CA Podiatrist Named Volunteer of the Decade  
 
When the Benicia Police Department decided to give Ron “Doc” Uhlman the Volunteer of the Decade award last week, they thought the best way to get him down to the city hall ceremony was to trick him. Telling the notoriously humble volunteer that he’d be receiving his pin for working with the department, instead, Benicia Police Chief Erik Upson presented Uhlman with an award honoring him for his 20 years of hard work. “This department loves him across the board,” Upson later said of Uhlman. “He does so much, with so much humility, that there’s really nothing that he won’t do to help the officers or the staff out here — whatever it takes.

“And this is a retired doctor who’s basically willing to do whatever to support this department, small and big,” Upson added. Uhlman originally grew up in Ferguson, MS and went into the medical field in Springfield before heading to San Diego and eventually San Francisco for podiatry school. Uhlman started his residency program at Kaiser Pemanente in Vallejo, working there for 28 years, while maintaining nighttime volunteer commitments first at the Benicia Fire Department, then later the Benicia Police Department.

NY Podiatrist Opens Medical Spa-Type Office
 
Patients may note the comfy atmosphere once inside the new location of Batavia Foot Care Center. But don’t get settled in, owner Dr. Dawn Dryden says. “I wanted a medical spa feel. I wanted a really inviting reception area for the patients to feel comfortable,” she said in her office in Batavia. “We call it a reception area, not a waiting room. Our patients don’t wait.”

Dryden had originally done some residency work at the practice, then owned by Dr. Gina Teresi, and became an associate. The two eventually partnered. Dryden took over the practice in 2011 as Teresi has slowly prepared to retire. She will work through the end of this year before going off to “enjoy life” with her husband and family, she said. Meanwhile, Dr. Zerah Ali has joined the staff to help out with the customer load. Since the practice opened, it has served more than 26,000 patients, Dryden said.

Hospitals and critical access hospitals have to do nine objectives, with one of them being a public health reporting requirement, down from 20 total objectives previously.

This sounds like your patient has a case of primary or idiopathic erythermalgia also called erythromelalgia. Look it up in any standard text on peripheral vascular diseases.

The symptoms described by the patient and the fact that it is activity/heat induced suggests the possibility of erythromelalgia. Treatment involves keeping the feet cooler and various meds from OTC aspirin to a host of Rx meds. Orthotics are unlikely to have any effect on this condition.

I agree that this appears as a CRPS-type case, but when CFOs were fabricated, she seemed to improve for an extended period. This, coupled with the high level of inverted stance position would point to a fibula-related equinus.  I published an article on this subject in the Sept issue of JAPMA in 2000. 
 
When the fibula loses its translation ROM, a wide variety of symptoms can develop. One relates directly to a personal inhibition which relates to the lost fibula motion. The effect is to leave the posterior tibia unopposed and thus the inverted appearance. The other involves the release of neuropeptides which promote vaso-activity and nociceptive hypersensitivity. This could explain her odd burning complaints. The one other entity which can create this type of symptom is mastocytosis (which is also referred to as mastopsychosis for obvious reasons). Heat is a definite etiologic factor.
 
Manipulation of the ankle and cuboid are easy to perform, safe, and often spontaneously effective. These are on YouTube and can be searched under my name and manipulation. I have seen similar symptoms on many occasions with excellent outcomes. In mastocytosis, periodic care is required, as is avoidance of heat in this tough, lifelong condition. Good luck. I hope the simplest solution works.

My practice mainly consists of general podiatry, especially routine foot care on patients with diabetes, PVD, and on certain anticoagulants. I perform many nail border avulsions (11730) and would consider this a red flag. I assume Medicare also found my numbers to be outside the norm by their silent investigation they performed without my knowledge. I THINK I HAVE YOUR ATTENTION! Around four years ago, two patients informed me that a Medicare representative knocked at their doors to ask them a few questions about their visits to my office concerning my nail avulsion procedure. The two questions they asked about the nail procedure were as follows: Was your ingrown nail painful? and Did the doctor inject a local anesthesic?
 
Needless to say, I am still in practice, so my patients' answers were "yes" and "yes." If you don't infiltrate a local anesthesic, you must document why. In my practice, I infiltrate 99.9% of the time. Documentation must include several points. Obviously pain and redness are important, but key phrases should include post-care instructions, dressing applied after hemostasis was achieved, and avulsion was performed to the level of the proximal nail fold (eponychium). Was permanent excision discussed? If not, why (PVD, diabetic, etc)? There are other documentation keys, but the point is that when you are performing a red flag procedure, do it by the book.

October is Breast Health Awareness month and a good opportunity to give back to the community and show your support for those with a history of breast cancer.
 
Symbols Supporting Breast Cancer Awareness Month
 
Consider putting together a team from your office to participate in a local walk to support breast cancer research. A notice could be posted in the office asking patients for donations. There are several inexpensive items relating to breast cancer awareness on Amazon and Oriental Trading as giveaways to all the patients in your office. We had an artist put pink ribbons on the front door and side window in our reception area, which did not take much time at all and was inexpensive. In November, we will remove the one on the door and leave the other up year round.

I had Lasik surgery done several years ago. It didn't work well for me, but from everyone I talk to, I am the exception. Most people whom I have talked to over the years say it helped solve their dependency on glasses. For me, it did not. I wear glasses now, and did within several days of the surgery. I had it performed in a big city by someone who does hundreds and hundreds of them.  As long as I wear my glasses, I am fine.

Lasik surgery is not indicated for people who are very active and can be hit in the eye by a ball (squash or tennis or baseball) or by an elbow playing basketball. 15 years ago, I had my cornea reshaped conservatively without any surgery. From Paragon CRT, "it is a unique oxygen permeable therapeutic contact lens designed to temporarily correct myopia and moderate levels of astigmatism by gently reshaping the cornea. Paragon CRT lens is ideal for: patients with up to -6.00D of myopia & up to -1.75D of astigmatism."
 
I put the Boston-like lenses in at night before sleeping and took them out in the morning to put them into the cleaning and disinfecting solution. About every 4 weeks, I received another set of lenses to further reshape my corneas. Within 4 months, my vision went from 20:200 to 20:25. I did experience eye strain by the end of a busy day and ended up wearing +1.0 reading glasses. It's worth your consideration. It is a conservative corrective procedure.
