Integrated Medicine Alliance (IMA) of Shrewsbury has announced the opening of its new IMA podiatry practice. The practice is led by Dr. Justin Cohen. IMA Podiatry specializes in diagnosing and treating injuries to the foot and ankle. Cohen received his doctorate in podiatric medicine from Temple University’s School of Podiatric Medicine, and completed three years of advanced residency training in podiatric medicine and surgery at Mt. Vernon Hospital and Wyckoff Heights Medical Center.

INTERNATIONAL MEETING NEWS
College of Mexican Orthopedists Hosts MIS Seminar
 
The College of Mexican Orthopedists hosted an international seminar on minimally invasive foot surgery in Cancun Oct. 2-4. There were 160 attendees and lecturers from Europe, South America, and North America. Many of the professors have written some of the most authoritative publications on MIS foot surgery and have built some of the largest surgical foot practices in the world.

The course consisted of the latest MIS procedures for forefoot, rearfoot, and ankle surgery. These procedures, which began in the U.S. by the AAFAS, have proliferated throughout the world. The worldwide network of orthopedists and podiatrists have led to the rapid development of new procedures. There were numerous studies and comparisons to traditional surgery. Emphasis was put on avoiding complications and how to deal with them when they happen. The next MIS event will be in Chicago, Oct 24-25, with an intensive cadaver seminar.

WI Podiatrist Charged with Medicare Fraud
 
A Grafton podiatrist has been accused by federal authorities of defrauding government health insurance programs by submitting false diagnoses for patients, according to a document filed in federal court. Dr. Alan Balkansky, who runs the Healthy Foot and Ankle and Grafton Podiatry Group, violated the federal False Claims Act, according to a civil complaint filed by the U.S. Department of Justice in Federal Court in Milwaukee late last week. Balkansky has charged for podiatry services that were not provided and for routine foot care to patients who did not meet the medical criteria to be eligible for coverage. He also failed to properly document treatment and charged for physical therapy services that were provided by massage therapists, the complaint says.

First, you did not mention if your patient has a positive Mulder's and/or Sullivan's sign. While not pathopneumonic, and especially if both are clinically apparent, then she likely has a Morton's neuroma. In end stages of neuroma formation, when scar tissue engulfs the inter-metatarsal/digital nerve, it is typically very unlikely that normal sensorium can be restored to the respective digits. In this case, because of your patient's expectations, I would probably order an MRI to confirm whether a true neuroma is present. If confirmed, then I'd shift my focus to educate your patient that her expectation for normal sensory return is unrealistic given that existent nerve damage, and you may create post-op neuritic pain where none existed pre-operatively, irrespective of technique utilized.
 
While I have had success utilizing the KobyGard to relieve inter-metatarsal/digital neurotic pain, normal sensorium to toes 3 and 4 was not restored in any case where a true neuroma was clinically apparent pre-operatively.

RESPONSES/COMMENTS (CLINICAL) - PART 1B

This patient should have an MRI with contrast to determine and confirm a Morton's neuroma and/or mono-neuritis. The size of the neuroma is of clinical importance to confirming interdigital treatment options. If this patient clearly is against the use of a standard sclerosing agent, my recommendation is a percutaneous interspace release with KobyGard (Osteomed device).
 
From a personal case perspective, I have done hundreds of releases with this type of presentation with good to excellent clinical outcomes. The caveat is, if you have a large Morton's neuroma, it will not work effectively. My other suggestion is radio-frequency ablation. I have performed less than two dozen cases and it has proven to have good to excellent clinical outcomes. Please refer to the evidence-based literature on these two particular options. Proper technique of these two procedures is paramount for good results.

Mr. Root responds to criticism of the affiliation that the APMA has with PFOLA. If his response was meant to assuage podiatrists, I believe he missed his mark. 
 
He's correct in asserting that there has been a push by the profession towards surgery of the foot and ankle. Some states require a 3-year program for licensure. But I'll bet the actual percentage of podiatrists performing ankle surgery is minuscule. And judging by the Medicare data published, individual podiatrist fees not...
