PRACTICE MANAGEMENT TIP OF THE DAY
How to Have a Well-run Practice - Part 12

The daily grind of seeing patients can sometimes be, well, grinding. It’s important to take a step back and reconsider how you do things. There’s likely another way to achieve the same, if not better, result, especially when it comes to staffing. Here’s a sample of what 943 practice owners, managers and physicians say has helped them create a better environment.

12. Treat your staff as your most valuable asset — because they are! Patients value consistency and recognize staff members when visiting the office.

RESPONSES/COMMENTS (RELEVANT RESEARCH)
RE: Can Cyanocrylate be Used as a Treatment for Hangnails?
From: Leonard A. Levy, DPM, MPH

Using cyanoacrylate (surgical glue) on hangnails is an effective treatment for hangnails affecting the finger, reports Jangar, RS, et al. in the Journal of the American Academy of Dermatology (Vol 81:5), November, 2019. Can it be as effective on toenails? They instruct that after cleaning the area, a small drop of the glue be applied to the involved area and allowed to dry. The glue sticks to the torn portion of the skin to the nail fold, preventing further extension of the tear to the dermis. This, the authors claim, enables the patient to perform daily activities without discomfort. After healing, the jagged portion tends to fall off naturally.

CODINGLINE CORNER
Query: Coding Subungual Hematoma

How would you code for hematomas at the great toes due to shoes? The patient has no date as to when it occurred and said just recently that the toes have started hurting. Is there an S code? Is there a specific injury code? We don’t know when or where it happened.

Miranda Hook, Witchita, KS

Response: Based upon the information that is provided in the post, the most appropriate ICD-10-CM codes for “hematomas at the great toes due to shoes (i.e., subungual hematoma) would be the following:
S90.211 Contusion of right great toe with damage to nail
S90.212 Contusion of left great toe with damage to nail

Of course, the appropriate 7th character would need to be added to the above ICD-10-CM codes.
A = Initial encounter and treatment
D = Subsequent encounter
S = Sequela

RESPONSES/COMMENTS (CLINICAL)
From: Michael Loshigian, DPM

I agree that additional views are necessary to fully evaluate the condition of this joint. Three standard and axial sesamoid views are necessary.

However, and with all due respect to Dr. Peacock, I don’t understand how anyone can make a determination from the image provided as to where the arrow is pointing along the dorsal to plantar dimension of the foot. This is not a radiographic marker but most likely an indicator added to the image electronically after the image was taken. What we can observe from the image provided is that the metatarsal head was not...

RESPONSES/COMMENTS (PM NEWS ARTICLES)
From: Amol Saxena, DPM

I agree that there needs to be a single board with possibly three sub-specialties, probably diabetic foot, sports, and reconstructive surgery. The problem is that the APMA and the schools promote the surgical aspects and all graduates coming out are supposed to be foot and ankle surgeons. There is too much variability in students, programs, and training. I see this more than ever as I am interviewing residents for my fellowship's 22nd year.

I've said all along that podiatry needs three things to survive: 1) Students need to pass the USMLE exam Part 1. 2) Podiatry needs to do research. 3) Podiatrists need to give back via donating time and money.

I think it is confusing to the rest of the world what podiatric "biomechanics" is. DPMs can't seem to say non-surgical (are we embarrassed?), so weird. It's like saying that the diabetic foot is "biochemistry."

RESPONSES/COMMENTS (PM NEWS QUICK POLLS)
RE: Are you getting a flu shot this season?
From: David Gurvis, DPM

While it is accurate to say the VAERS (Vaccine Adverse Event Reporting System) and the Compensation Program have paid out nearly 4.2 billion over it’s lifespan, it cannot be assumed these are all true vaccine-related injuries and to “suspect” otherwise borders on conspiracy theory and shows you have read superficially and have not really read the data.

A quote from that site: For example, one study of VAERS reports found that only 3% of the adverse events following immunization “were classified as definitely causally related to vaccine received.” People have their children vaccinated and later that week the child dies, unfortunately, say from childhood leukemia, or an unexplained event. They can report that to the VAERS and try to attach the compensation fund. Can a child be well on Monday and dead on Tuesday? Tragically yes. It happens. But not everything that transpired in the interim is “at fault”, including vaccines.

I follow the anti-Vax movement and they are extreme. They feel empowered to attach the fund for a simple red rash at the injection site. I do not wish to minimize the dangers, but the risk/ benefit ratios have been proven to fall on the side of the vaccines. This is the same risk/benefit we as docs mentally weigh every day we write a prescription or perform a procedure. Overall, vaccines save lives.
