PODIATRISTS IN THE COMMUNITY - PART 1
ND Podiatric Surgeon Joins JRMC Podiatry Team

Jamestown Regional Medical Center (JRMC) welcomes Kayla Emter, DPM, to the podiatry team. Dr. Emter joins JRMC Podiatrist/Foot & Ankle Surgeon, Dr. Rachael Renschler, to exceed patient expectations and provide quality care. This is the first time in JRMC history that the hospital has offered two podiatrists to care for the community.

Dr. Emter received her podiatric medical degree at Des Moines University College of Podiatric Medicine and Surgery in Des Moines, Iowa. Her residency was completed at Covenant Medical Center in Waterloo, Iowa.

PODIATRISTS IN THE COMMUNITY - PART 2
TX Podiatrist Loves Ability to Juggle Healthcare and Having a Family

Graduating with a degree in psychology and a minor in neuroscience, Susie Sant’Anna, DPM came to find her passion in podiatry after spending time in her residency program. “During my program, I met a gal who was a podiatrist and she told me it was great because you have a balance of sports medicine, office patients, and surgeries,” Dr. Sant’Anna said. “This was really appealing for me, as I wanted a great way to juggle working in healthcare and having a family.”

Dr. Susie Sant’Anna

Dr. Sant’Anna moved to Fredericksburg in 2015 and began seeing patients after being in Austin for 20 years. In 2017, she opened and began managing her own independent practice. “I love being in this field because you get to interact with patients and they often see results as soon as they step out of the office,” she said.

ON THE INTERNATIONAL LECTURE CIRCUIT
CA Podiatrist Lectures at Biomechanics Seminar in Helsinki

Dr. Kevin Kirby (US) was invited to lecture along with Dr. Joris De Schepper (Belgium) at a two-day Foot and Lower Extremity Biomechanics Seminar this last week in Helsinki, Finland. The meeting was sponsored by the Finnish Podiatry Association and the Podiatry Department at the Metropolia University of Applied Sciences in Helsinki (one of only two podiatry schools in Finland). 80 podiatrists from Finland and Sweden attended the conference with topics presented on “The Longitudinal Arch Load-Sharing System”, “Tissue Stress Theory”, “Changes in Podiatric Biomechanics Theory over the Past 25 Years”, “Biomechanics and Treatment of Pediatric and Adult Flatfoot Deformity and Running Foot Strike Controversies”.

Included also were workshops on “Gait Examination Techniques” and “Clinical Tests for the Modern Podiatrist”. Finland, with a population of only 5.5 million people, has approximately 400 podiatrists, with over half of them belonging to the Finnish Podiatry Association.

INTERNATIONAL PODIATRY IN THE NEWS
Expert Laments Dearth of Podiatrists in Nigeria

Despite five million Nigerians suffering from diabetes with tens of thousands of such people at risk of diabetic foot ulcers, the country only has two podiatrists, the Project Coordinator of the Gintervenes Health Foundation (GHF), Dr. Andrew Agun, has said. Speaking during a week-long training for nurses on Diabetes Foot Care in Lagos recently, and organized by GHF, he said what was more worrisome is that many Nigerians were increasingly being diagnosed with diabetic foot ulcers with no commensurate expertise to address their issues.

He said, "Podiatry is not taught in any university in Nigeria, so there is a big gap because when people have severe foot issues, the next thing is to take it to orthopaedic surgeons who would most likely amputate it if they can’t manage it." The best people to manage foot issues are podiatrists, and in Nigeria, there are presently only two of them, and they both work with private healthcare facilities in Lagos and Abuja.

PRACTICE MANAGEMENT TIP OF THE DAY
Legal Considerations of Practice Expansions - Part 2

2. Expansion through additional providersMany practices seek ways to expand their revenue without taking on additional debt. This can be a challenge even when patient volume is available since the hours in the day for patient care are limited. Financial expansion of a medical practice can be achieved through the addition and effective use of practice extenders. If you’re thinking about adding mid-levels to your practice, it’s important to consider some important questions:
  • Is the patient volume really there?
  • Is the reimbursement going to cover the expense of the provider?
  • Do you have adequate physicians to provide supervision?
  • Is your patient base willing to see a mid-level?
  • Do you have the space and equipment to add new providers to your practice?

QUERIES (MEDICAL-LEGAL)
Query: Requests to Trim Fingernails

I have recently been receiving an increase in requests from family members of my homebound geriatric house call patients to "trim" their fingernails. Have any of my colleagues been presented with these requests as of late? If so, would they share their thoughts or experience with this?

Maryann Trivlis, DPM  Belle Harbor, NY

Editor's comment: PM News does not provide legal advice. In New York (as well as other states), a podiatrist is limited in scope which does not include the trimming of fingernails. Medical malpractice does not cover treatments which are out of scope. Additionally, cutting fingernails could subject one to professional discipline.

RESPONSES/COMMENTS (OBITUARIES)
RE: The Passing of Paul Scherer, DPM

I'm writing this after reflecting on a call I just had with Dr. Scott Kantro. Scott summed everything up when he said, "In an era when things can be so blatantly transactional, Paul was the opposite." Indeed, that was Paul. He made people better. Perhaps, in honor of Paul, we can all work together to make those around us a little better. Rather than robbing Peter, let's give ourselves to pay Paul and pay it forward.

RESPONSES/COMMENTS (RELEVANT RESEACH)
From: Michael Forman, DPM

Thank you, Dr. Levy, for posting this information. Just off-hand, there could be two reasons for this: 1. Diuretics can have an adverse effect on the diabetic limb(s); or 2. Those who have the need for taking a diuretic already have a condition that would cause these problems.

YOU CAN'T MAKE THESE THINGS UP

RE: Outrageous Shoes of the Day

The Mycelium shoe challenges the usual conventions of footwear, expanding from the parameters of the foot to create a form limited only by physics.

Mycelium shoes (Photo: Kendra Barber)

The form of the network, its direction and variation, are informed by the physical location of the ground and foot: in areas where the shoe contacts the ground, the network is occupied with compact cells for greater wear resistance; in areas where the interior covers the skeletal muscles of the foot, the network is filled with softer, more flexible cells, providing for muscular flexibility to enhance and augment movement within the inner shoe cavity.
