Tim Dutra, DPM of Oakland, California, ha been elected to the Board of Directors of the Joint Commission on Sports Medicine and Sciences (JCSMS). The JCSMS consists of leaders from various sports-related organizations that get together annually to form and develop their own network and liaison strategies.

Function and Heredity Contribute to Bunions: NY Podiatrist  
 
According to Dr. Robert A. Russo, President of the New York State Podiatric Medical Association (NYSPMA), bunions form when the normal balance of forces exerted on the joints and tendons of the foot become disrupted. "Bunions occur after years of abnormal motion and pressure over the MTP joint," said Dr. Russo.

"Bunions tend to run in families and can be caused by the way we walk and our inherited foot type." Dr. Russo advises anyone with foot pain or an unusual bump on the big or baby toe joint to seek podiatric treatment.

IN Podiatrists Sponsor Charity Run
 
Eleven-year-old Aleigha Sweet is a living, breathing, definition of courage. For the fourth year, Aleigha will be participating in the Annual Run for Riley, this Sunday at Maple Creek Middle School. This is the 7th annual Aboite Podiatry Run for Riley“It will be different this year. This is the first year she will be participating with cancer,” her father Tom Sweet said.

Over the past six years, the run has raised almost $77,000 for Riley's Hospital for Children. The organizers are hoping to break $100,000 this year. The event averages about 800 people, so organizers see this as a very doable goal. Race registration is $25 per person and thanks to Aboite Podiatry, that entire fee goes directly to the children at Riley.

HEALTHCARE NEWS
Thousands of CT Doctors Fired by United HealthCare
 
In the midst of major changes in healthcare, UnitedHealthCare has sent thousands of pink slips to Connecticut doctors. Termination letters went to physicians caring for Medicare patients. Those letters were sent out to doctors caring for 'Medicare Advantage' patients. It's a plan, marketed to seniors to provide additional services through UnitedHealthCare. A mix of primary care and specialty doctors are affected by it. And it comes at a questionable time. 
  
The Connecticut State Medical Society is fighting back. The biggest concern is patient access to healthcare. "What the government is looking for is to manage better care by adding a patient-centered medical home so that you have a doctor who is totally invested with taking care of every aspect of the patient and coordinating it. This is clearly not a patient-centered decision," said Dr. Michael Saffir, President of CT State Medical Society.

First, some caveats regarding the video. The patient is wearing clothing which obscures most of the lower extremities. There is no attention to ankles, knees, or hips (no zoom). There is no sagittal view. The patient can’t get close to normal gait until the third step. By that time, he’s halfway across the screen with only two frames to make some assessment. The bottom line is, the person who submitted these videos did not give us enough material to appreciate an “unusual gait”.
 
Both videos seem to present excessive transverse plane motion at the knees and in-toed gait. On video, the feet seem “C-shaped” or forefoot adducted. The history and video findings suggest Charcot-Marie-Tooth disease (progressive peripheral neuropathy).

The most important lesson to be learned from this posting is that it is impossible to make a diagnosis from a gait exam and some cursory neurological tests. This patient needs to be referred elsewhere. That said, there seems to be muscular/neuromuscular weakness on the right side, along with a functional short limb. The quadraceps and hip muscles are most likely involved. In the foot, I suspect anterior tibial and peroneal involvement. The lowered beltline on the right is a tip-off to the inclined posture.

Having an implant fracture is not that rare an occurrence. We have all seen screws, plates, K-wires, and in this case, nitinol implants fracture. These can fracture for various reasons: defective implant, early unguarded weight-bearing, trauma, or surgeon error in inserting the implant. 
 
In this case, both arms of the second toe implant and also a fractured arm of the third toe implant are noted. If there is mal-alignment or pain, the implants should be removed. The technique employed should be...

It is no surprise that the manufacturers have no response as they indicate in their literature that this has never happened. As with all implants, the user has the duty of care to remove such fragments as may be causing pain, but this may not be all the pieces.
 
My own view is that any manufacturer that has no mechanism for removal of implants should be rejected as a supplier. We all know from experience that whatever you place in a foot may have to be removed, and a method for removal should be an absolute requirement before use.

If you have a YAG laser, you need to either wear a mask during your treatment, have a vacuum system, or do both.  It is well known that with these types of thermal ablative lasers, there is a laser plume effect that effectively aerosolizes the fungal elements as well as bacteria. In addition to dermatophytes, micro-organisms have been isolated from this "plume" and include staph epi., pyrogenese, faecalis, bacillis and c. albicans. You don't want to inhale these. 
 
Besides not being able to accurately control the amount of heat and energy being delivered to the nail (in my opinion) and given other risks associated with a YAG laser (burns and hematomas) are why I have chosen to buy Noveon lasers for my offices. It is photobiologic, with no plume, and calibrated to always deliver the same quantity of energy to the patient.
