AT THE COLLEGES
IL Podiatrists and Scholl Students Volunteer at the 2022 Chicago Marathon
 
The 2022 Chicago Marathon took place on Sunday October 9th. Podiatry was again well represented as an integral part of the marathon medical team. As in previous years, AAPSM fellows Jeffrey Baker, DPM of the Weil Foot & Ankle Institute and Michael Chin, DPM of the Running Institute were podiatric co-coordinators for the event.

Other attending Volunteers were Drs. Regan Bond, Larry Kosova, Katie LaViolette, Hannah Park, Lauren Schnack, and Alison Young. Volunteering as well were podiatric medical students from the AAPSM Scholl Chapter. The podiatry volunteers were stationed at two medical tents at the finish line area and provided medical care to 156 runners throughout the day.

Now there are patient-specific custom implants that are designed from CT scans of the patient. That means decreased operative room time and improved results."

Query: Tenotomy of the Posterior Tibial Tendon
 
My patient is a middle-aged male with diabetic neuropathy and peripheral vascular disease. Previous vascular intervention was successful in getting a wound closed and osteomyelitis resolved. In the process of resolution and healing, the wound care facility debrided the base of the 5th metatarsal and part of the cuboid which detached the peroneus brevis and removed the peroneus longus without re-attaching them. His foot is now severely inverted due to the unopposed posterior tibial tendon. Despite bracing and shoe modifications, his foot continues to progressively become more inverted. I try to employ the old risk/benefit ratio to these types of cases. A major reconstructive surgery, in my opinion, carries more risk than benefit for this individual. But one option that I am considering is performing a simple percutaneous ultrasound-guided tenotomy of the posterior tibial tendon which I think will make this foot much more amenable to bracing.
 
I have successfully performed percutaneous Achilles tenotomies to resolve forefoot ulcers in diabetics and have used other tendon balance procedures for dysfunctional feet due to other causes, but there does not seem to be a lot of literature on posterior tibial tenotomies as a tendon balance technique in diabetics. Comments and critique are welcomed.

RE: Stepping up Podiatry's Role in Fall Prevention

Many of us do not have to look far when it comes to the devastation that falls can have, particularly as we age. We all know the stats. The APMA just reminded us in their summer newsletter that 30-40% of adults 65 and older will fall. Falls are the leading cause of death related to injury in adults 65 and over, with an increase in morbidity and mortality after a hip fracture from an errant step. According to the CDC, $50 billion is the cost estimate to the U.S. healthcare system alone for the ripple effect of medical costs related to falls. This has only been made worse, no doubt, by the change in habitus that has occurred during the pandemic.
 
When my mother was young, she was graceful as an equestrian and diver. She skied by the time I came along but slowly succumbed to the steady decline that many Americans suffer. I would implore her to take a walk and get moving. I noticed that her walking became more unsteady over the years. She did not take me up on my “prescription” but I did get her to walk occasionally when I came over. I got a call one evening that my mom had fallen, broken her neck, and suffered a massive stroke.
 
Many times these tragic events happen with the cardiac or clot event preceding the fall but here’s the thing; John, her significant other, saw her hook her ankle on the low lying coffee table as she was carrying a tray of plates from the living room back to the kitchen. Perhaps if she had done some modest exercises at home, her fall may have been prevented; if nothing else, walking more regularly to try to control her weight and help her heart health.

I had the same issue. On a whim, I called a compounding pharmacist I know at Cornerstone Pharmacy in Little Rock, AR and provided her the ingredient list. I gave her the package insert for Cantharone plus and she was able to compound the near exact formulation (I think there is 1% difference in the canthiridin percentage). She provides it in 10 mL vials, with a glass stick in the cap. It works exactly the same as Cantharone Plus (as you would expect). It was even cheaper than what I was buying from the Canadian pharmacy.

I haven't noticed anyone complaining about completing a podiatric surgical residency and not being able to achieve board certification in podiatric medicine. It's only from podiatrists who don't match in surgical residencies who want to qualify somehow as board certified podiatric surgeons. Since 1986, I have been involved with institutions granting osteopathic or allopathic degrees to students who go through a match process similar but not identical to ours. I went through the first podiatric residency match in 1977 and this process was supposed to end the early signing of students into the most desirable programs and give everyone the opportunity to apply and perhaps interview for the more attractive surgical programs if that's what the student wanted.
 
If a fourth year MD or DO student wants to eventually become an orthopedic surgeon, he or she must apply to interview at the programs the student would like to train with. There is a screening process based on data all students submit. There is no guarantee...
