Talar Medical Funds $50,000 Commitment to APMA Scholarship Endowment
Talar Medical has successfully fulfilled its commitment to the American Podiatric Medical Association (APMA) Educational Foundation by completing the $50,000 donation it has committed to their scholarship endowment. This contribution will have a lasting impact on the future of podiatric medicine, supporting up-and-coming podiatric medical students year after year.

Amol Saxena, DPM, MPH, was the keynote speaker for the British Columbia Podiatry Association (BCPMA) meeting, the first live gathering since 2019. He presented on five topics including podiatry, bias, and possible solutions on Canada's "Day of Truth & Reconciliation".

The talk discusses how biases affect DPMs similar to people of color, particularly in the U.S. Dr. Saxena provided some options the profession can pursue, including an MD degree and unifying the boards.

5 Training Tips for Your Practice Staff
It is imperative to offer a few different things to ensure your staff is operating at the highest level possible, regardless of if they are licensed or not. It is imperative because ensuring they are trained well decreases liability issues and increases their value to your clinic.
4. Cross-train for many roles.
It's important to cross-train the entire staff to perform all of the roles in the office. Understanding that there may be some items that are simply not appropriate for an unlicensed person to perform (this does not apply to an office operating with strictly “med techs”; in that case there is no reason for the entire staff not to be trained to perform in the clinical roles), develop an abbreviated protocol for the front office to check off on for the clinical side as well.

Query: Old Fracture Fragment
My patient is an 18 year old, athletic female with chronic pain over her talonavicular joint. Her radiographs appear to show an old avulsion injury and a residual dorsal navicular fracture fragment. My surgical plan to address this pathology is to resect the fracture fragment as it is too small to attempt fixation. Any thoughts on a CPT code for this scenario would be appreciated. I am considering CPT 28104.

Response: In this scenario, I would consider:
CPT 28122: Partial excision (craterization, saucerization, sequestrectomy, or diaphysectomy) bone (e.g., osteomyelitis or bossing); tarsal or metatarsal bone, except talus or calcaneus.
It seems to best describe the surgery that was performed. When I read the definition of CPT 28104, it describes addressing a pathology that internalized in the bone. CPT 28122 seems to be a more appropriate code as it seems to address a pathology that is on the periphery. The scenario of a non-united fracture fragment fits this code best.

Well another one bites the dust. This is just another one of those MCR Part C (Medicare Replacement, I don't dare say the word Advantage) who continue to skirt the rules and make money off the taxpayers. But the patients are going to continue to seek out these plans since their mighty pocket book tells them to do so and Congress really wants patients off the FFS MCR roles so the Feds can shift risk to the private sector.
There is a way to fight back, but doctors clearly are inept to do so and will continue to either retire from clinical practice, become indentured servants to large corporations, or the few smart ones will go fee-for-service and not accept any insurance.
When will the medical profession wake up? If your neighborhood UPS driver can get a base salary of $~175K + benefits and thus avoid striking, I don't think it's too much to ask for doctors to obtain a raise once a decade (at least)!

Maybe next time you get a request for charts from Cigna for your patients with Medicare Advantage plans, you’ll think twice about sending them your charts without requesting payment first. Cigna will use the information extracted from your patients' charts to collect millions from Medicare. I would advise you not to ignore these chart requests, but rather send an invoice to Cigna demanding a fee per chart. Before retiring, my fee was $50 per chart. Suffice it to say, they never sent payment. Do not send charts without requesting payment. You work too hard for your money to let these insurance companies collect more from Medicare than you do.

I feel your pain Dr. Seuferling. My recommendation is to run as far away from HealthNet as possible! HealthNet secured the VA contract here in my state some years ago when there was a huge outcry for the VA patients not being able to get adequate care. Many providers signed on, got referrals, and started seeing these VA patients in their private practices. After I got denial on payment for the first 6-10 patients I saw, I stopped taking them. This went on for a year or two, with some mental health providers who saw a lot of VA patients being stiffed tens of thousands of dollars. To my knowledge, HealthNet paid very few of those claims and eventually lost the government contract. Sadly, both the providers and many vets lost out as they couldn't get care during that time. I did a TMA on a diabetic patient who had that "insurance", then had to fight them for 2 years to try to get paid, and HealthNet refused. I never got paid.
HealthNet must have some great lobbyists and friends in high places on Capitol Hill to still be in business ripping off doctors with these different insurance contracts. It is criminal! I recommend banding together with all the other docs in your area and simultaneously dropping HealthNet. This is why we need a union to stand together against these behemoth insurance companies. But alas, we just keep talking about it with no action. Why is that?
