IL Podiatrists and Scholl Students Volunteer at the 2023 Chicago Marathon
The 45th Chicago Marathon took place on October 8th, 2023. Podiatry was again well represented as an integral part of the marathon medical team. As in previous years, AAPSM Fellows Jeffrey Baker, DPM of the Weil Foot & Ankle Institute and Michael Chin, DPM of the Running Institute were podiatric co-coordinators for the event.

Other attending Volunteers were Drs. Regan Bond, Larry Kosova, Katie LaViolette, Trevor Smolinski, and Alison Young. Volunteering as well were podiatric medical students from the AAPSM Scholl Chapter. The podiatry volunteers were stationed at two medical tents at the finish line area and provided medical care to over 100 runners throughout the day.

Five Financial Reports You Need to See Monthly
There are five financial reports that physicians should review no later than the tenth day of every month, for the previous month's activity.
4. Credit Balances - Credit balances are a financial liability. If they are accurate, you must pay them back to patients or insurers. A six-doctor group thought they could avoid this "unless someone asked," so they did not run a credit balance report for years. When they finally did, it was 65 pages long and had about $74,000 in credit balances - all of which had to be researched, verified, and refunded.
Review and approve the credit balances that staff have researched and verified every month. This report should be as close to zero dollars as possible.

When in practice, I often found that these painful lesions often had an underlying cystic structure that contributed to the pain more than the overlying concentrated keratosis. A differential can often be made when there is more pain on lateral compression than on direct pressure. Under minimal local anesthesia, I would make a small circular incision with a mini blade which would allow me to easily insert a small curette and scoop out the underlying cyst intact.
Healing was very quick. Immediate weight-bearing was possible; even more comfortable than before. There were some recurrences and long-term weight redistribution was beneficial. But the procedure was often successful long-term and minimally invasive.

What makes this discussion very interesting is that even though Drs. Cozzarelli and Jacobs disagree with each other, they are both correct. On one hand, Dr. Jacobs is correct. Krystexxa treats gout by lowering urate levels where allopurinol and others have failed. On the other hand, Dr. Cozzarelli points out that Krystexxa is marketed as a treatment for refractory gout and all podiatrists treat gout when it manifests itself in the foot.
The debate over whether podiatrists should be treating gout with xanthine oxidase inhibitors is an old one, but things have changed. With the exception of Krystexxa, the average DPM can easily learn how to prescribe the orals. It is very unfair to compare it to the management of diabetes which requires far greater clinical training.
When I first entered practice in the late ‘70s, when MDs owned their practices, it was common for podiatrists to treat acute gouty flare-ups and send the patient to internists for the urate-lowering drugs. In the past five or more years. internists, especially those working for corporations, resent when we send patients back to them for allopurinol or Uloric. When I lecture on the management of gout, I survey the audience. Half treat the hyperuricemia and half do not. Neither side is wrong.

In no way was I inferring that a podiatrist should take over the responsibility of treating a systemic condition such as gout. I agree with Dr. Jacobs 100%. However, diagnosing, providing appropriate labs, and providing relief for a patient with acute manifestations of the affliction is well within our scope of practice. And so is, obviously, an appropriate referral to an MD or DO physician. A podiatric physician should be familiar with all forms of medication and treatment available to patients; but the pharmaceutical houses should likewise be aware of, and inform the public of, who can provide or refer those services.
I also understand Dr. Gurnick’s consternation. It can be very frustrating when podiatry is left out of the equation entirely. Keep in mind that Krystexxa is an IV medication. The very fact that a podiatrist received information on the medication, will allow the podiatrist to discuss in an intelligent manner the pro’s and con’s of the medication if he or she is asked.

Sadly, I must respond to the sophomoric comments recently made in PM News regarding my comments on Krystexxa. Krystexxa is utilized to lower serum uric acid and for the treatment of chronic tophaceous gout. The goal of treatment is to lower the uric acid to less than 6. It is not used to manage acute gout. It is a drug which typically requires multiple IV administrations. It is not a drug typically utilized by podiatrists. The comments made were a classic example of not knowing what you do not know.
I did not suggest that podiatrists do not or should not treat acute gout or in fact any acute monoarticular disorder. Rather, if you would read my comment rather than embark on an inaccurate diatribe, my point is that Horizon does and has participated in podiatry education. They do not appear to ignore podiatry. However, they accurately conclude that Krystexxa is a medication appropriately employed by rheumatologists in the majority of...
