PRESENT Residency Education Summit Midwest 2018 Residency Rumble Report
 
Attendees from 34 podiatric resident programs from 25 states and 3 countries met outside of Chicago to participate in the 16th Residency Education Summit. The team of Marie William, DPM Aventura Hospital and Medical Center won the podiatric residency education hat trick, taking first place for the third year in a row in the Residency Rumble academic game show.  Her very impressive group of residents joined second place winner Coleen Napolitano, DPM’s DVA-Hines/Loyola and third place winner Jarrod Shapiro, DPM’s Chino Valley Medical Center.

A submission by the team from DVA – Hines/Loyola comprising Katerina Grigoropoulos, DPM, Matthew Garoufalis, DPM, and Rodney Stuck, DPM entitled, “Manipulating Wound Geometry and Skin Tension to Accelerate Healing” took first place in this year’s Abstract Poster Competition. A Faculty Development Workshop took place, led by noted podiatric educators Suhad Hadi, DPM, Marie Williams, DPM, Javier Lafontaine, DPM and Jarrod Shapiro, DPM. Next year’s Residency Education Summit Midwest will take place at the Hilton Oak Brook Hills outside of Chicago on September 20-22, 2019.

RELEVANT RESEARCH
HBOT Appears to Improve DFUs: Study  
 
Background and Aims: Several treatment modalities and protocols for ischemic foot ulcers are available. However, little consensus exists on optimal treatment. The aim of this study was to compare Standard Wound Care (SWC) alone vs. SWC with adjunct hyperbaric oxygen therapy (HBOT) in the treatment of ischemic diabetic foot ulcers (DFUs).
 
Patients and Methods: Twenty-six patients with Type 2 diabetes mellitus (T2DM) presenting with a newly diagnosed ischemic foot ulcer were included. These were divided into group A (SWC with adjunct HBOT) and group B (SWC only). Participants were followed every week for 4 weeks and their ulcers were measured for their surface area and depth to assess any change in wound size.
 
Results: Both treatment arms succeeded in reducing ulcer area and depth (p<0.001). However, ulcer area (p<0.001) and depth (p<0.001) exhibited superior improvement in group A.
 
Conclusion: Adjunctive HBOT appears to improve wound healing in ischemic DFUs and merits further study.

Doctors and Tax Fraud Schemes - Part 3

Here is a list of the IRS’s own “2018 Dirty Dozen” list of most prevalent tax scams. 

5. Fake charities: Groups masquerading as charitable organizations solicit donations from unsuspecting contributors. Be wary of charities with names similar to familiar or nationally-known organizations and take a few extra minutes to ensure your money is going to legitimate charities. The IRS has tools to check out the status of charitable organizations. 
 
6. Inflated refund claims: Taxpayers should take note of anyone promising inflated tax refunds. Those preparers who ask clients to sign a blank return, promise a big refund before looking at taxpayer records, or charge fees based on a percentage of the refund are probably up to no good. To find victims, fraudsters may use flyers, phony storefronts, or word of mouth via community groups where trust is high.

Query: Surgical Assistant Coding
 
We do not have residents or fellows at our main hospital. In the state of Texas, we are not allowed to have PAs or NPs as well to assist. At times, when I have a large reconstruction or trauma case, my partner will scrub in to assist in the case. Are there specific coding techniques for him to submit for assisting on the case, specifically in Texas? If so how would we go about getting this submitted to the insurance?

Response: Use the CPT for the procedure, then add modifier “-80” or “-81” to the procedure code used. The fee you submit for the assistant surgeon is yours or the assistant surgeon decision.
 
-80 Assistant Surgeon: This modifier pertains to physicians' services only. A physician’s surgical assistant services may be identified by adding the modifier -80 to the usual procedure code. This modifier describes an assistant surgeon providing full assistance to the primary surgeon, and is not intended for use by non-physician providers.
-81  Assistant Surgeon: This modifier pertains to physicians' services only. Minimal surgical assistance may be identified by adding the modifier -81 to the usual procedure code, and describes an assistant surgeon providing minimal assistance to the primary surgeon. This modifier is not intended for use by non-physician providers.
 
Describe in the operative note what the assistant surgeon did during the procedure. I recommend advising the patient of the additional fees (although many doctors do not) I think it is a courtesy to explain the fees to the patient.

My heart is deeply saddened. His loss is most certainly a loss for our entire profession. I used to call him "the bio-magician guru". After all these years, he always remembered my name and spoke to me when I would see him at meetings! He was one of the great teachers that I was lucky enough to learn from and was one of the first who tried to get me to think like a bio-magician in the OR. My sincere condolences to his family and close friends.

Query: Problem Obtaining Health Insurance
 
I was wondering if anyone is having the following problem in obtaining personal health insurance. My wife (who works for the practice full-time) and I were considered a group and were able to get health insurance at the group rate. Now I have to apply as an individual and with 7 children being covered; we are looking at about $2,300 per month with a $13,000 deductible. This is totally unaffordable. Any comments?
