Foot Movement Inside Shoe Studied to Aid Runners: Aussie Podiatrist
 
Runners across the world will benefit from technology developed in Australia that monitors how feet move inside shoes to enable manufacturers to develop lighter and more responsive shoes. The high-tech collaboration between the University of South Australia and ASICS to develop a new, ultra-light ‘super sneaker’ involves 3D motion analysis, laboratory testing, and state-of-the-art treadmill technology with built-in force measurement capabilities. Research fellow Chris Bishop said the research collaboration aims to improve shoe comfort and performance for all runners. "The overall aim of this is to develop and translate the results we gain in the laboratory into a product design,’’ he said.

"The movement of the foot inside the shoe has eluded footwear researchers for some time. From my podiatrist background, we use orthotics, tape, in-shoe wedging, but once we put the foot inside the shoe, it’s a bit of a black box. We have been able to develop capabilities to describe in-shoe foot motion through state-of-the-art computational modeling. This provides insights into foot function during walking and running while wearing shoes, the effect of technical footwear design as well as the mechanisms of action of in-shoe foot orthotics.’’ He said dual density midsoles are thought to improve comfort and reduce injury for people with flatter feet, but they also add weight to the shoe.

Aussie Podiatrist Finds Herself in Role Reversal 
 
In the five weeks since Belmont’s Anj Aitken discovered her sore back was the result of aggressive and incurable cancers, life has changed dramatically. The cruel diagnosis forced the instant weaning of three-month-old daughter Leilah, allowing Aitken’s tumour-riddled spine to be surgically stabilized and an immediate commencement of chemotherapy, hoping to gain precious time with her family.

The caring podiatrist, musician, and devoted mum to three children, is known by those closest to her as someone who always assisted others. As she faces her ultimate fight, it’s time for a role reversal. The shock of Ms Aitken’s situation inspired two fundraising sites and a variety of fundraising events. The family has received unexpected community kindness, with offers of help and donations of goods to auction.

CT Podiatrist Pleads Guilty to $195,000 Medicare Fraud
 
Amira Mantoura, 53, of Greenwich, waived her right to indictment and pleaded guilty Monday in Hartford federal court to one count of making a false statement to the Medicare program. The false claims totaled about $195,000. Deirdre M. Daly, U.S. for the District of Connecticut, said “as a podiatrist, she (Mantoura) was fully aware and understood the procedure to perform a ‘nail avulsion’ and she understood that a nail avulsion was a surgical procedure to treat an ingrown toenail.” Daly said in a release, “Rather than perform a nail avulsion, in most of these instances Mantoura had merely provided her patients with routine foot care, including clipping the patients’ toenails.”

Deadline to Appeal 2016 Medicare Penalty Is Near
 
Physicians and group practices facing a Medicare pay cut in 2016 for unsatisfactory reporting of clinical quality data have until November 9 to request an informal review if they believe they have been unfairly penalized, the Centers for Medicare and Medicaid Services (CMS) announced last week. The penalty, based on performance in the Physician Quality Reporting System (PQRS) in 2014, will reduce fee-for-service payments next year by 2%.
 
Physicians, medical groups, and accountable care organizations can find out whether they are due for the pay cut next year by obtaining a PQRS feedback report for 2014. Information on how to get the report is available on the CMS website. Requests for an informal review must be made online through the agency's Quality Reporting Communication Support Page. Clinicians who ask for a review will receive an email from CMS within 90 days announcing the decision, which is final, the agency said.

PRACTICE MANAGEMENT TIP OF OF THE DAY
Early Lessons of ICD-10 for One Practice - Part 1
 
ICD-10 has arrived without much fanfare at our organization. This is probably because we’ve been dual coding (ICD-9 and ICD-10) for over a year and because it is the lull before claim denials start coming in. There are many lessons to learn from the ICD-10 transition —many, I suspect, will come out over the new few months. However, as we stand on the threshold of this momentous change, I can’t help but reflect on some of the early lessons:
 
1. This is a nail in the coffin for small, independent practices. The amount of complexity and support by both personnel and technology to get our organization transitioned to ICD-10 causes me to wonder how it can possibly be done by a one or two-doc shop. How many more solo practitioners will either transition to direct primary care or simply retire as the federal regulations require compliance with increasingly difficult-to-implement systems?

I was taught that fusing a lesser MPJ would always lead to further pathology. As a result, I have never performed a lesser metatarsal phalangeal joint fusion. Interestingly, I’ve never been told exactly why this would be an issue. There is an article by Michael Tritto, DPM where he describes fusing the second MPJ and obtaining good results in older patients who have subluxed 2nd MPJs. 
 
You have several different options to treat this patient. Determining the exact location of her pain is vital. Be sure she does not have...

Dr. Sloan correctly points out that those before 1994 are "grandfathered in" and do not have to recertify after 10 years. Can he or anyone else explain the fairness in that? When a rule is made, it should apply to everyone or no one. If the point of certification is to point out that one has a certain knowledge base or is more proficient, how is that achieved by giving a certain group of individuals a "free pass"? Grandfathering is nothing more than a scam that should be revoked immediately. 
 
As an afterthought, I assume if they aren't recertifying, they don't have to pay monies either? If so, that's a double joke.

I have had the same issues with ABPS. I passed the written exam, signed up for the certification, but after 7 years of sending in all my cases, I was told that I do not have "variety" necessary to become certified. I did explain to them that I work with a lot of elderly, and to me, their safety and quality of life means more than having variety. As a physician, this is what is expected of us, the well being of our patients.
 
Too bad my board did not agree with this. I was informed to retake the certification test and try again. Only this time, I have 3 years to come up with "variety" of cases. I tried the other board and unfortunately because I did a RPR/PSR 12, I do not qualify. It is an absolute embarrassment to explain to other professionals in medicine what we have to go through. My brother-in-law is a board certified general surgeon and he laughs at me every time I talk about our certification process.
