WA Podiatrist Discusses Plantar Fasciitis in Nurses
 
Although the right shoes can ameliorate certain problems, sometimes foot pain is caused by biomechanical issues. One common condition nurses experience is plantar fasciitis, or inflammation of the thick band of tissue that connects the heel bone to the toes, said podiatrist Julia Overstreet, DPM, director of the American Foot Care Nurses Association in Bellevue, WA.

A tell-tale sign of plantar fasciitis is stabbing pain in the heel with the first few steps in the morning, and the pain may return after long periods on the feet. The discomfort is usually the result of pressure placed on the plantar fascia. Wearing arch supports for four to six weeks can often remedy the problem — as long as nurses wear them at all times except in bed or in the shower, Overstreet said.

Closed Shoe Can Exacerbate Athlete's Foot: TN Podiatrist
 
Athete's foot commonly starts with a flaky rash on the outermost toes which could also spread to the soles, toenails, or the sides of the feet. Peeling of the skin can occur along with itchiness and a stinging sensation. Wearing a closed shoe, accompanied with sweat, can lead to these symptoms. "When you put your foot into a closed shoe, it really acts like a cooker because it's a warm, dark, moist area, so it actually cooks your foot," said Mark Hinkes, DPM, a podiatrist.

But one is also at risk of infection if he or she shares socks and shoes, walks in public places barefoot, has a foot injury, etc. Hinkes recommended seeing a podiatrist to make sure the underlying cause is not diabetes or weak immunity.

TX Podiatrist Presents Research in Berlin
 
Lawrence A. Lavery, DPM, Professor in the Department of Plastic Surgery at the University of Texas Southwestern Medical Center in Dallas, recently presented research data about diabetic foot ulcer recidivism of the Diabetic Foot Study Group (DFSG) in Berlin, Germany.

The DFSG is one of the specialty meetings as part of the European Associates for the Study of Diabetes. The DFSG is an international group of physicians, surgeons, therapists, nurses, and researchers discussing innovations, treatments and evolving technology to treat diabetic foot complications.

Negotiating and reviewing contracts is probably one of the more unnerving tasks physicians face. That’s good! Your subconscious is sending you “danger” signals. You simply need to channel this emotion into action to protect yourself.
 
1. Sales representatives are very good at selling the benefits of their company’s product to you and your practice, with emphasis on how these benefits outweigh the cost. Essentially, the sales meeting is all about what happens if everything goes as planned. You should never agree to the deal as presented until you are provided a contract and understand the terms thoroughly. The contract spells out what happens when things go wrong.

When I was in the Northeast, I utilized the services of ESWTUSA to provide shockwave therapy for my patients; however, they do not service Florida. Any recommendations for ESWT vendors who will bring the machine to my office and provide treatment for patients?

A few months ago, I asked about using Office Ally for a clearinghouse. I have been using it since then and am very happy with it. They are free only if less than 50% of your claims are for Medicare/Medicaid. If you send more than 50% Medicare/Medicaid claims, they charge you about $20 for the month. Not bad at all. My old service was charging $240 a month. 
 
The tech support with Office Ally is very good - better than the $240 a month service I was using. Claim reports are clearer and easier to view. I have not yet had any problems relating to the clearinghouse function. Every claim went through as expected and even the process of uploading a claim file is much quicker. It is worth taking a look at to save anywhere from $2,640 to $2,880 a year (depending on Medicare/Medicaid volume).

Sorry folks.. I am not a manicurist. Debridement of toenails is a valid medical issue but the fingernails do not (qualify). People can go to a manicurist for their fingernails. Are you going to file and buff them as well?

Periodically, we get a request to trim a patient's fingernail that's split, thick, deformed/mycotic and if it just needs trimming, we take care of this as part of their visit-at no additional charge. When we notice that a patient's fingernails need trimming, as they are very long and they can't do it themselves-again we will do this at no additional charge. When we see a patient with dirty glasses...we clean them. If they need to have their shoes and socks taken off and put back on-no problem.  
 
We are in the business of helping people and we need to think of these things as just part of taking care of our patients. They appreciate it and this goes a long way towards creating happy, trusting patients who would never think of going anywhere else or referring friends/family to anyone else for their foot care.

While I completely appreciate and understand Dr. Block's measured and legal response to this query, part of me is in complete agreement with him but the other part is rolling my eyes at this. While a podiatrist is limited in scope (which does not include the trimming of fingernails), and medical malpractice does not cover these treatments, please don't tell me that a medically trained podiatrist who went through 3 years of medical and surgical training who is now a doctor and/or a surgeon would say no to an incapacitated and frail senior citizen who asks you to help them cut their fingernails.
 
Back when I made home visits, I did it whenever I was asked. In my heart I could never say no. In my private practice, I have stroke patients, or patient afflicted with Dupuytren's who are unable to do it and have nails that are on the verge of breaking the skin of their hand and their wife or husband would ask (sometimes plead) for it to be done. How could you possible say no. At least I could never and still don't. Anyone ever treat a wart on one of your patient’s fingers? 
 
Was I out of scope and was I opening myself up to the possibility of a malpractice case when they could have gone on to a massive fingernail infection and lose their finger or arm? Yes, I was, but my odds of hitting lotto or being struck by lightning were better than that. If you fear this, then just don't do it.
