Risks of Barefoot Walking Outweigh the Benefits: NY Podiatrist
You take off your shoes every time you get home—and ask anyone visiting to do the same. But is walking barefoot bad for your feet? Even indoors? We checked in with Miguel Cunha, DPM, founder of Gotham Footcare in New York City, who said, “I always advise people from walking barefoot especially in public.”

Dr. Cunha told us that while there aren’t a lot of perks to walking barefoot, there isn’t as much risk if you’re walking on sand or on carpet, which offer more support. Still, he told us, “I advise people to wear in-door shoes at home with arch support (no flip-flops).”

OH Podiatrists Open New Practice
Podiatry specialists Kimberly Nichelson, DPM and Amanda Maszaros, DPM have opened an office as part of Sheffield Village’s medical community. Dr. Nichelson completed an advanced three-year foot and ankle surgical residency program through University Hospitals of Cleveland Richmond Medical Center and affiliated Cleveland-area hospitals and is affiliated with University Hospitals Richmond Medical Center.

Healthcare Trends Impacting Physician Practices - Part 1
Physicians are facing new challenges as they balance patient care with business priorities. Restrictive insurance rules, government regulations, and declining compensation are just a few of the obstacles that modern practices are up against. Despite these challenges, successful practices are identifying solutions to these business pressures. Here are five trends shaping the healthcare industry. As we approach 2020, administrators should consider how these trends may impact, and potentially improve, their practices’ outlook.
1. Practices for sale - Faced with rising costs and other pressures, some practices are selling ownership to hospitals or larger practices so physicians can focus on patient care. Hospitals acquired more than 5,000 practices between July 2015 and July 2016, increasing the number of hospital-employed doctors by 9 percent, or more than 14,000 physicians in the U.S. that year, according to a 2018 report from Avalere Health and the Physicians Advocacy Institute. With national health insurance companies also making acquisitions, fewer than half of all physicians now have an ownership stake in their practices, according to the report.

Query: Itraconazole Dosage Error
I prescribed itraconazole for a patient with toenail fungus in July. She failed therapy with terbinafine a year ago. PAS was done after 12 months confirming fungal involvement, most likely saprophytes. The itraconazole was prescribed as a pulsed dose of 200mg bid. The pharmacy dispensed 100mg capsules, labeled appropriately on the bottle with the correct instructions. The patient completed 3 pulses, but mistakenly only took 100mg bid. I am unsure how to proceed following this sub-therapeutic dose. I called Janssen Pharmaceuticals and they did not have an answer. Should I just wait and see, or have her repeat now at the correct dosage?

The only reliable assessment would be with an axial view. A special imaging device exists for this view, or you can elevate the heel and dorsiflex the toes with towels or gauze. A local injection, from medial distal plantar, with short- and long-term corticosteroids and Marcaine will relieve the pain.

It's just anecdotal, but I've used CA glue many times on my own finger hangnails and occasionally on a stubborn incurvated toenail. In every instance, the result has been almost instant relief and rapid healing. I've also used it frequently on small workshop and household cuts and scrapes with equally great results.
That said, if I was still in practice, I'd definitely hesitate to recommend that a patient do this. If anything went wrong, this would obviously be a huge problem.
