Collagen Grafts Can Help Promote Wound Healing: TUSPM Podiatrist

Doctors who specialize in wound management are growing skin to help people save their limbs and extremities. Pharmaceutical and biotech companies are extracting collagen -- a protein that makes up 75 percent of skin -- from donated skin and creating grafts, or patches, that can induce a patient's own skin to grow. The donated skin can come from a variety of sources: anything from a pig's pancreas to a baby's foreskin.

"In some cases, we can get four football fields of skin out of one baby foreskin," said Dr. James McGuire, head of wound management at the Foot and Ankle Institute at the School of Podiatric Medicine at Temple University, in Philadelphia. "If taken care of, skin can grow and grow."

The idea behind these collagen grafts is to create a sort of scaffolding into the wound, upon which a patient's cells can attach and grow. "Once you take all the cells out of there and strip it down to the collagen level, the body says, 'This is great; this is what we make our bodies out of,'" McGuire said. "And tissues will grow around it."

COMPLIANCE UPDATE

FTC Further Delays Red-Flag Rule Enforcement

The Federal Trade Commission delayed for the fourth time enforcement of a rule that requires financial institutions and creditors—including doctors and hospitals—to address the risk of identify theft. Regulators postponed enforcement by another six months until June 1, 2010, at the request of members of Congress, the FTC announced on Oct. 30, two days before the so-called "red flag" rules were to be enforced.

Under the rule, doctors and hospitals must develop and implement written programs to identify, detect and respond to potential identify theft. The healthcare providers accept deferred payment and are therefore covered under the rule, according to the FTC.

AL Podiatrist Appointed to State Podiatry Board

Governor Bob Riley has appointed Troy physician Sanjeev Singh to the State of Alabama Board of Podiatry. Singh was sworn in as a member of the prestigious board Tuesday by Pike County Probate Judge Wes Allen at the Pike County Courthouse.

“Gov. Bob Riley made a great choice in appointing Dr. Sanjeev Singh to the Alabama State Board of Podiatry,” Allen said. “This is a very important board and it is good for Pike County to have a local physician appointed to serve. We congratulate Dr. Singh. He is very deserving.”

Singh said he was honored to have been appointed by the governor and is looking forward to serving on the state board. Singh has practiced podiatry in Troy for 17 years and, laughingly, said he is now one of the “old men” in the medical business in Troy.

DIABETIC RESEARCH

Foot Ulcers Independently Linked to Mortality in Diabetics 

A history of foot ulcers is a significant predictor of mortality in diabetics, independent from comorbidities such as cardiovascular disease and nephropathy, according to study findings. Lead author Marjolein M. Iversen, from Bergen University College, Norway, states, "The results underline the importance of organizing future healthcare services with follow-up routines that allow for early and tight clinical monitoring in persons with a history of foot ulcer in primary care."

The new findings are based on data from 1,339 diabetic patients with a history of foot ulcers, 155 diabetic non-foot-ulcer subjects, and 63,632 non-diabetic participants in the Nord-Trondelag Health Study in Norway. Subjects were followed for 10 years for mortality. The mortality rate for diabetics with a history of foot ulcers was 49.0%, much higher than the 35.2% and 10.5% rates seen in the diabetics without a foot ulcer history and the non-diabetics, respectively.

After adjusting for demographic and lifestyle factors, diabetics with a history of foot ulcers were 2.29 times more likely to die during follow-up than were non-diabetic subjects. Among diabetic subjects, foot ulcer history was associated with a 38% increased risk of death.

Query: Orthotic For Hallux Limitus

I’m inquiring about recommendations for orthotics for a 13 year old 6'3" basketball player. Generalized complaints: Flexible, collapsing  with gorilloid navicular bone (no accessory bone). Maximum eversion with stance/gait and forefoot supination. He has limited 1st MPJ ROM, bilateral, and secondary adaptive dorsiflexion of the IPJ and a plantar keratosis. I’m wondering about how to support/control or accommodate the hallux limitus.

I see two problems here that are above and beyond your concern whether or not to remove the tightropes that are "in place." I submit to you that one or two of the tightropes have already broken (which is probably why her deformity has returned), and that makes your question regarding their removal moot. If I am not mistaken, I believe sagittal plane hypermobility is a direct contraindication for this procedure as the first metatarsal excursion will be direct cause for the tightrope to break. My other concern is that this patient has had "TWO midshaft" tightropes. This procedure is normally performed in the proximal 1/3 of the bone as it is thicker and safer for fixation.

The other complication of this procedure is the stress riser that is created when you drill through a bone, especially the second as it is locked proximally between the two cuneiforms. Not only is the 2nd metatarsal a thin bone, it is even "thinner" mid-shaft, and this patient has had two holes drilled. Her problem is not necessarily the recurrence of her bunion, but the probable metatarsal fracture she may sustain. As far as I am concerned, hypermobility = Lapidus.

RE: Sarapin for Morton's Neuromas? (David Alper, DPM)

Dr. Alper brings up an interesting question when he cites a neurologist whose answer to the question of whether Sarapin is effective was: "it has worked for my patients." It might seem to readers that the use of Sarapin in the treatment of neuromas might not hold its weight in the era of evidence-based medicine.
 
When I lecture at podiatry seminars on evidence-based medicine, the question often arises as to whether we can use a medical or surgical treatment that appears to be efficacious, but is devoid of supporting research papers. This does not mean that there is negative literature on the treatment, but simply there is little or no literature. I explain to attendees that we cannot discount our own clinical findings nor should we always relegate positive findings to the realm of placebo. Good medical research is extremely costly and small companies cannot afford to spend millions of dollars on good research. If a drug is generic or if we are dealing with a surgical procedure not involving the use of patented hardware, there may be no money out there to fund the research, even if the treatment appears to work. 
 
Treatments such as posterior tibial blocks, as well as other time-honored treatments, including off-label uses of medications, are not reimbursed by third-party payers because of a paucity of supportive literature. One of the goals of the American Society of Podiatric Medicine is to encourage our colleges to take on research projects for  podiatric treatments when there is no corporate interest willing or able to foot the bill.

I am sorry to have to report that Dr. Alan Schor passed away after a long illness on Sunday, Oct. 25, 2009. He was a 1959 graduate of Temple University School of Podiatry. He did his surgical residency at Civic Hospital in Detroit, MI in 1959-60, and went into private practice in N. Bergen, NJ, where he retired in 1992. He was extremely active with the New Jersey Podiatry Society. He was a past president of the ACFAS and was also a diplomate of the American Board of Podiatric Surgery. He was residing in the Scottsdale, Arizona area when he passed away. His activities on behalf of podiatric medicine were invaluable during his lifetime. He will be greatly missed by his family, friends, and the podiatric profession.

Podiatry will receive a gradually phased-in 6% overall fee increase during the multi-year transition period proposed by CMS. The exact amount your practice will generate will depend on your procedure mix. Increases in EM visits and simple office-based surgery comprise most of this increase. Fees for hospital-based and more complex surgery will generally decline. This increase is largely dependent on the extensive practice expense survey conducted by the American Medical Association and generously supported by your APMA dues dollars. I suspect this increase by itself will more than pay for joining APMA. There is no other podiatric organization capable of providing this input to AMA.
