Open Payments Website Launches
The federal government today launched a highly anticipated website detailing at least $3.5 billion in financial ties in a five-month period between medical device and drug companies and physicians and teaching hospitals. The Open Payments website is the first public repository of national data describing financial relationships between industry and healthcare providers. Critics of these payments say these payments can inappropriately influence clinical decisions.
The records are organized into three categories: general payments, research payments and physician ownership and investments. Manufacturers made 4.4 million payments to 546,000 physicians and 1,360 teaching hospitals from Aug. 1, 2013 to Dec. 31, 2013. To see how much a physician received during this period, click here. More records will be published for this period at a later date.

PA Podiatrist Named Residency Director at Heritage Valley Beaver Hospital
Dr. Kimberlee Hobizal has been named director of the podiatry residency program at Heritage Valley Beaver Hospital. In addition to serving as program director, Hobizal will have a clinical practice with the Association of Specialty Physicians Orthopedic and Sports Medicine, which is located across from the hospital.

Hobizal received her undergraduate degree from Baylor University and a master’s in health care administration and doctorate in podiatric medicine from Des Moines University. She completed her podiatry surgical residency at UPMC where she also completed a fellowship.

AAN Warns Against Opioids in Chronic Non-Cancer Pain
The risks of opioids far outweigh their benefits in chronic pain conditions such as headache, fibromyalgia, and lower back pain, according to a new policy statement from the American Academy of Neurology. The statement lists several best practices -- including checking a prescription data monitoring program (PDMP) before prescribing opioids -- and calls for primary care doctors to refer chronic pain patients to specialists if they are taking daily doses of 80 to 120 mg morphine-equivalent per day.
Gary Franklin, MD, of the University of Washington in Seattle, is the sole author of the statement, published in Neurology. "The evidence of harm is high, and the effectiveness is low," Franklin told MedPage Today, calling attention to a graphic in the statement that shows the imbalance. "That is the whole story." AAN is the first national medical association to adopt a policy statement regarding opioids. Franklin's home state has adopted dosing guidelines, which call for doctors to refer to pain management specialists when daily doses hit 120 mg morphine-equivalent -- a policy that the CDC has also endorsed.

Hospitals Ask Patients to Pay Upfront
Hospitals are increasingly asking patients to pay for procedures either upfront or before they are discharged. That's because Americans are shouldering a greater portion of their healthcare bills, and medical centers don't want to get stuck with patients that can't pay. Traditionally, neither patients nor providers knew the exact price of procedures until after the insurer processed the charges. Now, however, new technology is allowing hospitals to determine the patient's responsibility in advance of treatment.
Starting the cost conversation early is especially important now because patients are facing higher deductibles and larger payments for services. Some are surprised to find out that they have to fork over thousands of dollars before their insurance even kicks in, hospital administrators said. The policies available on the Obamacare exchanges are hastening this trend. Many enrollees are opting for the bronze and silver plans, which often carry deductibles upwards of $5,000 and $2,000, respectively.

Query: Are MRIs Needed Prior to Nerve Decompression?
A patient presents with what seems like "typical neuroma pain." The podiatrist treats the symptoms via injection therapy. The condition does not improve. The podiatrist is considering decompressing the nerve by transection of the deep transverse metatarsal ligament. My question is: would it be substandard care to proceed without an MRI? Or is it within the realm of standard of care to proceed with the treatment?

First of all, I make no representation as to treating carpal tunnel, I suggest you seek competent medical evaluation. That said, I found that changing to double-action nail nippers is much easier on my hands, less impact if you will. I also have found that new nippers have been a better investment than sharpening old nippers when they get dull.

Yes, I have symptoms. 15 years ago, I experienced a mild finger tingling and frank arm numbess which woke me during sleep, so I wore custom forearm /hand splints at night B/L for 4-5 years. I then got a mouth guard for grinding teeth in my sleep and the carpel tunnel symptoms ceased within a week. My dentist states that this is a a common benefit for healthcare carpel tunnel sufferers.
