MD Podiatrist Discusses How to Prevent Blisters in Fall Boots
 
“One of the most likely causes of blister formation from fall boots is improper fit. Motion can lead to blisters,” said Saylee Tulpule, DPM, a podiatrist at Foot and Ankle Specialists of the Mid-Atlantic. To avoid this, she recommends getting fitted at the store (where you can walk around in them before buying) and making sure your boots are laced tight enough (if they have laces).

“As podiatrists, we recommend avoiding cotton socks, since these can retain moisture and cause friction blisters,” Dr. Tulpule said. She recommends sticking with moisture-wicking materials like wool or with synthetic fibers like nylon and X-Static. If your feet are prone to sweating, she recommends using absorbent powders.

Look into OnyFix, a non-surgical treatment where an acrylic band is attached to the base of the nail and it trains the nail to grow out straight. No anesthesia required.

In treating several cases in the very young human population, some as young as 10 days old, I found that the most important issue is the care that the mother does. I have noted that many times the parents were told by their pediatric doctor that they will grow out of the problem. Conservative treatment such as mild soaks during the child's daily bath and mild massaging of the paronchyia, and applying Vasoline several times a day can sometimes decompress the ingrown edge.
 
In these very young cases, I had a pediatric anesthetist involved when doing even a simple edge removal. In all cases, they healed well after, with twice daily soaks and topical creams, and  if  infected, antibiotic  topically. They never needed oral ABXs. In these cases, educating the parents is the most important need. I would be hesitant to do a partial nail removal in this age group.

Board certification, in this day and age, is a requirement to maintain insurance contracts and hospital privileges in almost all cases. Unlike days past, when certification was a badge of honor and optional, today, not becoming board certified can mean the end of a physician's practice. Like any political world, our profession is intertwined among our state, the APMA, and multiple certification boards. To challenge this, especially on a state society level, is a political hot potato. Even individuals appear to be fearful in using their names in posting commentary. The APMA, through the HOD, in conjunction with the CPME, gives the green light to the boards of their choosing. It then follows that states will transfer this decision to their individual licensing boards, and therefore the acceptance of hospitals and insurance companies.
 
Knowing the severity of not becoming certified should make one question the fairness and oversight in the administration of such a certification board. One should know that the ABFAS (American Board of Foot and Ankle Surgery) and the ABPM (American Board of Podiatric Medicine) have "self-certified," and do not currently have any standardized third-party accreditation or ....

The official statement should be "See your sports podiatrist to evaluate if your running shoes can last you more than 200 miles." As podiatrists we are the experts in shoes. The 300/500 miles for shoes is outdated info. A study showing 40% collapse in the heel cushion at 200 miles is more indicative of what we should look for. I change my running shoes every 200 miles. My race shoes, I take a hard look at just over 100 miles.
 
The lighter, faster, and more high tech the shoe, the faster it usually will break down. This is usually related to the ability to create a lightweight, energy return-type foam. This new material cannot hold up as long and should not be compared to the heavy materials of the past. Of course, weight, stride, wear pattern, run surface, and general biomechanics all need to be considered.

Several years ago, I was given the honor at an APMA national meeting to be one of the judges of the posters that were displayed. I spent many hours reading every poster. Most were really great and showed that podiatry has the ability to merge science with medical practice. I was really proud of my profession and those that did the research. Unfortunately, there were few, if any, posters that dealt with biomechanics and foot orthotics.
 
Dr. Kesselman is correct. There is waning interest in that part of podiatry and it probably is a Catch-22. On one hand, it is not as lucrative as surgery and on the other hand because we are not doing the research, we don't have the ability to approach insurance companies and insist that they make it lucrative. Dr. Kesselman, I, and others can complain until the cows come home about this but we seem to be beating a dead horse. It is what it is.

The knowledge of biomechanics and its application in our evaluation and management of podiatric pathology particularly with regard to elective foot and ankle surgery is essential. Unfortunately, current understanding of the relationships between the forefoot and rearfoot are sometimes like visiting the Wizard of Oz. Does equinus cause pronation or does medial column instability result in Achilles contracture in an attempt to stabilize the medial column during mid-stance? The proper function of the peroneus longus and posterior tibial tendon may be more important than obsessing over the amount of eversion of the calcaneus which is very often neutral to slightly inverted. 
 
The efficiency of the 1st MPJ and stability of the medial column described by Shavelson as the “kickstand of the foot” is a very interesting commentary on where to focus your attention when you are performing surgery or prescribing orthotics. This fresh perspective, I believe, may serve as a simple way to view the foot/ankle biomechanically and to improve our surgical decision-making; thereby making outcomes more predictable and successful.
