PRACTICE MANAGEMENT TIP OF OF THE DAY

Seven Ways to Beat Telemedicine at Its Own Game - Part 4

4. Get proactive.

A payer is paying another doctor (possibly in another state and time zone) to care for your patients via telemedicine (while you are taking evening/weekend calls and getting paid nothing). Why not approach the payer and ask to provide your own telemedicine coverage at the same price? This is one time the continuity-of-care argument actually may be compelling.

RESPONSES/COMMENTS (ICD-10) - PART 1A
From: Peggy S. Tresky, MA

APMA provides a number of ICD-10 resources to its members at apma.org/icd10 to help prepare them for the transition to ICD-10 scheduled for October 1, 2015. One tool that APMA offers is the APMA Coding Resource Center (CRC), a fully integrated online coding and reimbursement resource for foot, ankle, and leg-relevant CPT, ICD-9-CM and ICD-10-CM (Volume 1, 2), HCPCS Level II codes, and Medicare LCDs CCI edits, and fee schedules.  A portion of your subscription helps support the APMA Educational Foundation Student Scholarship Fund.

In the past month, APMA has added to the CRC an interactive ICD-10 sample diagnosis section of a super bill containing 140 typical diagnoses and conditions for a foot and ankle specialist. In addition, we announced the presence of ICD-10 “shortcuts” that have been added to our ICD-10 Quick List of 1,400 commonly used diagnoses, conditions, circumstances, signs, and symptoms. This “ICD-10 Shortcut” feature opens a window for the ICD-10 code you choose, and shows you the “nth” degree (highest level of specificity choices) along with notes, instructions, and guidelines.

Last night, September 29, APMA hosted another in its series of ICD-10 webinars for members. This webinar was an ICD-10 “Town Hall” presented by Harry Goldsmith, DPM, and members of the APMA Coding Committee. Our ICD-10 InSight Webinars (more than 20 in total) are available for APMA members at no charge at apma.org/icd10InSight. In addition, take advantage of the current trove of ICD-10 resources that can be found at apma.org/ICD10, including ICD-10 “Test Your Knowledge” questions, a list of ICD-10 FAQs, and links to other related resources such as those provided by CMS.

RESPONSES/COMMENTS (ICD-10) - PART 1A
From: John Moglia, DPM

There is a free one-hour overview webinar at practicefusion.com on code conversion as part of their superbill creation. You can also download free icd9-10 conversion software from icd10america.com. My manager and I compiled a list of our most commonly used codes and created our own handbook of codes to be entered into our billing software, as Medisoft's list was not comprehensive.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
RE: The ABFAS Shark Tank

I feel like a little fish, swimming in the podiatry ocean waiting to be swallowed by the big sharks. I am writing this letter out of frustration not only for me, but for many young podiatrists. The unfairness of the rules and regulations make it impossible for the new podiatrists to make it. It’s enough that we are not respected by other specialties, but ABFAS makes it nearly impossible for new practitioners to be board certified to earn a living and be part of hospitals and insurance companies.

I recently had to go through the ABPS certification process and passed all written requirements, but my cases got rejected by some unknown reviewers who made silly comments about minutia not relevant to the treatment of patients. With all 10 cases having no complications and with satisfied patients, I find this outrageous to accept. With a PSR24+, I am at the mercy of this only board that I can take. It seems that ABFAS fails many new doctors initially for financial gains, since every time you fail, you pay more than $1,000.

The guidelines for case submissions are poorly defined. Doctors who cover hospital emergencies are obligated to log these cases, but because they never see those patients after, as they disappear, they get penalized for not providing a final resolution or x-rays. We are the only profession that has a board for forefoot and rear foot; orthopedics has one board for the whole body. We are killing our profession.

RESPONSES/COMMENTS (NEWS STORIES)

It was a pleasure to read that a professional soccer player came from England to America for radio frequency ablation for his chronic heel pain. I have performed many of these procedures with a high level of success and patient satisfaction. I was disappointed to find out that the largest insurance carrier in New Jersey will not reimburse for the procedure. They deem the procedure experimental and investigational despite the numerous published studies that show its success rate.

It is disheartening to know that I cannot offer such a beneficial procedure to my patients due to the lack of reimbursement. I am sure an out-of-pocket expense for a professional soccer player is a non-issue, but for working class people, an out-of-pocket expense is a big issue.
