Best Treatment for Hamstring Injuries is Prevention: CT Podiatrist
Hamstring injury may vary from minor strains, a small tear of the muscle, to possibly a complete ruptured and separation of the muscle. These injuries are very common to athletes who undergo sudden fast motions in their respective sports. According to podiatrist Dr. Robert Weiss, "The best treatment for these problems is prevention. The athlete must understand the muscle function in training."

A warm up of muscles is important in all sports. It is a means of raising the body temperature one and one half to two degrees above normal before starting any physical activities. "As body temperature rises, muscle and tendons become more lubricated and elastic. The nerve impulses become more effective in muscle fiber function, and the joints gain an improved range of motion for better function and less friction. A good exercise prior to all-out fast running sports is two 100-yard jogs; then stretching for about 10 minutes, working out all the major muscles, including push-ups and sit-ups for endurance," says Weiss.

100,000 Diabetics Unnecessarily Lose Legs in Amputations Yearly: Indian Podiatrist
“Nearly 62 million Indians suffer from diabetes. What is startling is the little-known fact that 100,000 diabetics unnecessarily lose their legs in amputations every year,” says A.P.S. Suri, diabetic foot surgeon and founding president, Indian Podiatry Association, New Delhi. Dr Suri also heads the podiatry department at Fortis C-DOC and is the director of the Diabetic Foot Care and Wound Care Centre, Delhi.

A survey of type 2 diabetics in Puducherry, published in 2014 in the Indian Journal Of Endocrinology, found that only 54% of the respondents were aware that diabetes could lead to reduced foot sensation and foot ulcers, and only 22% of diabetic patients had their feet examined regularly by a health worker or doctor.

Canadian Podiatrist Discusses Ingrown Toenails
“Ingrown toenails usually start off innocently enough,” says podiatrist Caroline Leverett. “At first, you might just notice a slight redness around the toe, mild swelling, and a sensitivity to pressure.” But she goes on to explain that, left untreated, it can get much worse. “As skin and tissue grow around the sharp point of the nail, the end of the toe may swell and redden. A pocket of pus might also develop, signaling a possible infection. Once infection arrives, you can expect increased pain, white or yellow pus oozing from the toe, and perhaps some blood. An unpleasant odor could also occur.”

As with most medical conditions, Leverett says, prevention is best. And there are a number of easy ways to prevent ingrowing toenails. “Always cut your nails to follow the natural shape of your nail, and don’t cut them too short at the sides; the corner of the nail should be visible above the skin. In addition to that, keep your feet clean and dry, avoid tight shoes, and use cotton socks instead of synthetics whenever possible.”

Haitian Immigrant Tries to Immigrate to U.S. to Become a Podiatrist
Wilenda Nicolas, 6 months pregnant, prepares a mattress on the floor for a little girl in a migrant shelter with some 100 other Haitians in Tijuana, a Mexican city bordering the United States. Nicolas and the girl are among thousands of Haitians who used to live in Brazil but have arrived in Tijuana in recent months, overwhelming immigration authorities on both sides of the U.S.-Mexico border.

Nicolas arrived in Brazil last year, working at the slaughterhouse for $200 a month. But she decided to go to the United States to pursue her dream of becoming a podiatrist, borrowing money from her family to pay for the trip. Like others, she was waiting in Tijuana for her turn to convince U.S. authorities to let her immigrate to the United States. Her day finally came last Friday. Under searing heat and with her backpack on one shoulder, she stepped on the border bridge with Damie and some 60 other migrants, whose cases will be reviewed by U.S. immigration officials.

| MACRA UPDATE | |
GOP Lawmakers Say MACRA Could Kill Small Practices and Fuel Consolidation
Republican lawmakers are warning the CMS that if its draft rule on the new physician payment framework is finalized as is, small practices may close their doors, leading to more consolidation and higher healthcare costs. A group of Republican physicians in Congress wrote to the CMS just weeks before a final rule is expected. When MACRA goes live in 2019, it promises bonuses for top-performing doctors and clinicians and penalties for underperformers on a variety of measures, especially quality of care.
The lawmakers urge the CMS to be flexible by lowering patient minimum reporting thresholds. The agency wants providers to report quality measures on 90% of their patients from all payers, and 80% of Medicare patients. Currently, only 50% of Medicare patients are required. Small providers would have a harder time obtaining the information technology and data needed to demonstrate that quality.

6 Ways to End a Doctor-Patient Relationship - Part 3
As doctors, we have all treated patients who were very difficult to deal with, due to their non-compliance, an abusive attitude, or just a mismatch of personalities. For most of us, there is no need to stay in a relationship that is clearly not serving the patient nor the doctor well. What is the best way to terminate a doctor-patient relationship?
3. You do not need to give a reason as to why you are terminating a patient. Most of them already know. I rarely do because that only leads to further arguments. If you tell the patient they were mistreating your staff, they will come back with a list of infractions your staff committed against them. I just leave the reason out and make it so the termination is clear. At the point I make this decision, there is no negotiation left. I truly consider all options before reaching this point and it is a point of no return. If you are not sure, don't do it.

To address Dr. Borreggine's and Dr. Rees' concerns regarding ADA Section 1557, the final rule has the same provisions as the HIPAA privacy rule, in that you must exercise a degree of reasonableness. The exact wording is: “Requiring covered entities to take reasonable steps to provide meaningful access to individuals with limited English proficiency. Covered entities are also encouraged to develop language access plans.”
No where does it prohibit a family member from interpreting for a patient. So, especially in a small practice, having all sorts of interpreting skills in-house would be unreasonable. If your practice has a large percent of non-English speaking patients of one particular language, then it would behoove the practice to have a staff member who can translate. I see it all the time in practices that have many Spanish-speaking patients, or Russian or Hindi ones; they have staff who can speak their language.
So your language access plan can be as simple as: "In the event that we cannot communicate with a patient in their native language, we will rely upon an individual who accompanies the patient in their care, or in the case where that is not feasible, we will make use of telephone/Internet translating service XYZ." In most cases, this will be an infrequent occurrence. Also the offense occurs only if the patient files a complaint, so the key is to try to accommodate the patient in a way that the patient is most comfortable with - again with reasonableness and dignity.

As I was reviewing ADA Section 1557, I came across this on the HHS website, which I would like others with more knowledge to comment upon. Is it possible that we in private practices might not be considered a “Covered Entity” according to the definition from the Office of Civil Rights, and hence be excluded from section 1557 compliance?
Civil Rights Obligations of Covered Entities - Nondiscrimination laws enforced by the Office for Civil Rights (OCR) prohibit discrimination and require covered entities to provide individuals an equal opportunity to participate in a program activity, regardless of race, color, national origin, age, disability, or (under certain conditions) religion or sex.
What is a Covered Entity? - A Covered Entity is any entity that receives federal financial assistance from the Department of Health and Human Services or is covered under Title II of the Americans with Disabilities Act as a program, service, or regulatory activity relating to the provision of health care or social services.
How do you know if you are not a Covered Entity under OCR jurisdiction in its enforcement of nondiscrimination laws? If you receive some type of federal financial assistance from another federal department or agency but it does not include any HHS assistance, you are not a covered entity under OCR’s jurisdiction. You may also not be such a Covered Entity if you are a healthcare professional who accepts only Medicare Part B insurance payment. (Bold and Italics added by me.)

Since we are on the subject, I want to remind everyone of the dangers of using Original Barge Cement. Barge is heat-activated, contact cement, which means you should be carefully using a heat gun to not only dry the cement, but to heat activate it. Remember the hoods in the chemistry lab - that's really what you should be using. I believe activated carbon will filter the volatiles, if you wear a breathing filter; but if you don't vent, everyone in the office is exposed. An alternative is to take it outside. You need plenty of ventilation to the outside, and may be required to "scrub" the exhaust, by local or state ordinances, or the EPA. This stuff is highly toxic to the liver.
