MI Podiatrist Discusses Biofilm and Wound Healing
 
According to Conway McLean, DPM, "When an ulcer remains open, numerous types of bacteria will “set up shop,” colonizing the surface of the lesion. These often don’t invade the tissues of the host, which would mean infection was present. Instead, multiple types of bacteria will form a cooperative colony, encased in tough, thick gelatinous material produced by the microorganisms. These colonies are referred to as biofilm, a common concern with implanted hardware as well as chronic wounds. Biofilm is a frequent obstacle to healing, altering the chemistry of many wounds, making it difficult for the body to grow tissue to fill the defect. Fighting biofilm is challenging. It’s also the reason many ulcers fail to heal."

"A newer weapon in the battle against biofilm formation is a specialized medication, applied in a sheet of collagen, whose sole purpose is the killing of biofilm. When left to persist, biofilm can ruin the best wound care plan. Along with the biofilm fighter,  it's possible to use a series of treatments to the surface of the wound utilizing an ultrasonically-charged mist. Studies reveal the benefits of this therapy are many, including killing bacteria, improving blood flow, even breaking up debris. This can be followed by the application of lab grown skin, " said Dr. McLean.

Query: Coding a Surgical Repair of a Lisfranc Fracture Dislocation
 
What are the options to code for a Lisfranc fracture dislocation repair? I fixated the medial cuneiform to the second metatarsal. Additionally, the repair involved the fixation of the first and second metatarsal-cuneiform joints. Would this be an open reduction and internal fixation procedure?

Response: From the information you’ve provided, it doesn’t sound like you did an “open repair”. So, I’m assuming you did closed reduction and then percutaneously fixated multiple joints involving tarsal and metatarsal joints. Therefore, I suggest CPT 28546, percutaneous skeletal fixation of tarsal joint dislocation.

Every child is different, each and every clinical presentation is different, and every parent is different. It’s my personal choice to not deal with a kicking and screaming baby in the office, having both parents hold the child down which does nothing than to raise anxiety for everyone. I would not use phenol in these cases as I feel it is important to deal with the hypertrophic skin. That being said, when presented with similar cases, I take them to the O.R., monitored anesthesia care, Winograds, and closure with Vicryl and Steristrips. A few months later, the toes look perfect.

RE: Recent Changes in ABFAS Recertification

With respect to the recent changes for recertification that have been instituted by ABFAS: My board certification expires in August 2025. I went to the website and it instructed me to take 4 of the 30 question tests prior to the expiration date. I did so and maintained above the required MDT P-value. For all intents and purposes, I do not have to take another test until the end of 2025 as I have met the requirements.
 
The whole point in the change in format was to have us keep up-to-date periodically as opposed to having us study for one test at the end of the certification period. I called to ask if I could continue to take the tests for practice and I asked to make sure they wouldn’t be officially scored. I was shocked to hear that they would have to be scored, meaning the next test or 5th would replace the first one I took. This makes no sense.
 
At a point where I don’t have anything to achieve by taking them, other than testing my own knowledge and keeping up-to-date, why not just designate us in the system as having fulfilled the requirements and now let us take the exams for practice? Isn’t that the whole point, to keep us reviewing and learning on a continual basis? ABFAS is missing the opportunity to allow docs to continue to use the LEAD testing system even after they fulfill their obligations.

I agree with Dr. Sherman wholeheartedly that it is time that one board finally offers both pathways in our profession. Although Dr. Rogers’ gambit with CAQ may not be in the future, it puts the onus on APMA to allow the boards to create a single pathway for medicine and surgery. It is high time to allow ABFAS to create a certification pathway in podiatric foot and ankle medicine and ABPM to create one in podiatric foot and ankle surgery. There are no laws against the boards to compete like this except it may disrupt the "Podiatry Lives Matter" gatekeepers.
