Foot Hygiene Often Lacking: NY & CA Podiatrists
By now, you probably think you have your shower routine down pat. And even though you likely never forget to scrub your underarms or wash your hair, there may be other extremities that don't get as much attention. The truth is, much like the rest of your body, your feet are covered in more than just dirt at the end of a long day. "The feet are the closest proximity to the ground, which means they easily pick up dirt and bacteria when walking around," Emily Splichal, DPM, a New York-based podiatrist, told Well+Good.
|
Drs. Emily Splichal and Robert Lee |
Robert K. Lee, DPM, chief of podiatric foot and ankle surgery at UCLA Medical Center, Santa Monica, told Self that bacteria on your feet "feed on waste products that come out of your sweat glands, and they produce odor as they build up." Podiatrists also recommend making a few changes to your overall hygiene routine to take better care of your feet, like making sure to wear clean socks made of moisture-wicking synthetic fibers, rotating your shoes every other day to allow for proper drying out, and considering orthotics if you're experiencing lingering issues.

RELEVANT RESEARCH |
Topical Oxygen Heals Hard-To-Heal Diabetic Foot Ulcers
Topical oxygen has been used for the treatment of chronic wounds for more than 50 years. Its effectiveness remains disputed due to the limited number of robust high-quality investigations. Frykberg, et al. recently published a study that has shown the efficacy of multimodality cyclical pressure topical wound oxygen (TWO2) home care therapy in healing refractory diabetic foot ulcers (DFUs) that had failed to heal with standard of care (SOC) alone. Patients with diabetes and chronic DFUs were randomized (double-blind) to either active TWO2 therapy or sham control therapy, both in addition to optimal SOC. The primary outcome was the percentage of ulcers in each group achieving 100% healing at 12 weeks. A group sequential design was used for the study with three predetermined analyses and hard stopping rules once 73, 146, and ultimately 220 patients completed the 12-week treatment phase.
At the first analysis point, the active TWO2 arm was found to be superior to the sham arm, with a closure rate of 41.7% compared with 13.5%. This difference in outcome produced an odds ratio (OR) of 4.57 (97.8% CI 1.19, 17.57), P=0.010. After adjustment for University of Texas Classification (UTC) ulcer grade, the OR increased to 6.00 (97.8% CI 1.44, 24.93), P=0.004. Cox proportional hazards modeling, also after adjustment for UTC grade, demonstrated >4.5 times the likelihood to heal DFUs over 12 weeks compared with the sham arm with a hazard ratio of 4.66 (97.8% CI 1.36, 15.98), P=0.004. At 12 months post-enrollment, 56% of active arm ulcers were closed compared with 27% of the sham arm ulcers (P=0.013). In conclusion, this sham-controlled, double-blind randomized controlled trial demonstrates that, at both 12 weeks and 12 months, adjunctive cyclical pressurized TWO2 therapy was superior in healing chronic DFUs compared with optimal SOC alone.

PRACTICE MANAGEMENT TIP OF THE DAY |
Top Tips for Succession Planning - Part 2
Positioning yourself and your practice for a smooth transition takes time and thoughtful planning.
2. Plan
-
The easiest answer as to when you should start succession planning for your practice is "always":
-
Almost all strategic and professional decision-making for the practice should be conducted with an eye toward long-term succession planning.
-
Remember, there's no such thing as "too early," but there is certainly such a thing as "too late."

MEDICAL-LEGAL TIP OF THE DAY |
Presenting Your Medicare Case to the ALJ - Part 13
32. Is it okay to read your answers to the judge? Absolutely not. As soon as you start reading, the judge will stop paying attention to what you are saying. The hearing is a conversation between you and the judge. Think about it. If you are debating a topic with someone and they start reading something from a piece of paper, are you still paying attention? Of course not. And the judge is not going to pay attention to whatever you are reading, either.
33. What is the difference between a perfect answer read to the judge and a conversational answer that is slightly incomplete? A conversational explanation that is slightly incomplete beats a stilted, read-from-a-piece-of-paper explanation every time.
34. How much emotion should you include in your oral presentation to the judge? None.

From: Brian Lee, DPM
I guess I'm old school. In my opinion, Foredom cable drills are still the best. I still have 2 that I've been using for over 30 years! They have much better low-end torque than Osada, even with their stepdown handpiece. Not as compact, but definitely less expensive.
