AT THE COLLEGES
PA Podiatrist Establishes Scholarship at TUSPM

Richard Stuempfle, DPM, a local podiatrist, recently made a $50,000 gift to Temple University College of Podiatric Medicine to create the “Dr. Richard G. Stuempfle 55 Endowed Scholarship Fund.”

Dr. Richard Stuempfle

This scholarship will be awarded annually to a fourth year student enrolled at Temple who has demonstrated a combination of outstanding academic achievement and financial need. This is not Dr. Stuempfle’s first gift to the school, but this one will forever be a part of the institution.

PODIATRISTS IN THE NEWS
Fungus is Most Common Cause of Peeling Feet: NY Podiatrist

Your feet aren't exactly pleasant to look at, but rough, peeling skin makes them look that much worse. "Peeling feet is definitely common, and there are various reasons why," says Rebecca Pruthi, DPM, a doctor of podiatric medicine and owner of Foot Care of Manhattan. "We're on our feet all the time and the constant pounding and walking can cause cracking and scaling of the skin," she says.

Dr. Rebecca Pruthi

According to Dr. Pruthi, the most common reason patients have peeling feet is a fungal infection like athlete's foot, but they might not always realize it. "A lot of times it just presents itself as peeling skin and patients don't have the itchiness, so they don't know it's a fungal infection," Dr. Pruthi says. So, if your skin starts mysteriously peeling all of a sudden, get it checked out sooner rather than later. Once you have a fungal infection on your skin, it can easily infect your toenail, which Pruthi warns can be very difficult to get rid of.

PODIATRISTS OVERCOMING ADVERSITY
CA Podiatrist Survives Breast Cancer

Dr. Katrina Di Pasqua exudes cheerfulness and warmth and a confidence that would put a patient at ease. The 55-year-old, Brooklyn-born doctor of podiatric medicine says she wanted to go into medicine for as long as she can remember while growing up in New York City. She got into podiatry through a stint working in orthotics and prosthetics: artificial limbs. For the successful medical professional, wife and mother of two teenage children, life was very good. Then last year, she heard the terrifying word: cancer.

Dr. Katrina Di Pasqua

“After a routine mammogram, they called me and said I needed to come back in,” she said. The follow-up tests confirmed the worst. But the news wasn’t all bad: She was only in stage one, early in the disease's progression. Another piece of good news, she was negative for the BRCA gene, the gene indicating a hereditary disposition to breast cancer. And fortunately, her experience with the chemo treatments was not bad. She is finished with chemotherapy and radiation, and will be officially finished with her treatment in December.

ON THE INTERNATIONAL LECTURE CIRCUIT
CA Podiatrist Keynotes Latin American Diabetic Foot Symposium

Dr. David G. Armstrong, Professor of Surgery at the Keck School of Medicine of University of Southern California and Director of the Southwestern Academic Limb Salvage Alliance (SALSA), provided the first day keynote address to hundreds of delegates from throughout Latin America.

Dr. David Armstrong with Summit Chair Dr. Fermin Martinez Dejesus.

The "Second Summit of the Americas for the Diabetic Foot" was held this week in Mexico City, and featured speakers from a dozen nations and three continents. Armstrong's lecture focused on lessons learned in team building worldwide... specifically on the "toe and flow" model that he helped to popularize along with his colleague Professor Joseph Mills.

MEDICAL PRODUCTS IN THE NEWS
FDA Clears Glucose Monitoring System that Doesn't Require Fingersticks

The Food and Drug Administration has given its clearance to the first continuous glucose monitoring system that can be used to make diabetes treatment decisions in adult patients without calibration using a blood sample from a traditional fingerstick test. The FreeStyle Libre Flash Glucose Monitoring System, made by Abbott Diabetes Care, uses a small sensor wire inserted below the skin’s surface to measure blood sugar levels.

A mobile reader above the sensor wire enables users to determine if these levels are too high or too low. The reader stores 90 days of glucose data, displaying the user’s current reading, the latest eight hours of continuous glucose data, as well as a trend arrow showing if glucose levels are going up, down, or changing slowly.

PRACTICE MANAGEMENT TIP OF THE DAY
Things to Consider When Retiring from Practice

4. How will you spend your time in retirement?

This is more essential for physicians than other professions. Medicine is often a personal calling. Many physicians have foregone hobbies and personal time in exchange for clinical training, practice building, and patient care. A gerontologist friend once told me, "retiring from medicine without retiring to something is risky." And he added, "For many of us, medicine has been a jealous mistress. We've missed many family events because of patient responsibilities." Study data indicates that for many physicians, their patient relationships are what they value most professionally. When those interactions and "thank-you's" no longer exist, there will be a void to fill.

CODINGLINE CORNER
Query: Medicare Claims Data Rates

A consultant looked at our Medicare claims data and said our collection rate is in the mid 80s for collection of Medicare allowable when it should be 93%. Can anyone tell me if this figure of 93% is what the average practice collects, or what else it may represent?

Jennifer, Office of Charles Perry, DPM, Cambridge, OH

Response: There is no magic number when it comes to a collection rate or ratio. The variables are immense when looking at these kinds of ratios. For instance, you may have a high percentage of Medicare-Medicaid patients, or you may have a very high number of patients without a secondary or supplemental insurance coverage secondary to Medicare. That will definitely lower the collection rate. It will also vary depending on the specialty, as some specialties render a higher number of “not medically necessary” services (those services that are truly necessary for the care of the patient but Medicare considers them to fall within the medically unnecessary). Is your consultant correct, though, that your collection ratio of Medicare allowable is too low?  It’s very possible.

Perhaps your consultant identified that your staff is not collecting the way they should at the time of service. Perhaps, they noted that your staff is not using the Medicare ABN properly and collecting (again – at the time of service). Perhaps, you are not accepting credit cards for the deductible or giving your patients the option of writing their credit card number on the statement you send them. There are too many facets of good medical collections for anyone to be able to say you should or should not have a 93% allowable collection rate based on your question. I’ve seen some with a collection ratio in the upper 90 percentile and some doing a fantastic job in getting an 80% ratio due to their location, demographics, local economic condition, insurance mix, etc…and I’ve been teaching this since the mid-80s.

RESPONSES/COMMENTS (NON-CLINICAL)
From: Burton Katzen, DPM

I bought a simple pay as you go (10c per minute) phone with a different phone number from my cell. I log in all my surgery patients on that phone with phone number, procedure abbreviation (ex.HV), and date of procedure, so I'll know who they are and what I did when they call..

I only call patients from that number so they don't have my personal cell. I keep it on on weekends (I do the majority of my larger surgeries on Fridays), and call them Saturday from that number. Monday through office hours Friday, I call forward that number to my personal cell so I don't miss calls, but they still don't have my personal cell

I always tell the patients they have my cell number, and if they call on the weekend, thanks to caller ID, I say "Hello Mrs. ____. They absolutely love it! They think they have my cell, don't have to go through an answering service, and they get me right away. Total cost -  $50-$75 per year.
