ASPS Hosts Young Physicians’ Workshop
 
ASPS recently hosted a hands-on cadaveric workshop at the APMA Young Physicians’ Institute (YPI). While historically not a clinical event, previous attendees of the YPI requested the addition of surgical content. In response, ASPS hosted a half-day workshop, held the afternoon prior to the start of the seminar.

The workshop, like the YPI, was held at PICA headquarters in Franklin, TN. Faculty members Drs. Patrick DeHeer and David Yeager covered topics including 2nd MPJ (Hallux/2nd toe cross-over), plantar plate repair, Lapidus bunionectomy, and ankle fusion  - all topics chosen by the APMA young physicians themselves. PICA, Crossroads Extremity, Wright Medical, Osteomed Power, and Smith and Nephew were sponsors of this workshop.

FL Podiatrist Discusses Use of Human Amnion/Chorion Membrane 
 
Plantar fasciitis and tendonitis are common conditions seen in athletes. These conditions are largely due to trauma or overuse and the area becomes aggravated and inflamed. "In most instances, these conditions are self-limiting and respond very well to conservative treatment but as a last resort, may require surgery," says podiatrist Dr. Aden Huynh.

"If all of the conservative treatments have been exhausted with no symptomatic relief, an alternative treatment option of using a dehydrated human amnion/chorion membrane called Amniofix may be indicated. Amniofix is an injectable medication and typically administered directly at the site of injury. Recovery time is minimal and patients can ambulate in a walking boot immediately," says Huynh.

Patient Pricing Transparency Movement Hits Stride
 
Pricing transparency is becoming a game changer for patients, providers, and employers. It has to be. The rise of high-deductible plans means, more than ever, patients are being forced to manage their flexible spending accounts carefully. In turn, patients and employers are pressuring providers to lift the secret veil that has clouded its prices in mystery for a long time, in order for them to go "comparison shopping," said Cheri Kane, managing director at PriceWaterhouseCoopers (PwC).
 
One of the complicating factors around patient pricing transparency, and why physicians may not be ready for it, is regulation. While the federal government is strongly pushing this ideal with various measures, such as annual Medicare charge price data dumps, there are no laws requiring it nationally. On a state level, some have considered making it a requirement, but few have acted. Even in Massachusetts, where it's law, the results are less than ideal because physicians are unclear on the requirements they are facing, said Emily Anne Nolte, a senior associate at PwC.

The final rule-making modifications rule enables providers to pick any continuous 90-day reporting period in calendar 2015, down from 365 days that were required before the change. The change is what providers were pleading for. Successful attestation is required by February 29, 2016.

A few years ago, I was watching an interview with a cardiovascular surgeon reporting on an emergency open heart procedure he preformed on a well-known person. During the interview, the surgeon stated that the procedure was a "routine" open heart procedure. To specialists, there are procedures we do often, but using the term "routine" could possibly give the wrong impression as to what is actually being done.
 
I am sure there are practitioners who bill incorrectly and even commit fraud, but I also believe in such an adversarial medical climate the word "routine" could give the wrong impression to the significance of what we do every day. Reporting on fraud is very helpful, but is it necessary to mention the practitioner's name?

Editor's Comment: Unfortunately, in the pre-Internet days, podiatrists convicted of Medicare fraud simply hired young podiatrists to "work for them" and essentially continue to bill under these naive practitioners' Medicare numbers. The result was that these newbie DPMs eventually got caught in the same web as their Medicare-excluded mentors. Publishing names has virtually eliminated this "podiatrists eat their own" phenomenon.

I had Lasik surgery about 10 years ago. I have been able to function well in the office and while doing surgery. However, as one’s eyes age, seeing up close becomes a problem for all of us, even with Lasik. Lasik can’t fix the eyes getting older and needing reading glasses (as I’m starting to find!). Overall, I would do it again in a heartbeat.

I had my Lasik surgery two years ago after struggling with heavy glasses and pain on the nose and ears. In general, I like the results. Just like any surgery, I wish I had been told more about the recovery and potential complications. It took several months for the vision to be stabilized. I complained to the optometrist about the blurred vision during every post-op visit. She finally realized that I was used to seeing sharp images with the glasses. Even though my vision was measured at 20/20, I have very minor residual astigmatism (.5/.75). Long distance images are not as sharp. I have a bifocal reader for clinical work and when I want to see very well in the distance. I don't need glasses for computer or for the majority of my day.
 
One year after the surgery, a piece of toenail flew under my glasses into my right eye, which had never happened before. I went to the primary care. The nurse practitioner thought she saw a "cloudy area" of likely nail dust with inflammation. She tried to scrap it off. I called the optometrist for an emergency visit. She didn't see any problem. The cloudy area was actually the Lasik surgery graft. I certainly did not know what the graft would look like. It was scary, but everything turned out just fine. I now have dry eyes, which don't usually bother me much and can be taken care of with a lubricant.
