According to Conway McLean, DPM, “Once again, Fall Prevention Month, occurring every September, has come and gone. Little mention was made of the event in the popular media this year, not indicative of its importance, which is reflected in the statistics. Roughly 36 million falls are reported each year among older adults. About 50 billion dollars are spent annually on the costs of caring for these falls, with 32,000 deaths occurring every year.”

The immobility sometimes required for treatment of a simple broken leg can set in motion a downward spiral of events, from pneumonia to bed sores. A concussion can cause lasting changes in memory and cognition, while a broken hip may lead to permanent confinement to a wheelchair. But we know the factors increasing these risks: we need only raise awareness of the breadth of the problem and the many methods of mitigating the risks,” says Dr. McLean.

I have recently been introduced to services whose purpose is to come in, provide quality educational-training programs, specific to your needs, and train your staff on procedures and patient care. For example, Bridge the Gap Medical Education Services LLC in Texas performs just those services. They are on a mission to “bridge the gap” between what a nurse or tech learns in school, and what they need to “retain employment and provide excellent patient care” in a clinical setting.

It is incredulous to me that these "plans" (I love that Dr. Kesselman won't use the 'A' word. I have felt that way since these plans' inception!) continue to get these huge fines; however, we don't read about their CEOs (who I have no doubt know exactly what their business is doing) getting sanctioned, charged with any crimes, etc. All the while, doctors who play by lesser criminal rules face jail time, fines, and license revocations (of course doctors shouldn't cheat either).   
 
I would like to share something interesting that happened to us: my group also gets nearly weekly requests from these plans requesting records. We have always requested payment ahead of time. Most, after some hemming and hawing, eventually pay and we send records (we charge $30 /record). However, a recent one caught my attention with one particular "plan". After requesting payment for a records request, they were ADAMANTLY against paying, and literally threatened that it was a violation of our contract to withhold the records. They insinuated that they would "report us" if we didn't send in the records (even though they never mentioned to whom).  
 
Luckily, I have many lawyers in my family, and had one write a letter back. We advised them to provide a copy of the contract where they think we had to provide records (our contract with them said no such thing). We also advised them that threatening language toward us wouldn't be accepted, and that if they continued that rhetoric, we would report them to CMS, and the insurance commissioner of our State. Within 7 days, we received a check for $30, times the number of charts requested.

I’ve been thinking about the recent posts in PM News and I recalled two old movie titles and how they seem to fit the current state of affairs in our profession. One movie is called Ad quid venisti and is translated from the dead Latin as “Why are you here?” The other is Quo Vadis, and is translated from the dead Latin language as “Where are you going?” I use the word dead on purpose. The plots of the movies aren’t important, but the titles are.
 
I get confused trying to come up with answers to these two questions and find myself in what Piaget called a state of disequilibrium. I’m not sure what the answers to those two Latin questions are as they apply to podiatry. I wonder if anyone can answer the questions with any...
