On October 1, American Academy of Podiatric Sports Medicine (AAPSM) Fellows were represented at the Illinois Podiatric Medical Association (IPMA) meeting held in Lincolnshire, Illinois. The AAPSM track featured Michael Chin, DPM presenting on neuromas and other causes of forefoot pain, including a novel approach to neuroma surgery which preserves the nerve.

|
Drs. Maggie Fournier, Michael Chin, and Brian Fullem |
Maggie Fournier, DPM spoke about stress fractures in athletes and also discussed relative energy deficiency in sports (RED-S), which has replaced the "female athlete triad" in terms of nomenclature. Brian Fullem, DPM discussed heel pain, including what treatments are effective according to evidence-based medicine, which was highlighted by Cochrane Reviews declaring shockwave therapy as the platinum standard of care for heel pain.

UK Podiatrist Describes Formula for Perfect Heel Height
Choosing the perfect shoe that won't give you blisters all comes down to the shape of your feet. Consultant podiatrist Emma Supple says that some women are built to wear them skyscraper-high, while others will suffer after mere moments in the lowest of heights. To work out which category you fit into, Emma says there's a three-step formula to calculate your perfect heel height (PHH) in a matter of minutes.
|
|
Emma Supple |
According to Supple, the measurement is based on the flexibility of a curved bone that connects the foot and the leg. She told the Daily Mail: “If the talus tilts downwards when you are holding your leg out straight and relaxing your foot, then you have a lot of mobility and can wear high heels with ease.” While this is good news for those with a flexible talus, other women just aren’t cut out for wearing them and there’s nothing they can do about it.

IN Podiatrist Pioneers Surgical Fusion Plate
Arthrosurface® has announced that the new CheckMATE® 3.0 Fusion Plate has been launched for end-stage toe arthritis. One study showed that 35%-60% of the population over 65 years old has osteoarthritis of the big toe, making this a common problem. The CheckMATE® plate was designed to offer advantages over existing systems.
|
|
Dr. Patrick DeHeer |
Dr. Patrick DeHeer, surgeon pioneer for the CheckMATE® plate commented, "The goal was to design a plate that is low-profile, but strong and rigid, providing the best of both internal and external fixation. This could allow patients to bear weight on their forefoot earlier, and to minimize secondary surgery and hardware irritation. This new plate has incorporated all these elements, making it a very attractive option for surgeons who perform first MTP fusions."

AHIP, Blues Push Congress to Lift Ban on Patient Identifier
The leaders of nearly two dozen healthcare industry organizations want Congress to continue to push for lifting an 18-year-old ban that prevents HHS from developing a national patient identifier. The Healthcare Information and Management Systems Society, payers such as America's Health Insurance Plans (AHIP) and the Blue Cross and Blue Shield Association were among the 22 groups to sign a letter sent to the chairs and ranking members of the House Appropriations Committee and its Labor, Health and Human Services, Education and Related Agencies Subcommittees.
The letter asks the committee to “develop a coordinated national strategy that will promote patient safety by accurately identifying patients to their health information.” Providers and other users of electronic health records now often use a technique called probabilistic matching. It matches patients to their electronic records using mathematical algorithms that take basic demographic data in those records, such as first and last names, date of birth and sex, and calculates the probability that the patient's records being queried belong to the patient seeking care. Critics of existing patient-matching techniques allege they add costs and patient safety risks and hinder effective health information exchange.

9 Tips for Surviving after the ICD-10 Grace Period Ends - Part 8
Oct. 1, 2016. This date has likely been highlighted on many a calendar at physician practices around the country. That's because this date marks the end of the one-year grace period from CMS for practices' use of these new diagnostic codes for Medicare fee-for-service claims. In other words, the flexibility on not having to be specific in ICD-10 coding will end on this day:
8. Isolate Frequency of Unspecified Code Assignment
"The first step is to identify the current frequency of unspecific code assignment. For small practices, an overall view of the frequency can be reviewed. For medium-sized practices, each specialty and/or specific practice location should be reviewed and then narrowed down by individual provider. If a high frequency of unspecified codes are identified, each code-set should be reviewed to determine what more-specific codes are available for code assignment.
"For small practices, this can be addressed with the provider(s) immediately. For medium-sized practices, a brief record review should be performed to identify opportunities for more specific code assignment. These results should then be shared with providers to educate them regarding the assignment of more specific codes." - Kat Austin, CPC, director of coding and data standards at American Health Information Management Association (AHIMA)

Query: Cheilectomy and Amniotic Interposition
If a doctor does a cheilectomy and interposes an amniotic membrane into the joint prior to closure, is that a billable fee separately? I thought that the surgical center would bill for the supply and the surgeon really doesn't bill anything except the CPT 28289. Is that correct?
Mike King, DPM, Alpharetta, GA
Response: Correct, you would bill for the cheilectomy procedure. The insertion of the amniotic graft would be incidental to your procedure and would not be separately billable by you.
You or the ASC should pre-certify the graft to see if the insurance company will pay the ASC for that item. Most insurance companies in my area consider that graft material experimental and investigational, and therefore a non-pay item.
