NY Podiatrist Discusses Why Running Shoes Need Replacement After 300-500 Miles
 
Is the 300 to 500 mile rule for replacing running shoes founded on research? Do you really need to replace your shoes that often? Yes, says podiatrist Bruce Pinker, DPM, who works with walkers and runners at Progressive Foot Care in New York. “The midsole, found between the insole and the outsole, breaks down and wears out in this range,” he explains.

He says that the midsole of sneakers is usually composed of ethylene-vinyl acetate (EVA) or polyethylene-vinyl acetate (PEVA), and the material serves as a shock absorber. However, the rebound effectiveness decreases over time. “Running or walking without this shock-absorbing material fully intact could possibly lead to injuries,” cautions Dr. Pinker. That could mean stress fractures, shin splints, plantar fasciitis, or runner’s knee.

I have read many of the responses to ABPM’s recent plans for a CAQ in surgery and many are unbelievably frustrating. The current process ABFAS has for certification seems unnecessarily drawn out, expensive, and tedious. I know most of my colleagues feel the same way. If we are truly concerned about patient safety, as many of the responses seem to indicate, why are we not ensuring that recent residency graduates are “up to snuff” before letting them perform surgery unsupervised for 6-7 years? 
 
Why am I allowed to perform ankle surgery for 6 years without supervision, building my surgical skills and techniques along the way with that experience, and then at the end of that period if I don’t submit the right cases or want to pay more fees, I am all of a sudden deemed no longer qualified or unworthy to do more surgery? How does that make sense? Either our training received in school and residency is sufficient or it is not. Either a surgeon feels comfortable to do a surgery or they don’t (and if they don’t then they shouldn’t do that surgery regardless of the training they have received). We should have whatever necessary testing done at the time of residency graduation and not continue the charade for years on end. I applaud ABPM’s effort to provide a simpler and more streamlined process for achieving surgical “worthiness”. Please continue your efforts ABPM!

I have been involved at all levels of training in podiatry. Starting with full-time teaching at TUSPM, residency training for 47 years, committee work for ABFAS on all examination levels. I was part of the team that developed CBPS, and remain on the Case Review Committee for my final year. I was also chairman of my state board in Connecticut for 40 years. I am part of the residency training program at Yale and was Section chief of podiatry in the department of orthopedics, at Yale School of Medicine 1997-2017 until we became a department at Yale New Haven Hospital with a full time department chair. I say all this to allow you to understand my perspective on this critical issue in podiatry.
 
I have read Drs. Oloff, Jacobs (both friends) and Dr. Ford (well respected) on this issue and all make salient points. I would like to add to the discussion with some other critical observations. #1. It is the licensing examination that sets the “minimal competency level...

Robert Phillips, DPM recently highlighted the importance of performing biomechanical exams in the September Podiatry Management Magazine. I would like to suggest some additional resources. There are two recent texts that I think are helpful for the student or seasoned practitioner. The first is Practical Biomechanics by Richard Blake, DPM volume 1 and recently published volume 2. The second is Pathmechanics of Foot Disorders by Doug Richie, DPM. There are also  book reviews of both on YouTube in the podcast Fit Foot U. Rich Bouche, DPM also has a great synopsis of provocative biomechanical tests which are available in the archives AAPSM in his lectures and on YouTube on Fit Foot U.
