Five Financial Reports You Need to See Monthly
There are five financial reports that physicians should review no later than the tenth day of every month, for the previous month's activity.
3. Adjustment. Also known as "write-offs," adjustments are the detailed categories to which staff post the amount between an actual payment and your standard fee. These include contractual adjustments and multiple procedure discounts, of course, but also adjustments for denials (such as patient ineligible or a coding error), and A/R clean-up actions (such as bad debt and small balance adjustments).
The report should be several pages long; adjustments are supposed to be granular. If your report contains broad categories such as "miscellaneous" or "insurance write-off," that's a problem. Ask your manager to explain vague sounding categories, as well as large dollar amounts, and request the addition of more detailed adjustment categories if necessary.

Query: Challenging Scope
I have a patient with a posterior talar osteochondral defect (OCD) and periarticular osteophytes on the anterior lip of the distal tibia. My plan is to address the talar OCD arthroscopically through posterior portals and the anterior osteophytes through standard anterior arthroscopic portals. Any thoughts on this unusual combination of procedures. I have been considering CPT 29891. Any other thoughts? Is there a better code or combination of codes?

Response: I agree with CPT 29891 (Arthroscopy, ankle, surgical, excision of osteochondral defect of talus and/or tibia, including drilling of the defect) for the initial portion of the surgery addressing the talar OCD. I would consider CPT 29894 (Arthroscopy, ankle, surgical; with removal of loose body or foreign body) to address the anterior osteophytes.
The NCCI edits indicate that CPT 29891 and CPT 29894 are not bundled. This patient’s ankle has two separate pathologies, even found at different locations. These two separate pathologies required the surgeon to actually address the ankle from two different approaches (including change from prone to supine). I have no qualms if the surgeon bills for CPT 29891 and CPT 29894.

I recommend that you excise them with a punch biopsy tool. I’ve done this many, many times, and it works very well on the recalcitrant ones. Of course, you can say it’s non-covered and have the patient pay any fee you want. However, you may not use CPT 17110 - chemical destruction of a wart- unless you feel that it is a wart. I know that there are some studies out there that say that IPKs are warts. Sometimes you don’t know, so I say do a biopsy.

Dr. Jacobs' comments on Krystexxa's FDA indication are not accurate. He references that it is indicated for the treatment of hyperuricemia.
Direct from the Patient Package Insert "INDICATIONS AND USAGE KRYSTEXXA® (pegloticase) is indicated, for the treatment of chronic gout in adult patients refractory to conventional therapy. Gout refractory to conventional therapy occurs in patients who have failed to normalize serum uric acid and whose signs and symptoms are inadequately controlled with xanthine oxidase inhibitors at the maximum medically appropriate dose or for whom these drugs are contraindicated.
Limitations of Use: KRYSTEXXA is not recommended for the treatment of asymptomatic hyperuricemia."

With all due respect to Dr. Allen Jacobs, podiatrists are often the first to diagnose acute gout, and we are often the doctors who treat acute flare-ups and chronic gout as well. I have come in to my office many times after regular office hours and on a Saturday or a Sunday for a gout emergency, and it is one of the most helpful and thankful things we can do when a patient is having an acute gout attack or flare up to help reduce or alleviate their severe acute pain disability and suffering.
Yes, it is true that we as podiatrists also make appropriate referrals to other specialists for systemic long-term management. However, we are often the first and best educators to our patients about the causes of gout, the short-term and the long-term affects as well, including localized effects in the foot and ankle and the long-term effects of poorly or uncontrolled...
