NY Husband & Wife Podiatrists Injured in Boat Crash

The operator of the powerboat that crashed into a marshy island off Seaford on Sunday night killed three people and injured four. The boat was heading home to Copiague from a Freeport restaurant and fish market when he made a navigation error that he was trying to correct before the impact, two close friends said Tuesday.

Nassau University Medical Center upgraded the conditions of three survivors Tuesday from critical to stable condition. The survivors are Sugamele, 50, of Massapequa, who suffered neck injuries; Raymond Soluri, 48, of Massapequa, who had fractured vertebrae in his neck; his wife, Laura, 54, who suffered head injuries; and a man from Farmingdale who is 54 and fractured both legs.

NC Podiatrist-Daughter Honors Dad and Tradition

On the wall in Dr. Ainsley Roberson Rusevlyan’s waiting room hangs a portrait of the local podiatric physician’s late father, Dr. James “Bunk” Roberson. The portrait is one of many tributes to the elder Roberson, a longtime podiatrist who died a little more than a year ago. Also scattered about the waiting room are smaller reminders of Rusevlyan’s father, including a miniature tobacco barn and more items that are symbolic of his other passion: farming.

Though inspired by the late Roberson, the atmosphere of the office is also reflective of the young doctor who is a self-described “Southern lady.” After completing her undergraduate education at East Carolina University, Rusevlyan went on to graduate from Temple University. Before returning to the area, she was chief resident at a veterans hospital in Philadelphia. “I got great training, very hands-on training,” she said. Though she received job offers in Philadelphia, Rusevlyan wished to come back home. “I came back to be with my family, and practice and continue that tradition,” she said.

HEALTHCARE LEGISLATION

Reform Bill Might Require Doctors to Participate in The Public Plan

House Democrats in ongoing caucus meetings are considering additional options for cutting costs in a nearly $1 trillion health reform bill that leadership is pressing to finalize in the coming weeks. A surtax on the nation’s highest earners has been favored as way to offset costs, “and I think that’s the approach the House will follow,” Rep. Gerald Connolly (D-VA) told reporters.

Members of the caucus continued to wrangle with the format of the public plan. Setting reimbursement on negotiated rates would result in an estimated $25 billion in savings over 10 years, but rates set at Medicare plus 5% “would save $110 billion and that’s a big difference,” Connolly said.

Requiring doctors to participate in the public plan as a condition for accepting Medicare patients could potentially make up that difference, saving up to $90 billion over 10 years, offered Rep. Anthony Weiner (D-NY). The trade-off is physicians would know what their rates would be, “and they wouldn’t have to deal with insurance companies,” Weiner said.

Query: Why Are Feet Ticklish?

Recently, I was asked by a patient, "Why are feet so ticklish?" I was embarrassed because I didn't have a knowledgeable answer. What is the physiological explanation? What part of the brain controls this? What is the neurological pathway? When do infants develop the sensation? Why are some people not ticklish? Could ticklishness be used as a neuropathy marker? Is ticklishness or lack thereof used diagnostically? Why can't you tickle yourself? Anybody have answers or a good source?

RE: Scope of Practice to Treat Obesity (Tip Sullivan, DPM)

Are the kids obese because they have flatfeet and they cannot (or chose not to) participate in sports or exercise ? Or do they have flatfeet because they are obese?

The standard line for heel pain was that the patients were obese. In a study by Godling and myself  in The Foot, about 20 years ago, we ran a control group without heel pain and found that their body mass index was statistically the same as the heel pain group.

Conclusion: podiatry patients are generally obese. (Godling was a Fellow from England and added: Americans are generally obese!)

Have your local compounding pharmacist mix it for you in a 50ml vial; it has a shelf-life of one year, and is very cost-effective.

I use 5% ethyl alcohol by volume. I have him withdraw 2.5ml from a 50ml vial of .5% Marcaine with epi. that gives you an end product which will have a consistent concentration rather than mixing it yourself from little glass ampules. I hate cutting myself on those antiquated little things. I keep a couple of them handy, but only for the rare times that I run out of my multi-use vial.

I must point out several important considerations  when one reads an article such as that cited by Menz, et al. which provides a systematic review of treatment outcomes with custom vs pre-fabricated foot orthoses.

Menz, et al. point out the paucity of quality randomized comparative trials comparing the outcomes of custom and over-the-counter foot orthoses in the treatment of common podiatric conditions. For Dr. Turlik to make the conclusion that...

I have been affirmed in two different states, by a CAC or a Medicare foot specialist, that each absent pulse is counted for the same reason: each limb is a "body area" as stated under "E/M Services Guidelines" of the AMA CPT manual.

Dr. Purdy has an amazingly creative definition of socialism, a selective memory when it comes to history and economics, and a horribly bad case of confirmation bias. But he does write authoritatively with lots of passion.

What he is saying is to eliminate Medicare and Medicaid, and deregulate insurers. Then tell people that they need to be responsible citizens and buy insurance if they value their lives. And, I'm sure we need a tax break in there somewhere. And if they don't.... they are screwed.
