PODIATRISTS AND PUBLIC HEALTH
2nd Post Doctoral Public Health Fellow Receives MPH

The APMA PHPPM Committee along with the Podiatric Health Section of APHA have announced that this year’s post doctoral podiatric public health fellow, Dr. Kyle Bruce, has completed this year's fellowship program culminating in his receiving the Master in Public Health degree from the Geisel School of Medicine at Dartmouth College.

(L- R) Drs. Kyle Bruce and Dyane Tower

Dr. Bruce follows Dr. Dyane Tower, the first Dartmouth public health fellow who is now an assistant professor at Dartmouth Medical School and clinician in the Department of Orthopedic Surgery at DHMC. Bruce, who prior to graduating the fellowship completed his three year residency at Henry Ford Hospital in Michigan, has accepted a position at River Bend Medical Group, a group practice in western Massachusetts.

PODIATRISTS IN THE COMMUNITY
NJ Podiatrist Goes from Foot Racer to Foot Doctor

In December 2002, Red Bank Regional High School senior, Katy Trotter, had the best showing in Shore Conference history when she finished second overall at the National High School Cross Country Championships in San Diego. Now, Katy Trotter, DPM, one of the most accomplished high school runners to ever come out of New Jersey, has returned to the East Coast to join the practice of renowned sports podiatrist John Connors, DPM.

(L- R) Drs. Katy Trotter and John Connors

After completing her college degree, Trotter decided that she wanted to study medicine and attended the Dr. William M. Scholl College of Podiatric Medicine in Chicago. “I have always been interested in medicine but was not committed to going to medical school for the majority of my college career,” said Trotter. “I had exposure to podiatry growing up in Little Silver with Dr. Connors, but it was not until after I graduated from Stanford that I realized podiatric medicine was the career for me.”

ICD-10 UPDATE
Just When You Thought it was Safe, ICD-11 Looms for 2018

At Security Health Plan, a 230,000-member payer owned by Marshfield Clinic in Wisconsin, the impact of ICD-10 won't be felt immediately. Security received its first claims files the evening of Oct. 1 for services provided earlier that day, said Sara Foemmel, the plan's claims operations director. Few hiccups are expected. “We've tested a tremendous amount with all of the providers,” Foemmel said.

But in 2018 comes ICD-11. Foemmel said her colleagues have just discussed the new codes, which are in beta testing now. “We just want to get through ICD-10 in the first couple of months, and then we'll get a game plan for ICD-11 once more information is released,” Foemmel said.

PRACTICE MANAGEMENT TIP OF OF THE DAY

Seven Ways to Beat Telemedicine at Its Own Game - Part 5

5. If you can't beat 'em — work with them.

If you choose not to offer extended or weekend hours, forge a relationship with your favorite urgent-care center and make sure your patients know about it. Make sure the urgent-care center sends you notes on your patients; that's your ROI for this relationship, along with increasing odds that your patients won't wander to another provider.

QUERIES (CLINICAL)
Query: Nail Striping

I have recently seen this 60-year-old Caucasian male in my office for complaints of toenail fungus. He does have toenail fungus on some other toenails but this particular toenail has an unusual pink stripe which appears to blanch minimally with pressure. There has been no injury to the toenail, but the patient is worried about the possibility of a mass in this area.

Nail Striping

No drainage has come from this area, either purulent or serosanguineous. The patient is concerned, but does not want to have his toenail avulsed or biopsied needlessly. Any ideas of what this might be?

RESPONSES/COMMENTS (CLINICAL) - PART 1A
From: Fay Mushlin

We have outfitted 3 office locations with podiatry chairs from Hill Laboratories Company in Frazer, PA. They are able to customize a chair to fit your individual needs. Hill is a family owned business since 1945 and are very focused on customer service. Howard Hill is the owner and his niece Robin are both there to help you with any questions. They have a very comprehensive website.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1B
From: Judith Rubin, DPM, Steven J. Kaniadakis, DPM

After seeking one for myself, I spoke to a rep from Midmark and he said Boyd chairs are a quarter of the price and substantial. If you are looking for a long-term purchase, you may want a Midmark bariatric chair that can hold up to 550 pounds.

Judith Rubin, DPM, Cypress, TX

Simple. Get the Midmark brand.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2
From: Don Peacock, DPM

The ABFAS is a certifying board that has specific requirements for certification, and these rules are clearly outlined on their website. As an ABFAS diplomate, I know firsthand of the process. It is tough and rewardingly enjoyable. I had lots of cases to submit and the candidate can submit the cases he chooses to submit in accordance with the guidelines. From this list, 10 are chosen for further review. The guidelines are very clearly spelled out, and the ABFAS has loads of info to help the candidate submit cases properly.

Your complaints are unfounded and untrue. You have PSR24+ training and meet all requirements for certification. So what you are saying is that you volunteered to get in the ring and you got knocked down. So, get up and train for the next match. That's what a champion does. If you continue to do surgery, it will not be the last time you get knocked down.

You should feel honored to have a pathway to certification as I had. Remember, many of our colleagues have no pathway to certification by the ABFAS. One of the best foot surgeons in the world lives here in NC and he failed the surgical board his first time through. Quit bellyaching and resubmit. You are not the only one that has had to redo this process. Get it right and you will be certified by the ABFAS.

Don Peacock, DPM, Whiteville, NC

YOU CAN'T MAKE THESE THINGS UP
RE: Paint Thinner for Onychomycosis?

In the stress of ICD-10, we must try to keep laughing, and our patients sometimes help. Recently, a patient on oral antifungal for his toenails presented in my office. When asked if he had been taking his medicine, he replied," I take one of those "turpentines" (terbinafine) every day. I guess that must be the generic!
