NY Podiatrist Discusses Edema
When your feet are so swollen it hurts to even put your shoes on, that feeling of panic might kick in. The good news is that in most cases, it's nothing serious. Podiatrist Dr. Jacqueline Sutera explains common reasons why your feet might look and feel more balloonish than usual and offers tips on how to treat the swelling. If you've been traipsing around all day, your busy schedule might be the culprit. A long day of standing or walking can lead to swelling in your lower extremities, says Sutera. Take regular rest breaks to give those puppies a breather, and keep them elevated, ideally above the height of your heart to get your circulation flowing again. The good news is that the swelling should go away overnight while you’re sleeping, says Sutera. Wearing tight shoes or gaining weight (especially during pregnancy) can also cause swelling.
|
|
Dr. Jacqueline Sutera |
If both of your feet remain swollen, however, you could have a circulation disorder such as varicose veins, a venous insufficiency — your veins aren’t bringing blood back to your heart, so it pools in your legs— or rarely, a blood clot, says Sutera. Make an appointment with your doctor if your swelling doesn't seem to be related to any of the above factors.

TN Podiatrist Running for Re-election to Signal Mountain Town Council
Dr. Robert Spalding has been on the Signal Mountain Council since 2014. He is currently up for re-election. Dr. Spalding is an original founder of the Walden’s Ridge Emergency Services and is a Board Member of those services. With the effort of Dr. Spalding and the Signal Mountain Council, in order to remain competitive with other communities and prevent the turnover of these personnel, the salaries of trained police, and fire personnel, was raised for the first time since 2007.
|
|
Dr. Robert Spalding |
Thanks to Dr. Spalding’s hard work with the Hamilton County Emergency Management Agency and the Red Cross, there is now a permanent Red Cross disaster trailer located on Signal Mountain. For the first time, Signal Mountain now has a county wide EMA/Red Cross blanket insurance policy for the three designated shelter systems within the mountain schools.

IN Podiatrist Estimates $65,000 Savings by Using Omega3-Rich Fish Skin for Wounds
Kerecis, a company using fish skin to heal human wounds and tissue damage, will present results of seven studies of its technology at the Symposium for the Advancement of Wound Care (SAWC) Fall meeting. One of the studies compared the cost of treating 27 diabetic foot ulcers with the Kerecis Omega3-rich, fish-skin graft with the cost of providing the standard-of-care treatment to the patient group. The study, which was conducted by podiatrist Dr. Chistopher Winters at the American Health Network in Indianapolis concluded that the fish-skin treatment saved an estimated $65,000.
|
|
Dr. Christopher Winters |
Skin from animals is not an ideal substitute for the patient's skin because heavy processing is needed to eliminate the risk of disease transmission. This harsh, anti-viral treatment removes most of the material's natural components, reducing its effectiveness. Fish skin, however, does not need anti-viral treatment and is therefore much more similar in structure to human skin than the anti-viral-processed skin substitutes. Because there is no risk of disease transmission, fish skin is only minimally processed, retaining its similarity to human skin.

| PM JURY VERDICT REPORTER | |
Alleged Unnecessary Plantar Fasciitis Surgery - Alleged CPRS (NY)
Facts: The plaintiff, in his 50s, contended that the defendant podiatrists at the defendant podiatric practice negligently failed to adequately attempt a conservative course before performing a plantar fasciotomy and heel spur resection. The plaintiff contended that due to the surgery, a portion of bone fractured, and that he developed more intense pain. The plaintiff maintained that CPRS that was affecting the foot area was confirmed by a finding of color changes and diminished pin prick sensations.
The defendant maintained that a full course of conservative treatment had failed, that because of severe pre-surgical pain, the plaintiff had greatly desired the surgery, and contended that it was appropriate to proceed with surgery. The defendant maintained that the plaintiff suffered known risks of a portion of the heel fracturing. The defendant also denied that the plaintiff developed CPRS.
Result: The jury found for the defendants.

6 Ways to End a Doctor-Patient Relationship - Part 2
As doctors, we have all treated patients who were very difficult to deal with, due to their non-compliance, an abusive attitude, or just a mismatch of personalities. For most of us, there is no need to stay in a relationship that is clearly not serving the patient nor the doctor well. What is the best way to terminate a doctor-patient relationship?
2. Do it in writing. Patients may not take verbal terminations seriously. We may tell a patient they need to find a new doctor, but it should be followed up with written notification. There should be no doubt as to the decision being made.

| RESPONSES/COMMENTS (NON-CLINICAL) - PART 1 | |
RE: ADA Section 1557
From: Joseph Borreggine, DPM
As of Oct 16, 2016 ADA section 1557 goes into effect. According to this section, medical offices, healthcare facilities, and hospitals cannot any longer impede a patient based on race, creed, language, sex and/or sexual orientation from receiving medical care. Hence, "cultural competency" is now being mandated by the federal government to force "political correctness" in healthcare delivery and no longer make it an option.
I have done research on this section and how it needs to be implemented by all healthcare providers and hospitals. It is quite arduous and will be costly more for the...

| RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A | |
From: Richard Rees, DPM
I am interested in how many doctors' offices are going to be compliant to HHS' new rule on interpretation requirements for each office on October 16th. For small offices such as mine, this could be a financial death sentence.

RE: Podiatry Practical Tip Of The Day
From: Keith Gurnick, DPM
Who doesn't do some form of orthotic adjustments in their offices? Do you ever have difficulty opening or unscrewing the Barge can after the used can has sat for a while? Does your can of Barge always seem to be glued shut when you need it in a hurry for a quick adjustment?
|
|
A simply solution to a "sticky" problem. |
This is due to cement that gets in between the nozzle and the cap and then dries out. There are many techniques to "unstick" or open the can when this happens. This can include using a hand-adjustable pipe or grip wrench, but this might put a permanent crimp in the cap from squeezing so hard. Sometimes banging on the cap will...
Editor's note: Dr. Gurnick's extended-length letter can be read here. PM News readers are invited to send in their practical podiatry tips. We will be awarding a $100 Macy's gift card to the best tip sent in during this contest period.
