OH Podiatrist Discusses Sever's Disease
If your active school-age child complains of heel pain, you may chalk it up to ill-fitting shoes. But that may not be the whole story. True, new shoes can cause heel pain. But physical activity, especially sports, can also trigger a problem called Sever’s disease, or pediatric calcaneal apophysitis. "While Sever’s disease is temporary, kids can have it for several years until their growth plates close," says podiatrist Christopher Herbert, DPM of the Cleveland Clinic.

"Kids typically feel better with reduced activity. We give them anti-inflammatory medication like ibuprofen (acetaminophen doesn’t work as well for this). That usually reduces symptoms. We also have them apply ice over their sock and wear a heel cup if they’re still sore. This cushions the heel and provides added shock absorption. If these measures don’t work, kids often have to use a walking boot to really reduce their activity," says Dr. Herbert.

OCPMA Symposium with USC Returns After 3-Year Hiatus
After a 3-year hiatus, the Orange County Podiatric Medical Association (OCPMA) recently held its annual scientific symposium in Huntington Beach. Entitled “Advancements in Foot and Ankle Surgery”, the symposium was the association’s first partnership with the Keck School of Medicine of the University of Southern California (USC).

“The symposium was, by every measure, a terrific success,” noted OCPMA president and symposium co-chair Jeffrey Haupt, DPM. The meeting, held at the Hyatt Regency Resort along Pacific Coast Highway, welcomed top flight speakers from USC, Orange County, and nationwide.

Negotiating Your Contracts: Hidden Pitfalls and Traps - Part 4
Negotiating and reviewing contracts are among the more unnerving tasks physicians face. That’s good! Your subconscious is sending you “danger” signals. You simply need to channel this emotion into action to protect yourself.
4. Beta testing is code for “we don’t know what we are doing but are hoping for the best.” Watch out for signs a startup or small vendor is trying their product to see if it works. It means they don’t really know and can’t really say if it works. This also means you are the test case. Ask for references and check them.

Query: Billing Non-Foot & Ankle Orthopedic Cases
I am wondering if and how a DPM would bill for assisting in non-foot & ankle orthopedic cases. Occasionally, my orthopedic partners need assistance for hip fractures, etc. No PAs/NPs are available. Can I bill Medicare for an assistant fee? Would I use modifiers -80-82 (physician) or modifier -AS (non-physician)?

Response: If your state allows you to assist in other non-podiatric surgeries, then simply bill whatever codes the primary surgeon billed and append your claim with the -80 modifier for each line item.

RE: Requests to Trim Fingernails (Michael Lawrence, DPM)

I agree with Dr. Michael Lawrence that there are times when we can further help an aged or infirm patient by cutting their fingernails. I understand why my colleagues have second thoughts about this because of malpractice issues, however to refuse this simple request would be horrid. I wish I could offer better advice. I admit that we have a few patients who have their fingernails trimmed in our office. I admit that I have a few dermatologists who referred patients to me for surgical correction of nail deformities. I performed these procedures although it may have been a foolish decision. And one more shameful admission, as a podiatrist who has an interest in the law and has studied law - I don’t even know if the state of Ohio allows us to work on fingernails.

Every once in a while, someone has a question regarding voice dictation. I have been using Dragon Dictate for 10 years. Up until recently, I found the program to be fair. I recently upgraded to Medical Version 4 and I am pleasantly shocked how well it works. If you have been holding off waiting for an almost perfect dictation program, now is the time.

The University of California, San Francisco (UCSF) is currently accepting applications for 12-month Fellowship Program for Limb Preservation training that begins on August 1, 2019. The UCSF Limb Preservation Fellowship educates the podiatric physician as a clinically skilled podiatrist in a multidisciplinary approach to the diabetic and vascular complications of lower extremities, as well as generating new knowledge in the treatment, management, and prevention of lower extremity amputations.
