PODIATRISTS AND PUBLIC HEALTH
Podiatric Partnership Funds New Students

More than 100 podiatric students will know the benefits of an American Public Health Association (APHA) membership for free, thanks to a partnership between the Podiatric Health Section and the American Podiatric Medical Association. The American Podiatric Medical Association is funding 135 APHA student memberships across nine podiatric medical colleges — 15 from each school — following a resolution passed by the medical association’s board of trustees.

Knowing that fees are often a barrier to student memberships, the Section began a student scholarship fund five years ago that the medical association administered with the help of Section member donations. The groups previously funded about 12 students annually, but this year were able to fund more, said Janet Simon, DPM, the Podiatric Health Section’s Governing Councilor and chair of the medical association’s Public Health Preventative Podiatric Medicine Committee.

PODIATRISTS IN THE COMMUNITY
MA Podiatrist Retires after 50 Years

After 50 years working as a podiatrist in the Dorchester community, Dr. Phillip F. Smith of Ashland recently retired. Smith, 80, came to Boston after receiving his doctorate of podiatric medicine from Illinois College of Podiatric Medicine (now Rosalind Franklin University of Medicine and Science), with his wife and then two children in 1965 to set up his podiatry practice.

Dr. Phillip Smith

As the only African-American podiatrist in New England for 30 years and in Massachusetts during the entire lifetime of his practice, Smith served patients from the Roxbury, Dorchester, and Mattapan areas. He also was committed to serving the community through his staff appointments at six community health centers throughout Boston. In addition to these appointments, Smith was on staff at the Roxbury V.A. Hospital, Brigham and Women’s Hospital, The New England Hospital for Women and Children, and Carney Hospital.

PRACTICE MANAGEMENT TIP OF OF THE DAY

Seven Ways to Beat Telemedicine at Its Own Game - Part 6

6. If you can't work with 'em — beat them.

Drug store clinics are about convenience, period, and perhaps easier access to antibiotics. Have you thought about setting up your own minor-care kiosk at the grocery store or mall? Could you link it to your patient portal and capture patient data trends? A kiosk could be a wonderful patient attraction/retention tool.

QUERIES (MEDICAL-LEGAL)
Query: CPT 99214 and Billing Calculators

A colleague recently expressed dismay that his EMR billing code calculator frequently recommends he use CPT 99214 for an EMR visit, and yet he almost always bills CPT 99213 due to his concern of being "flagged" for an audit. If a doctor is performing all the required "bullets" and documenting this in his EMR, is he underbilling by not using the suggested code?

John M. Hurchik, DPM, Andover, MA

Editor's comment: PM News does not provide legal advice. EMR billing calculators simply suggest the proper code to bill. They are not approved by CMS as a definitive method of coding and would not be defensible in court. Unless a podiatrist is treating patients with very severe and complicated conditions, such as infected diabetic feet, it is very unusual to code CPT 99214. If the patient does meet all the bullet points, then it is certainly appropriate to bill this level of service. Frequent billing of this code will, however, likely trigger an audit as you will be billing beyond the second standard deviation of other podiatric submitters of this code.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A
From: James Shipley, DPM

Dr. Peacock, you're incorrect. Based on current guidelines, candidates no longer get to choose which surgeries they can submit. Surgery cases are now chosen by the ABFAS certifying board members. This process may be totally random or it may not be; candidates aren't informed on how cases are chosen. As for being "rewardingly enjoyable", I have been through the ABFAS certifying process and am board certified, and I'm still waiting for that "rewardingly enjoyable" part to kick in I guess. I'm excited to see what these enjoyable benefits will be :).

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2B
From: Name Withheld

The biggest issue here is that no longer are candidates allowed to select which cases they would like to submit. This change does not affect anyone who is already certified by this board/club. Nowadays, all cases performed have to be logged and the cases are picked for the applicant. This was not the case 5 years ago. I continually see the work of some of these diplomates who were grandfathered in and it is really nothing special.

The cost associated with this podiatry board is exorbitant. There are other boards out there that are much more reasonable and inclusive (MD, DO, DPM). Honestly, anyone who says they don't have cases that didn't turn out exactly as planned is either a liar or doesn't do enough cases.
