vials and re-draws from them to give injections to his patients. The illegality here is two-fold: First, this is fraud due to double billing, because the patient is charged for the full 50cc vial in the hospital, and then the patients are charged in the office for the same vial. Second, there are no preservatives in the single-use vial, hence the chance of infection is huge when using on multiple patients, especially when many of the patients are diabetics with infections. This podiatrist has been reported to the OIG as well as to the county health board. 
 
A very similar situation had occurred in Las Vegas, in which a physician was doing the same thing, with the state health department coming in and having to contact all the patients who had been injected to check on their health and well-being. I foresee the same situation here in that the health department will come into his office, and have to contact all the patients who have received injections and place them on notice that they have been injected with a contaminated medication. This is only the beginning of the end, with the hospital now being liable also by allowing him to remove the medications from the hospital.

For the past 12 years of practice, I have had zero residual nail dust in my treatment rooms, but I understand where you are coming from. During my first two years of practice, I had the same issue. When I moved to a different office, I had a central vacuum system installed. The vacuum is in a closet and shared by all treatment rooms. The rooms are connected to the vacuum via 2" PVC pipe above the ceiling tiles. The noise is contained to the closet, which has 2" thick foam added to the walls. 
 
I bought a cone nozzle (#ACC-SHEATH) from Jan L. It attaches directly to the end of a Dremel Multi-Pro and has a 1/2" tube connection. I also bought a reducer from Allegro Vaccums. It reduces the 2" outlet down to 1/2". The last item was a piece of 1/2" vinyl tubing I purchased from the local hardware store. There is strong suction noise in the treatment room, but no motor noise. The vacuum also serves as a floor vacuum for each room and we vacuum in between patients. No nails, no dust. I Dremel every patient. It is still going strong after 12 years of heavy use. I'm guessing I have about $1,500 in the system.

Like Amol Saxena, Joe Borreggine, and Jeff Robbins, I too would like to see a single board that represents all podiatrists. One single board would help eliminate the current confusion as to what a podiatrists is. I call on the two boards to begin talks to move this process forward. I think we can achieve a single merged board with a well-constructed 5-year plan. I agree with Dr. Saxena that we need to be training all podiatry students to take the USMLEs.
 
I have spoken to Kathy Satterfield, the new Dean of the Western University School of Podiatry and she is a strong advocate of this. I’m sure the other deans aren’t far from this view. We can run pilot projects, test the students to assess current education gaps, expand the curriculum, do remediation, and...
