NY Podiatrist Provides Nail Care Advice
If you’re prone to getting ingrown toenails or have fragile toenails that constantly break, simply trimming them to a specific shape can help, according to Miguel Cunha, DPM, a board-certified podiatric surgeon and founder of Gotham Footcare. “It’s important to avoid cutting your toenails round or angled to prevent the formation of ingrown nails,” says Dr. Cunha. “Once the nails are cut flat and straight across, the edges on either side can be filed down so that the corners are less pointy and slightly beveled to follow the shape of the toe.”

“Toenails should all have roughly the same shape, color, and thickness. If something is abnormal in the appearance of the nails, it is important to determine the reason why by going to a foot specialist,” says Cunha. “It could be micro-trauma to the nails which commonly occurs among athletes, a fungal infection, or an underlying systemic condition such as psoriasis which can lead to the formation of psoriatic nails. Any of these things can alter the normal appearance of your nails,” he adds.

According to Robert Weiss, DPM, "The enormous increase in the number of people who exercise suggests that exercise offers more to an individual than mere psychological benefits, especially during this COVID-19 era. In an individual there is a clear relationship between mind and body. There is some research which suggests that exercise programs may be useful in treating many physically and mentally strained conditions."

"It has been shown that exercise reduces the symptoms of emotional distress in individuals. This would include addiction to alcohol and other drugs, sleep disturbances, over-eating, and smoking. A proper exercise program may inspire these people with an opportunity to set their goals. When these goals are understood and reached, it can generate a sense of pride, accomplishment, and self-esteem, thereby making it easier to love oneself," says Dr. Weiss.

St. Mary’s Healthcare welcomes podiatrist, Alpha Opese, DPM to its newly remodeled podiatry health center. Dr. Opese completed his Doctorate of Podiatric Medicine from Kent State University, in Cleveland, Ohio. He then completed an intensive three years of PMSR/RRA (podiatric medicine and surgery residency with reconstructive rearfoot and ankle surgery) from Our Lady Lourdes Memorial Hospital, in Binghamton.

FL Podiatrist Retires After 4 Decades of Treating Athletes
Podiatrist Timothy Runyon, DPM spent four decades treating the feet of the Tampa Bay Rays, pro wrestlers, Olympians, mayors, and thousands more. St. Petersburg Mayor Rick Kriseman on Oct.1 issued a proclamation in Dr. Runyon’s honor, the day of his retirement. It deemed that in the city, the month of October would be a celebration of Runyon’s career, with the mayor highlighting the doctor’s “academic excellence and servitude to both his profession and community.”

Early in his career, he worked with the Chicago Bears' team podiatrist, practicing on the sidelines of Soldier Field. In St. Petersburg, he built his reputation by volunteering in the medical tent at high school track meets. Between his local patients and those he saw during 20 years of mission trips to South America, he estimated he treated some 200,000 patients.

Cancer Wellness Center (Named After OH Podiatrist) Moves
Stewart's Caring Place has moved from a cramped rented space in Fairlawn to a newly built location in Copley, nearly double the size of its previous site. The non-profit cancer wellness center, which offers free services and programs for cancer patients and their families, outgrew the 7,400-square-foot space it previously rented.

Stewart's Caring Place opened in 2004 and is named after Dr. Stewart Surloff, an Akron podiatrist who was diagnosed with lung cancer in 2001. The center opened through an endowment left to the Jewish community when he died.

The New Model of Providing Care During the COVID-19 Pandemic
Lee Rogers, DPM, et al. recently published a paper in the journal Wounds titled "The New Model of Providing Care During the COVID-19 Pandemic." Here are their conclusions: "During this crisis, ingenuity is required to find solutions that will work to care for patients with wounds while minimizing their exposure risk. The goal is to provide treatment to these patients while they are safe and at home. Patients with wounds fit several of the high-risk criteria for COVID-19 infection and mortality. They are often older, obese, and immunosuppressed, as well as have multiple co-morbidities."
"The answer is not to avoid clinical evaluation and wait for wound infection, but to mitigate the risk of COVID-19 and ulcer infection. Governments and payers have given providers new freedoms to care for their patients in lower-risk settings. As providers adjust to the new model of care during the pandemic, the wound center without walls (WCWW) concept can help manage patients through the continuum of care in all settings."

Presenting Your Medicare Case to the ALJ- Part 12
31. What will the judge ask you? Assume the judge has read your brief before the hearing. And the judge will read it if the letter part of the brief is no longer than three pages. Then pretend you are the judge and make a list of the questions you would ask a provider/supplier appearing before you regarding the same denial. This isn’t tricky. The judge wants to know if your service was “medically reasonable and necessary,” whether your service was “experimental or investigative,” and why she should find in your favor if the service was prohibited in the LCD. Therefore, you want to prepare for the hearing by writing out short answers to the following questions:

Why was the service medically reasonable?

Why was the service medically necessary?
-
If the contractor said the service was “experimental or investigative,” why do you say it wasn’t?
-
If the contractor’s LCD prohibits the service you provided, why should the judge allow it? You also want to make a list of every other denial reason offered by the contractor or the QIC, and you want to write out a short response to each of those denial reasons.

Query: Steroid Flair Diagnosis
Is there an ICD-10 code that would be appropriate for a steroid flair following injection? Symptoms post-injection would be increased discomfort and localized swelling at the injection site with increased temperature.

Response: The so-called “cortisone flare” reaction has been reported to occur in approximately two percent of patients. This occurs when the injected cortisone crystallizes and causes pain worse than before the shot. Fortunately, these “flares” usually do not last long and resolve with icing after 12 to 48 hours. I do not know a distinct ICD-10 for such an instance but the closest might be: T50. Z95A – Adverse effect of other vaccines and biological substances, initial encounter.
In my clinical experience, those patients somehow always do the best with the injections, likely a hyper-immune response to the material.

Query: CARES Act Provider Relief Fund
I and another podiatrist are independent contractors in private podiatric practice, owned by a semi-retired podiatrist. Our work is in the clinic and long-term care facility settings. We pay the owner a fee for staffing, management, and space, based on the individual collection. Although our patient volume has been reduced by about 30% during the pandemic, the staff has been working full-time. The staff bought their own masks and face shields. In April, we each received the HHS stimulus payment, some more than others. The questions are:
1) Should we give part of this payment to the owner of the practice? He received a small amount of HHS funds and may have received a PPP loan.
2) If we give part of this payment to the owner, how do we report this use of funds to HHS?
