Custom Orthotics Often More Effective Than Pre-fabs: NY Podiatrist
Samantha Landau, DPM, MPH, an instructor of orthopedics and pediatrics at the New York College of Podiatric Medicine in New York City, compares standing for long hours to tallying up the mileage on your car. “As we age, we all add miles to the tires of our lower extremities, so to speak. Our ligaments tend to loosen and our foot mechanics aren’t as optimal as they once were.” As a result, the more you stand, the more likely you are to have foot pain.

Shoe inserts, like insoles and heel pads, take pressure off the painful areas in your feet and ankles. This makes them a great investment if you’ve been diagnosed with foot abnormalities, like bunions, hammertoes, or a Morton’s neuroma. Dr. Landau says that custom orthotics are often more effective than those you can purchase at the pharmacy or drug store. Custom orthotics are prescribed by a foot doctor and are designed to support the unique shape and curvature of your feet. Insurance often covers the cost.

According to Ralph Wentz, DPM, "Stress fractures of the foot are becoming more common in runners, especially first-time marathoners. Often, first-time marathoners enter a race with little or improper long-distance training. The lack of experience coupled with the repetitive impact on the feet can produce enough stress to cause hairline breaks in the bones of the foot."

UK Professor Recognized for Services to Podiatry Profession
A Staffordshire University Professor has received a top honor recognizing his contribution to the advancements of the podiatry profession. Professor Nachiappan Chockalingam, Director of the Centre for Biomechanics and Rehabilitation Technologies, received an Honorary Fellowship from the Royal College of Podiatry at an event in the House of Lords.

He has supervised and trained numerous podiatrists through the University’s post-graduate provision.

Vimazi Adds 2 US Podiatrists to Medical Advisory Board

Hospitals are Dropping Medicare Advantage Left and Right
Medicare Advantage provides health coverage to more than half of the nation's seniors, but a growing number of hospitals and health systems nationwide are pushing back and dropping the private plans altogether. Among the most commonly cited reasons are excessive prior authorization denial rates and slow payments from insurers. Some systems have noted that most MA carriers have faced allegations of billing fraud from the federal government and are being probed by lawmakers over their high denial rates.
"It's become a game of delay, deny and not pay,'' Chris Van Gorder, president and CEO of San Diego-based Scripps Health, told Becker's. "Providers are going to have to get out of full-risk capitation because it just doesn't work — we're the bottom of the food chain, and the food chain is not being fed." Van Gorder said the health system is facing a loss of $75 million this year on the MA contracts, which will end Dec. 31 for patients covered by UnitedHealthcare, Anthem Blue Cross, Blue Shield of California, Centene's Health Net and a few more smaller carriers.

5 Training Tips for Your Practice Staff
It is imperative to offer a few different things to ensure your staff is operating at the highest level possible, regardless of if they are licensed or not. It is imperative because ensuring they are trained well decreases liability issues and increases their value to your clinic.
1. Develop a training plan. Don’t just throw your new staff to the wolves! Instead, develop a training protocol and follow it. Have a new employee check off on all of the skills necessary to function in their role. Most licensing boards are fairly lenient on what a practitioner can do “with the proper training.”

RE: CMS Withdraws LCD Policies that Dramatically Restrict Access to CTPs

The recent stay of execution on the cellular tissue products (CTP) LCD is temporary at best. If the LCD does not get revamped and rewritten, it will simply be removed (as it has in other MACs) and provide no safety net to providers. Then within a few months, they will institute Targeted Probe and Educate (TPE) audits as well as ramping up the post-payment audits from numerous other carriers, the latter of which has already started. Either way, the final word on this has not been written, but certainly in no way is this a victory for providers or patients.
The carriers have shot their cannon across the bow because of the abuse of these products. It is that simple and they will do what they need to in order to get their money back. I have seen lots of audits on this and most providers, despite having good intentions, have poor documentation and have to refund huge sums to the carriers. This is all for a mere 6% profit. Is this really worth it?
The regulations which can choke even the most dedicated physicians are a loss for patients as well. This is a temporary win for the CTP industry who will sell no matter what until the well (which it will) runs dry.

RE: Podiatry: The Sky is Not Falling

I have read many posts on PM News discussing podiatrists abandoning routine foot care. I wonder if there is any truth to this. Many years ago, CMS set up a webpage that I believe still exists, where you can look up the exact Medicare billing codes used and their frequency by any podiatrist in the country (and any medical doctor also, for that matter). This was discussed at that time on PM News, and the point was that despite all our advances in surgery, the majority of podiatrists make the majority of their income from routine care codes. Has that actually changed, or are we just discussing rumors?
I have read many posts on PM News discussing forming a union and acting jointly to change reimbursements. Actually, podiatrists in many states already belong to a union through their state podiatry association, OPIEU, and have for many years. The ability to act jointly is not...

Dr. Harvey's comments comparing podiatrists and orthotics to orthodontists and braces made me chuckle, a bit. Some of the dental procedures are definitely being encroached upon. Last night, I saw an ad on television advertising at-home corrections for missing teeth. The company sends you a kit. You take the impression and they make you something you can add to your own mouth without spending thousands of dollars on dental fees. I wonder if dentists seeing those ads feel the same way that we do when we see shoe stores and non-podiatric professionals making orthotics.
