LA Podiatrist Discusses Ingrown Toenails
 
How can you tell if an ingrown toenail is infected? "What you would look for is a red or swollen nail border. Usually, you will see some drainage, and it will be painful to the touch and in footgear," says podiatrist Dr. Mallory M. Przybylski.

"One way to prevent ingrown toenails is widening the footgear. A lot of times we will see children with ingrown toe nails because their shoes are too tight. Their feet grow but they forget to tell mom that they feel pain. If you can avoid tight socks or footgear, you can sometimes avoid this problem," says Przybylski.

CA Podiatrist Publishes Analysis of Wound Care Credentialing Organizations
 
In an effort to shed light on the many facets of certification for today's wound care professionals, Kenneth B. Rehm, DPM, Diplomate of the American Board of Multiple Specialties in Podiatry, has authored an article titled "Certified in Wound Care. What Does It Really Mean?" This article apprears in the current issue of Today's Wound Clinic magazine.

Rehm's article offers a glimpse into the diverse certification organizations that currently offer credentials in wound care. These organizations, Rehm notes, are not a "one-size-fits-all" solution. ICD-10 UPDATE
ICD-10 Has Been Like Y2K, So Far 
 
Some 32 years after work began on developing the International Classification of Diseases 10th Revision of diagnostic and procedural codes, most providers, many with fingers crossed, jumped off the ICD-10 pier at midnight Thursday. And by most accounts, the first 12 hours have gone off without a hitch.
 
Health information technology experts predict that most large-size providers will swim right along in a sea of far more numerous, complex and very specific new codes. The organizations most likely to have trouble, however, are smaller providers and health plans, which for several reasons lagged in implementing IT upgrades and training to handle the more robust ICD-10 codes.

Query: Routine Foot Care & E/M Coding
 
I have been in practice 30 years and have always been under the impression that if a patient on Medicare returns back to your office for scheduled periodic covered palliative foot care (i.e., trimming of nails and calluses) that, unless there is a new problem, all you can bill for is the palliative care services.  A podiatrist in my area told me that he bills an E/M code at the same time as CPT 11719 and CPT 11055 codes, even if there is no new problem because he checks the pulses and reviews medications the patient happens to be on. Have I been missing something all these years?

Response: You are correct! In my opinion, your colleague is asking for trouble. Most LCDs used to say that billing an E/M service on the same visit as performing a routine foot care service had to be for a different diagnosis. The E/M could not be used to re-qualify the patient for routine foot care coverage as that was built into the routine foot care coding and payment. 
 
Even though that statement no longer appears in most LCDs, I doubt that the MACs have changed their computer tracking of those services. There are more than a few DPMs who have recently gotten into trouble with the ZPICs, OIG, and/or the FBI for doing what your colleague suggests. To be "forewarned is to be forearmed" is an old English proverb that is certainly as true for this scenario as anything that came up in jolly old England. Except perhaps, the punishments are a little more civil today.

So, here we are... hours into the ICD-10 transition. The sky has not fallen, but it might still. "The proof will be in the pudding," as they say. Whether we get paid on these new codes will be up to the insurance companies that we bill. We will see what happens over the next few weeks and months. 
 
The warnings that we may not get paid in a timely fashion may still occur; so be prepared for some "expected" financial shortfalls in your revenue stream during this transition. It is a good idea to identify any available...

I had this same situation nearly 30 years ago. I have been board certified by the American Board of Lower Extremity Surgery (ABLES) since 1988. I find that all insurers and the majority of hospitals accept this board. I suggest that you contact ABLES, as they are more concerned about people as opposed to payment.

When I came back home, it was made very clear to me by the orthopedic powers that be that hospitals were for orthopedic surgeons to do foot surgery, not podiatrists. Consequently, I, like other Ontario podiatrists before me who were trained in foot surgery, set up my office so that...
