NY Podiatrist Provides Footwear Advice for Pregnant Women
Back pain is a common pregnancy symptom, and it doesn't always disappear when your baby arrives. Drop in to any physical therapist's office or massage studio and you're bound to see some baby bumps in the waiting room. That's because ligament-loosening hormones, weight gain, and a shifting center of gravity all conspire to cause 2 million pregnant women to cry out from back pain every year, especially between the fifth and seventh months.

Supportive sneakers or flat boots are best for everyday use, but if you want some height, try rubber-soled wedges, which distribute body weight across a larger surface area and offer shock absorption. You'll also want to ditch your ballet flats and flip-flops, says New York City podiatrist Jacqueline Sutera, DPM. Instead, wear flats with an arch support or try insoles or orthotics.

| SUCCESS TIPS FROM THE MASTERS | |
Bret Ribotsky: When evaluating venous disease in a leg with a veinous ulceration, how do you image to find the pathology?
|
|
Dr. Jodi Schoenhaus |
Jodi Schoenhaus: You must know the anatomy and understand how to use an ultrasound to image vessels. Visualization is different than looking at muscular skeletal structures. Then you look at, for example, the great saphenous vein. The normal diameter is 4.2mm, but if it's larger, then it's an indication of reflux.
Meet the Masters is broadcast each Tuesday night at 9 PM (EST). This week's guest is to be announced. You can register for future events by clicking here.

Vermont Gets Preliminary OK To Move Toward All-Payer System
Vermont has received tentative approval from the Obama administration to establish an all-payer reimbursement system for healthcare providers in the state starting in January. Maryland long has had an all-payer system, but it covers only hospitals. Vermont plans to use an accountable care organization-type structure that would cover all providers. All-payer systems require all insurers, whether private, Medicare, or Medicaid to pay similar rates for services.
Under the state's all-payer proposal, which is modeled on Medicare ACOs, providers would be paid global rates determined by the patient populations and health outcomes. Fee-for-service payments would be scrapped. Some experts say more states should consider such a system to speed the transformation to value-based payment.

9 Tips for Surviving after the ICD-10 Grace Period Ends- Part - 5
Oct. 1, 2016. This date has likely been highlighted on many a calendar at physician practices around the country. That's because this date marks the end of the one-year grace period from CMS for practices' use of these new diagnostic codes for Medicare fee-for-service claims. In other words, the flexibility on not having to be specific in ICD-10 coding will end on this day:
5. Focus on the Most Relevant Codes
"Practices should take a look at what changes are specific to the conditions they treat. Also, in the move toward Advance Payment Models, the data received from claim forms could factor into future payments, so we want to make sure we use the codes to the best of our abilities." - Rhonda Buckholtz, CPC

| CODINGLINE CORNER | |
Query: Heel Fissures?
We commonly see complaints of heel fissures. Isn't a "heel fissure" by definition an ulcer, whether it be partial or full thickness, and therefore eligible for a debridement code?
Jennifer, Office of Charles Perry, DPM, Cambridge, OH
Response: If you are treating an open wound (e.g., full thickness) beneath cracked and bleeding heel fissures, then CPT 97597 would likely be your code. The more common scenario, however, is a presentation of hyperkeratotic tissue (cracked skin around the heels) without an open wound. There is no debriding of dermis; there is debridement or paring of hyperkeratotic tissue. If that is the case, CPT 11055 is your likely code. And, yes, that is a routine foot care code...and, yes, most payers will not reimburse that code without "at risk" findings present.
