APMWA Announces 38th Annual Student Writing Competition
The American Podiatric Medical Writers Association (APMWA) has announced its 38th Annual Student Writing Competition.
-
All papers MUST be non-technical in nature and original. Appropriate subjects include practice management, ethics, law, education, healthcare, or any topic that would be suitable for a lay publication.

There is no word limitation. Papers will be graded for content, style, grammar, neatness, and overall impact.

First prize one thousand dollars ($1,000), Second prize seven hundred and fifty dollars ($750) Third prize five hundred dollars ($500) sponsored by an APMA Educational Foundation endowment from Dr. and Mrs. Steven Berlin, and recognition in the APMA NEWS and the APMWA Newsletter. Honorable Mention Certificates may also be awarded.

Defense Verdict in PA Podiatrist Malpractice Suit
The case stemmed from plaintiff John Benjamin’s allegations that Dr. Spencer Monaco failed to properly treat a degenerative condition in his foot called Charcot foot. According to court papers, Monaco performed an ankle surgery in late May 2021, which required external fixation. About six weeks later, Benjamin called the hospital concerned about a possible infection where a pin was located. The fixation was removed, and records reported no signs of infection.
For months, Monaco said Benjamin was making a good recovery, however, several months later Benjamin’s leg became severely swollen, and it was determined he had a severe infection. After some efforts to drain and clean the infection, Benjamin’s leg was amputated just above the ankle. The defense lawyer said his client made an excellent witness, who was direct on the stand, and that the jury also likely believed Benjamin’s Charcot foot condition was so advanced by the time he sought Monaco’s treatment that he likely he would have needed to have undergone the amputation regardless. The jury came back with a defense verdict.

Five Financial Reports You Need to See Monthly
5. Patient Balance "Trio Pack:
• Patient A/R. Generate this by patient account in descending balance order, not alphabetically, so that the biggest balances will be on page one. Ask for a status update on the top 10 accounts, every month. Train staff to call patients, not just send statements and letters, and establish payment plans using automated, recurring, payment-plan software. Be sure front-desk staff ask for past-due balances when patients come in for their appointment.
• Payment plan status. Be aware of those patients who are current and who are not. Train staff how to speak with and assist the noncompliant patients.
• Accounts recommended for collection or bad debt. Some patients simply can't or won't pay. Keeping them on the books for years is foolish financial management and frustrating for staff. Ask staff to prepare their recommendations on which patient accounts should be sent collections, and ask them to provide back-up details. Approve the accounts for bad debt or send them to collections, but get them out of the A/R.

Query: Weekly Dressing Changes
We have a patient who we see for weekly dressing changes due to severe edema, venous stasis ulcerations. and venous insufficiency. The patient has traditional Medicare and they paid the first date of service, but not subsequent visits.
Is there a modifier we can attach to the procedure code to show medical necessity? The code we used is CPT 29581. Medicare stated that payment is adjusted because the payer deems the information submitted does not support this many/frequency of services.

Response: First of all, there is no global period for CPT 29581 so the frequency should not be an issue. Nor should there really be a need for a modifier. It is a standard of care for weekly applications of such compression dressings for the diagnoses you mentioned.
I think you may want to take a look at the “information submitted” and see if the medical necessity is clearly spelled out. Did they even ask for a record review, or simply “denying to deny?” I would appeal, make sure your documentation clearly states the medical necessity, the standard of care, and why this is taking place. It seems to me to be a “deny to simply deny” event.
