Indian Podiatrist Uses Novel PMMA Foot and Ankle Prosthesis
The Department of Podiatric Surgery at the Amrita Institute of Medical Sciences successfully replaced the destroyed foot and ankle bones of a 30-year-old patient with a novel prosthesis using polymethyl methacrylate (PMMA). The surgery was led by Dr. Ajit Kumar Varma. The prosthesis was affixed through a surgical technique now known as the Amrita Sling Technique.

Dr. Ajit Kumar Varma |
“The patient developed a spinal cord injury due to a fall from a height. This caused severe neurological deficit of his lower limbs. After some time, muscle strength of the lower limbs improved and he started walking. But when the bones of the right foot and ankle started bearing weight, they developed Charcot’s destruction as they were osteoporotic. We took three-dimensional color CT images of the patient’s normal foot and ankle on the left side. By creating a mirror image of these pictures, we got an idea about how to repair the damaged right foot and ankle bones," said Dr. Varma.

NJ Podiatrist and Wife Own and Operate Medical Spa
Dr. Nicola Delorio became familiar with collagen during her medical residency, and injectible skin treatments were a growing part of her plastic surgery practice starting in Linwood in 1998. So Delorio's clientele grew, and a decade later, she and her husband, Dr. Gerald Galinus, opened Accent Aesthetics, Medical & Laser Day Spa in Galloway Township.

Lasers are used for a variety of treatments, from tightening skin, to erasing fine lines, to removing irregularities, to spider veins and acne scarring from skin. The spa also offers laser hair removal. "We have two lasers for that, one for darker skin and one for lighter skin," Delorio said. Galinus, a podiatrist who runs the business side of Accent Aesthetics, also performs some of the laser treatments.

TN Podiatrist Publishes Diabetes Prevention Book
For more than 30 years, Dr. Mark Hinkes has seen his patients with diabetes suffer from pain, ulcers, and infections, just because they didn’t follow sound foot healthcare guidelines. His new book, Healthy Feet for People with Diabetes, focuses on prevention.

Dr. Mark Hinkes and New Book |
The comprehensive guide to foot care has easy-to-follow steps for those most at risk of having foot problems. “While it’s a great guide for patients, it has also been well received by podiatry office staff, foot care nurses, and even pedicurists,“ Dr. Hinkes says. Hinkes is Chief of Podiatry and Director of Podiatric Medical Education at the Veterans Affairs Medical Center/Tennessee Valley Healthcare System in Nashville, Tennessee.

AZ Podiatrist Uses Technology to Help Heal Diabetic Wounds
With up to 12,000 patient visits annually, the UA's Southern Arizona Limb Salvage Alliance clinic at The University of Arizona Medical Center – University Campus is one of the busiest in the world for diabetic limb salvage. In the five years that Dr. David G. Armstrong has been director of SALSA, a collaborative clinical and research alliance dedicated to advancing care of the diabetic foot and preventing amputations in North America and worldwide, he has honed his keen propensity towards technology-based solutions in treating and connecting with patients.

“There is a massive trend of merging consumer electronics with cybernetic appliances,” says Armstrong. SALSA’s collaborative research team, the Interdisciplinary Consortium on Advanced Motion Performance, works on developing and tweaking gadgets that enhance quality of life. The team, known as iCAMP, is headed by biomedical engineer Bijan Najafi, with whom Armstrong works closely.

| PRACTICE MANAGEMENT TIP OF THE DAY | |
Bad Body Language That Hurts Your Career
Avoid these actions that can make you come across as insecure, incompetent or indifferent—all behaviors that can limit your career success:
-
Slouching. When you enter a room, the people around you take about four seconds to make up their minds about you. Stand tall, with square shoulders and your head upright to convey confidence—especially when you meet people for the first time.
-
Touching your face. That can often lead people to deem you as insecure or untrustworthy. Additionally, touching your lips can convey that you don’t agree with the other person.
-
Chewing your pen or eyeglass stems. That can make you look nervous or unconfident about the status of the negotiations.
-
Using a weak tone. While not a gesture, tonality does fall under the scope of body language. If you tend to raise your tone at the end of sentences—the way you do when you ask a question—you will have trouble convincing others to accept your point of view. If you are trying to influence someone, make sure your tone drops down at the end of sentences.

| QUERIES (CLINICAL) | |
Query: Acral Melanocytic Nevus
This patient is a 57 year old Afro-American female who presented to the office with two unilateral "purple"lesions of approximately 2 months duration. She stated that they began just as noticeable "brownish" lesions and changed color to "purple."
|
|
Acral Melanocytic Nevus |
Two 2 mm biopsies were taken from the lesion marked by the arrow. Path report came back acral melanocytic nevus, compound type. She is diabetic and on dialysis. Is any further treatment or diagnosis other than observation needed since there are multiple lesions? Would dermoscopy be of value? Any opinions would be appreciated.

| RESPONSES/COMMENTS (CLINICAL) | |
From: Robert Colligan, DPM
Many years ago, I removed a calcified bony prominence from a distal phalanx, with a radiographic appearance much like this one. And strangely enough, several years later, I came across another patient with the same thing. Both reports came back as benign parosteal osteochondromatous proliferation (BPOP). Very little has been written on the subject, but I do recall there was an article written on the subject in JAPMA (2006). This incidentally was given a slightly different name: Bizarre parosteal osteochondromatous proliferation.
In both instances that I witnessed, the lesion was removed, and has not yet recurred.

| RESPONSES/COMMENTS (MEDICAL LEGAL) | |
From: Hal Ornstein, DPM
Whenever you mail something important to a patient, you should always send it return receipt - signature requested. Be sure to document that this was done, and place a copy of the receipt in the patient's medical records.

| RESPONSES/COMMENTS (NON-CLINICAL) - PART 1 | |
From: Michael M. Rosenblatt, DPM
This suggests that, at a minimum, it is NOT easy to decrypt an iPhone. Also, there are some excellent encryption apps for sale. I submit that the forces of HIPAA are less to worry about than the forces of an NH chart audit. I do agree that it is vital to purchase a good encryption program for your cell phone, and even look into ones that large businesses use, even if they cost more. If you are going to use your cell phone to document NH visits, you should definitely encrypt it. But you probably should do that anyway, especially if you have banking info on it, as many of us do. There are websites to help you with this.
This is a temporary technology issue that will eventually be fixed, if not already. If you are concerned about smartphones, don't use them, but instead use a non-Internet connecting device, like an ordinary digital camera. Dr. Giaimo suggests an ordinary HD webcam without internal memory. Just please don't use the "encryption" excuse not to do what you need to do to protect yourself. Technical arguments will not keep you out of jail. Good documentation will.

| RESPONSES/COMMENTS (OBITUARUIES) - PART 1 | |
I met Dr. Petrolla in 1988 when I visited his office for the first time as a new rep when I started working for Pedinol. He was so gracious and generous with his time to speak with me. I felt I was talking to an old friend! The last time I was in his office, he was showing me videos of his grandson Luca. I will miss seeing his warm smile. I am grateful for the wonderful people I have met along the way in this profession, and Angelo was sure at the top! My condolences to Heather and his family.

| RESPONSES/COMMENTS (OBITUARIES) - PART 2 | |
I would like to add my sympathy and respects to the Petrolla family for their loss. I was a resident at the "Foot Clinics of Youngstown" along with Angelo, and my fondest recollections of him were his broad smile and relaxed, confident attitude. He was pleasant to be around, professional, and bright. Youngstown was noted for the phenomenal lunches given to the residents, and Angelo wasn't second place to anyone in enjoying them. I only recently heard he was ill, and his passing was very sad to me. Heather, his beautiful wife, would visit the clinic occasionally, and we all looked forward to seeing her. I am sorry for their loss and will pray for him.

RE: One Way to Reduce Post-op Drug Costs
Truth can be stranger than fiction. Today, I saw a 30-year-old patient who is status-post two weeks bunion surgery. She is breast-feeding, and today her husband informed me that in between applications of topical pain medication, she is putting breast milk on her wound. I told him, "why not. I can't imagine it's bad for the wound."
